1 Practical Placement Handbook This handbook applies to all courses that require students to complete practical placement hours. This guide outlines all requirements, expectations and procedures pertaining to practical placement. It should also answer any questions that you as a student may have.

3 P a g e 2 What is Placement and Why is it so Important? Placement is workplace based learning which gives you the opportunity to apply your knowledge in an appropriate industry based setting. This opportunity is generously offered by workplaces under supervised/mentor conditions, and is designed to relate to the current training program you are undertaking. Placement is allocated to you by the Placement Coordinator. This is a compulsory component of your program, and once you have been allocated a position within a workplace, you will be required to accept this allocation. You are expected to attend on days that have been designated to you and record the hours that you have been present at placement. This will be checked weekly by your trainer and reported back to the Placement Coordinator. Please note that you will be monitored as you are completing your hours, and that FYI Training will be liaising with Centre and Facility Directors, Mentors and Supervisors throughout the completion of your placement You may have a question or experience an issue whilst you are attending your placement. Please ensure that all inquiries are directed to the Placement Coordinator. Practical Placement Coordinator details: Victoria Gyors Ph: placement@fyitraining.com.au Do I Need to Complete Placement? Yes, placement is not only a valuable resource and a wealth of information when it comes to assessments and general education; it is also a compulsory part of your course set by the AQTF. Each unit of competency (accept for First Aid) requires you to complete a nominal amount of hours in a workplace based setting. The amount of hours you are required to complete will be written on the course information flyer you received upon enrolment. Formal Checks For placement you will be required to get either a Police Check, Working with Children s Check, or both depending on the course you are completing. Courses requiring Police Checks: Certificate III in Aged Care Certificate IV in Aged Care Certificate III in Home and Community Care Certificate III in Health Support Services Certificate III in Health Services Assistant Certificate IV in Leisure and Health Courses requiring Working with Children s Check Certificate III in Early Childhood Education and Care Diploma of Early Childhood Education and Care Courses requiring both: Certificate IV in Disability

4 P a g e 3 Forms You Need to Know Practical Placement Agreement This form must be taken with you when you first go to your induction and meet the Director at your placement host. You must have it filled out and signed by them and yourself. If you are under 18 years of age then a parent or guardian must sign it as well. Once this is all completed it must be returned to the Practical Placement Coordinator prior to the start of your placement. As stated, the practical placement agreement sets out the responsibilities of FYI Training staff, employers and students for programs delivered by FYI Training. Definitions of terms within the Agreement: Practical Placement: Any structured workplace learning that is part of a written agreement between a training organisation and an employer/organisation. It includes work observation, work experience and other forms of workplace learning. Placement host: Workplace providing practical placement activity Workplace Supervisor: Employee or owner of host organisation identified as contact person for student and FYI Training. Practical Placement Agreement: Written agreement signed by student, placement host and FYI Training to specify hours of placement and activate insurance cover. You need to read through the agreement to ensure that you understand the rights, responsibilities and obligations, of all parties involved. Once this form has been correctly filled out and returned to the Placement Coordinator, you will be able to start placement. If you are still unsure about anything you can direct your inquiry to the Placement Coordinator. Page 2: This section must be filled out by your placement host You need to complete your personal details as both FYI Training and your placement host will have a copy of this agreement.

5 P a g e 4 Page 3: This entire page needs to be completed prior to you returning your agreement to the office. FYI Training needs to know exactly what days and times you will be attending placement, and on what date this will commence. This document covers you for Workcover, so if an accident occurs on placement and you are injured but it is not stated on the agreement, then you will not be covered. Page 4: We will sign this once you have returned it to the office. Above this signature section is the rights, responsibilities and obligations of FYI Training. This is what we are agreeing to and what you can expect of us. Make sure you read through this.

6 P a g e 5 Page 5: This is the section that your placement host must sign. Just like the previous section, above where they sign it lists what their rights, responsibilities and expectations are. They must sign it for it to be accepted when you return it. You need to sign this; if you are under 18 then a parent or legal guardian needs to sign it. Again, ensure you read through and understand what you are agreeing to. Page 6: This entire page needs to be completed. This is the medical consent form which identifies who should be contacted and what should be done in case of an emergency or injury. It notifies both FYI Training and the placement host as to whether there are any medical conditions, injuries, allergies or medications that may impact you while out on placement.

7 P a g e 6 Log Book Your log book is the document which is supplied to us as evidence that you have satisfactorily completed this component of your program. This document will need to be taken to every placement visit and signed off by the Supervisor. Depending on what program you are completing, your log book will vary as they are individually designed to suit each course and their specific needs and outcomes. During your first week in class, you will be supplied with a log book and the FYI Training Practical Placement Coordinator will come to explain how this is used. For more information regarding log books, please refer to page 7. Incident Report This form has been implemented to ensure that any incidents that occur whilst attending placement is reported back to FYI Training in writing. This form has been designed to ensure that the student attending placement has the opportunity to submit formal documentation in regards to any incidents that may occur. The incident will be investigated, and an outcome will be decided upon by the Placement Coordinator and Training Manager. The action taken and its outcome will also be documented and this will be fed back to the student, reporting the incident. This provides us, the opportunity to identify any recurring issues and to assist us with the continuous improvement of the work placement program. FYI will work to act on behalf of the student to represent their concerns to the service and where possible, confidentiality will be maintained. We envisage that this process will be used to improve the quality of the experiences of our students on placement. FYI Training has a duty of care to our students and under no circumstances will place them in an unsafe work environment. This form is a communicative tool between the student and FYI Training and we advise that you do not approach service managers on your own accord. You will need to complete these two sections to the best of your ability. Include as many details as you possibly can as this will assist FYI Training to understand the situation and what occurred.

8 P a g e 7 You also need to complete this third section. We would really like to hear if you have a solution or an idea as to ways of preventing the incident from happening again. These final two sections will be completed by either the practical Placement Coordinator or the training manager once you have returned it to the office. Log Books and Journals As stated previously, your log book is extremely important as it is documented evidence that you have completed your placement hours. You need to take it with you every time you attend placement, because it is required to be signed off by your placement supervisor. Throughout the duration of your course either your trainer or the Placement Coordinator will be required to cite your log book each week, and record the hours that have completed. This is a compulsory monitoring process which is required to be conducted by both your trainer and the Placement Coordinator. Your trainer will conduct two observations, whilst you are attending your placement. Please note that these observations are required as an observational assessment tool, and units will not be deemed as competent, unless the observation has been completed. The first observation will occur when all of your assessments have been handed in and deemed competent for the first cluster of your program. This date will be negotiated between the trainer and yourself. The trainer will then advise the service of their planned visit. The second visit will occur when you have completed all of the assessments for the second cluster, and they have been deemed competent. If you have been granted a reduction in hours, then your trainer will organize your observations differently depending on how many hours you are required to do. You will be notified prior to when your trainer will be coming out to observe you so it is very important that you attend placement that day. It is your responsibility to ensure that you keep your log book in a safe place. Loss of this documentation will require the student to complete those hours again or you will need to organize your supervisor to complete a statutory declaration stating that you have completed the amount that was filled out on your log book. The Supervisor can only complete this statutory declaration if they have some evidence of your time in the service. The best way to monitor this is to sign in the visitor s book, every time you attend the service. Without this evidence, Supervisors cannot state the hours you have completed your placement. The time you arrive and

9 P a g e 8 leave the service must be recorded factually. Your shift requires you to complete 7.5 hours. Leaving your placement early will not be tolerated. Once you have completed your log book, it must be returned to FYI Training as evidence of completion of this component for your program. The journal component is only mandatory for the students completing the Certificate III in Children s Services, but it is highly recommended that all students complete one as they can be very useful for classroom based assessments. The journal should reflect the theory that you are learning in class and how it applies to working in the industry. Discuss what you are experiencing, the connections that you have made with children/clients/residents, and relate these experiences to what you are doing to the knowledge and discussions being covered in class. Do not discuss the routine of the center or facility as this will be very similar across the board and your trainer will already know it. You need to complete a journal entry for every day that you attend placement, and these are also cited by your trainer. Also seek comments from your supervisor as it will give you good feedback on what you are doing well and what you might need improvement in. Once you have completed your placement component, you will need to hand in your journal along with your logbook to the office. When Contacting Your Placement Once you have received your allocated placement, you are required to contact them immediately and introduce yourself and touch base. Services are advised that you will be contacting them, so this will need to be completed as soon as you are advised, as services will tend to re-allocate your spot with other providers. The Placement Coordinator for FYI Training will provide you with the details of your placement, in writing. Please ensure that when contacting the service, you make it clear that you are a student of FYI Training, as services do provide placement to other education organisations. Ensure you talk to either the Director or the Placement Coordinator at the service, as they will be the person who we have been liaising with. On your initial contact with the service, you will need to organize a time for you to go and meet the Director and/or Supervisor. During this meeting you will need to get all of the necessary forms signed, take your Police Check or Working with Children s Check, and to tour the facility to familiarize yourself with the service. You will also be required to negotiate the days and times you will be required complete your placement. Please note, you are required to take the following documentation with you on your initial visit; Police check and/or Working With Children s Check A copy of your resume A copy of your passport or passport photo to place on your file (Aged Care only) Placement Agreement To be signed off by all parties and returned to FYI Training Your placement will most likely will photocopy your police check or working with children check as they need to make sure that you have one for when you start, and your resume can come in handy as some placements like to have one of these on file in case they decide to offer you a job. A first impression is always very important, so make sure you dress nicely, look presentable and are very polite and courteous as this placement could one day become your workplace!

10 P a g e 9 Days, Times and Hours You are required to attend placement on the days you have been specified on your agreement. Changes to these allocated days cannot be made unless the Placement Coordinator has been notified in writing and this has been approved. These restrictions are for your own safety and it is important that the documentation reflects what is in fact occurring at placement. You are not to do placement on weekends, public holidays or over the Christmas break. This is because our office is closed and if there is an incident or emergency while you are out on placement, there will not be any staff available for you to contact. You are required to do an 8 hour day. All of our placements have been organized so that all students will experience working in their chosen field. Therefore you are required to complete an 8 hour shift with a half hour lunch break and a short morning or afternoon tea break. Your lunch break is not counted as part of your placement, so your log book should reflect this (i.e. 9am 5pm = 7.5 hours). There are different types of shifts that you will need to complete depending on what course you are completing, please refer to the When Starting Placement section of this handbook for more details. You need to contact your placement and FYI Training about late arrival, leaving early and absenteeism. When there are any interruptions to your placement, you need to make sure your placement and FYI Training have been notified, before your shift is due to commence. If you are running late to placement, need to leave early, or are going to be away for any reason, then you are required to notify your placement and the Placement Coordinator prior to the occurrence. Please note that this should only occur under extreme circumstances, and failure to comply with your Placement Agreement can result in the suspension of your placement. You need to get any change of times/days approved by your placement and ensure you let your practical Placement Coordinator know. Of course there are some unforeseen circumstances that will require you to change the times or days of your placement (e.g. change of circumstances means that you will need to start doing school drop offs or pickups). We understand that this is unavoidable and if you need to change the times or days that you are at placement, you will need to discuss this with the Director and Placement Coordinator. Most placements are quite flexible and are able to assist students with their requests, as was stated previously, some situations are unavoidable. Once you have permission from the Director of the service, you must notify the Placement Coordinator who will note this on your agreement. If you are injured outside of the agreed time and days stated on your agreement, then you will be at risk of not being covered under the Workcover Act. Students must continue to do placement and are encouraged to see if they can pick up extra days during the school holidays. Most courses go over the school holidays. Students will have a two week break where they will not be attending class, but they will need to continue placement. We encourage students to complete extra hours over the holidays. We recommend that you complete at least three days of placement each week of the school holidays. This will ensure that your program is completed in a timely manner.

11 P a g e 10 What to Take With You While You Are Out on Placement There are three main things that you will need to take with you while you are attending placement. You need to ensure that you have this documentation each time you visit the services, as this is documented evidence. The first is your placement log book You must take this with you every time you attend placement. It is the main piece of documented evidence (with regards to placement) for your program. Your Supervisor is required to sign off on it every time you are in attendance. If this does not occur, your hours will not be counted towards the compulsory amount required to complete your program. The second is your ID card This card clearly identifies you as a student of FYI Training and will ensure that staff, families and visitors to the service, can clearly identify who you are. Make sure that you wear your ID card every time you attend placement. The third is your lunch and snacks It is not compulsory to take food with you, but it is always good to have it with you as you only get half an hour for lunch and having to leave your placement and go buy lunch can take a lot of time out of this break and not leave you with very much time to eat it. When taking food to placement, check if there is any food that you cannot take with you, as all child care centers and some aged care facilities have a no nuts policy and you cannot have these on the premises, due to allergies. If you do decide to go buy lunch then make sure you are back and ready to start placement again after half an hour. As was stated earlier, you don t want to exceed this time as it will reflect poor time management.

12 P a g e 11 Conduct, Responsibilities and Expectations Placement is virtually an extended job interview. Your placement host will be looking at all the following aspects to see if you would be a suitable employee and a good fit for their organization. Safety You must adhere to all occupational health and safety requirements at all times. Not complying with these requirements will jeopardize not only your own personal safety, but also the safety of those around you. The service has a duty of care to their clients, and this will be a clear breach of this policy. Following these requirements include: - Whenever required, wearing safety equipment and protective clothing - Following safety procedures - Make sure you immediately inform your workplace supervisor and Placement Coordinator if there are any accidents or incidents Attitude Towards Placement Having the right attitude towards placement and your placement host is extremely important. You will need to: - Complete all jobs that are assigned to you by your supervisor - Tell your supervisor when you are unsure about what you are doing - Listen to any instructions that you are given and ask lots of questions - Be happy to learn - You always need to be well-mannered and polite with all staff members and - Always wear your FYI polo shirt, black pants and enclosed shoes - Ensure your presentation is clean and professional - Smokers are to ensure they have washed their hands and rid themselves of any nicotine smells before reentering the service Punctuality, Attendance and Reliability at Placement Employers will expect you to be reliable and punctual to all your shifts. You will be required to: - Arrive to placement 15 minutes early each day - If for any reason you cannot attend placement you need to contact both the Placement Coordinator and your placement host immediately and provide both with a medical certificate for that day - If you are running late for a shift, contact your placement to let them know as soon as possible - Only take the allocated time for breaks, taking too long for breaks will reflect badly on your performance Medical Issues If there are any medical conditions that you may have that could affect your performance while attending placement, then you will need to inform both the Placement Coordinator at FYI Training and your contact person at placement. This is because if anything happens while out on placement an action plan will need to be in place to deal with it. A medical certificate will need to be obtained to cover any time lost on placement, as well as a medical clearance to return to your placement.

13 P a g e 12 Dress Code When attending placement, you are representing FYI Training and you are required adhere to the dress code that we have in place. You are required to wear your FYI Training blue placement polo shirt, black pants, enclosed non-slip shoes, and your ID card. Your placement shirt identifies you as a student. This will ensure that staff and people visiting know and understand that you are only a student and won t expect you to know everything and won t ask you to complete duties that you have not been trained for or leave you unaccompanied. They will understand that you are not a staff member and it will remind them that you are still learning. Black pants are part of the placement uniform. You may not wear leggings, jeans, track pants or skirts. Leggings can look very unprofessional as they are too tight, jeans are very restrictive and you will not have a full range of movement, track pants look very unprofessional, and skirts are not appropriate attire for any of these industries. Even if other staff are wearing them, as it is not part of our uniform, you may not. We prefer students to wear black slacks, try to find a pair that are inexpensive as what you are doing out on placement can get messy. You may not wear ripped or torn pants as; again, it looks unprofessional and messy. These are your potential employers and you do not want to appear disheveled or inappropriate as this will reflect poorly on you. You must wear enclosed non-slip shoes as this is a workplace health and safety requirement. This can be runners, ballet flats, almost anything. As long as they won t fall off, your feet are covered, they have a non-slip sole and they don t have a high heel then you will be fine. If you do not wear appropriate footwear (i.e. thongs) then you will be sent home and that day will not count towards your hours at placement. You need to wear your ID card at all times while out on placement. As was stated previously, it will assist the staff members at your placement to learn your name and for clients (in the aged care and disability industries) to do the same. Also be mindful of personal appearance and hygiene. Hair should be tied back neatly and personal hygiene is a priority. Again, if you are a smoker, you are required to ensure you have washed your hands and a student must not smell of nicotine. Ensure you comply with service requirements, and smoke well away from the building.

14 P a g e 13 When Starting Placement When you start your placement you will need to sign in to the center, facility or organisation (if they have a sign in book) at the start of each shift. This is general procedure and can also be used as a back-up in case something happens to your log book. You will then need to report to either the director or your placement supervisor so then they can direct you to where you need to be and explain what you will be doing. Shifts You will need to complete different types of shifts while out on placement; these will depend on what course you are completing. NOTE: Lunch breaks for all placements count towards your total amount of hours Aged Care: The aged care placement will progress through three stages. You will be starting off on lifestyle, this is to show that you are new and you will need supervision and guidance will all tasks. Lifestyle is where you will get the opportunity to spend time with the residents and get to know them; you will be working with the activities staff at this time. Your lifestyle shifts will start at 9:30am and run until 4:30pm. After around four six shifts on lifestyle you will then progress on to personal care work (7:00am - 3:00pm). By this stage you should be capable of completing some tasks without assistance but still requires some help and close monitoring with other tasks. The final stage is where you are confident in all tasks and only require minimal supervision and direction. Throughout your placement ensure that you use your initiative and do not be afraid to ask questions whenever you are unsure of anything. Early Childhood Education and Care: Whilst working in a room, all children s services students must have a supervisor at all times. Throughout the duration you will be rotating rooms so then you spend fifty hours with each age group. You will need to keep an eye on this and let the director or you supervisor know when you will need to switch rooms and they will organise the rest. You will also need to try the different types of shifts. You are required to do a minimum of two shifts on open and two shifts on close. On open you will start your day when the centre opens (at either 6:00am or 6:30am) and do a full shift which will finish at either 2:00pm or 2:30pm (depending on what time you started). When you are on open, you will learn about how the centre runs in the morning and the routine of family grouping and how it is structured. On close you will start at 10:00am or 10:30am and finish at 6:00pm or 6:30pm. In this time you will see what happens in the evenings when the centre closes and what the children do. Both of these shifts are important as when you enter the industry you will need to know this routine (it will be very similar for every childcare centre). You can choose when you would like to complete these shifts but you will need to discuss this with the Director of the service. For the rest of the duration of your placement you will need to do a regular shift of eight hours at the same time on the same day(s). Disability: All students attend placement in Pascoe Vale, you will need to be there at 8:20am as the buses will leave at 8:30am. You will be partnered up with a staff member and will be taking a group of clients out into the community to do various activities. If you are attending this site, you will be required to apply for a Working With Children s check as there is a bus that will take clients out to volunteer in a school (that is, if you would like to go with this particular group). The buses will return at 4:00pm and this is when your placement will finish. Also as part of this program you are required to complete 48 hours of placement at an aged care facility. This will ensure that the personal care requirement of your placement is also covered and this will be arranged for you. Health Support Services: You will be allocated placement in an aged care facility and will complete an 8 hours day for a total of 160 hours. You will have the opportunity work alongside professionals in the industry to gain skills in laundry, cleaning and food preparation services. This may be combined with the Health Services Assistant qualification for a dual qualification. Health Services Assistant: You will be allocated placement in either a hospital or aged care facility and will complete an 8 hours day for a total of 160 hours. You are given the opportunity to work in the industry with professionals and will gain experience in assisting nursing staff with their duties. This may be combined with the Health Services Assistant qualification for a dual qualification.

15 P a g e 14 Placement Do s and Don ts Take Direction Placement is a hands-on tool for you to be educated in your chosen industry. Staff members will direct you on what to do, so use this to your advantage and make the most of it. This workplace experience in invaluable and ultimately you are building work skills which will be of immense help to you, when applying for positions. If you ever have any concerns that you are being treated unfairly or you are not receiving the direction that you need, you will need to fill in an incident form attached in this booklet and submit it to FYI Training Pty Ltd. Do not approach the Director in regards to these issues, FYI Training will work as an advocate, on your behalf. Constructive Criticism Do not take offence to constructive criticism as staff members only want to see you become better at working in your chosen industry. Listen to them and work on improving what they are suggesting as this will make you a better employee at the end of the course. Again, if you feel like you are being treated unfairly, submit an incident form to FYI Training as soon as possible. Use Your Initiative This is the best thing you could do to help yourself. Your supervisor or mentor will notice this (even if you don t think they do) and will inform the director of your progress and attitude. Even if you have nothing to do and you can t think of anything, ask someone if they would like you to do something for them. Under no circumstances should you be standing around doing nothing (i.e. playing on your phone), this will reflect badly on you and your work ethic and you will be doing yourself a disservice. Allergies Be aware that where you work, there are people that could have severe allergies. In all childcare centres, nuts are banned and cannot come onto the premises, therefore no children services students should be taking nuts with them to placement. It is highly recommended that no student at all should take nuts, or products containing nuts (eg Nutella) to placement, as this is an issue that can arise in both aged care and disability services, and it is better safe than sorry. Please ensure that any snacks such as bars, may also contain traces of nut products. Read labels carefully before deciding whether to pack them as part of a snack. Banned Substances Under no circumstances can illicit drugs or alcohol be to be taken to placement. Failure to comply with this will result in the withdrawal of your placement and cancellation of your training program. FYI Training has a zero tolerance of this behaviour. Smoking Smoking is only allowed during designated breaks. You are required to check with your placement host about their smoking policy and designated smoking areas. Do not come back from breaks smelling like nicotine and ensure that you wash your hands thoroughly, before returning to your shift. It is not okay for students to be smelling like cigarettes, as this can be off putting for the children and clients at the service and for those in particular who suffer with respiratory or asthma conditions.

16 P a g e 15 Emergency Situations This section explains what you are to do in various situations that you may come across whilst you are out on placement. If you happen to be in a situation and you are unsure of what to do, contact the office and ask as they will be able to assist you. Incidents/Medical Emergencies If an incident occurs on placement, the student needs to notify FYI Training immediately and fill out an FYI Training incident report and an incident report form at their placement. If it is an incident where the student is injured then the student will need to attain a medical clearance before they are able to return to placement. If it is an incident where another person was injured and the student is negligent, the student will need to report to FYI Training for a meeting to discuss what happened. Gastroenteritis Outbreak This is mainly aimed towards students completing their placement for aged care or disability. If there is a gastro outbreak at the facility that you are completing your placement, then you are not to attend placement until it has cleared. Your placement will go into lockdown and they will notify your Placement Coordinator, who in turn will contact you to inform you when it is safe for you to return to the service. Please note that we cannot log any hours, if the service is in lock down. If you are completing the children s services course and there is a child diagnosed with gastro at your centre, you will be able to continue your placement as the staff will isolate the child and the parents will come and collect him or her. It is much easier to stop the spread of infection at a childcare centre compared to an aged care facility, so it is safe for the student to continue their placement in this instance. Evacuations Upon orientation/induction at placement, all students should have been informed of where the emergency exits are and where the designated meeting point is located. In case of a fire, all students must follow the procedures of the service. This will differ, depending on the service you are attending, so ensure you are clear of the centres policy, and please ensure you ask your service coordinate, if you are not.

17 P a g e 16 Practical Placement Non-Compliance Falling Behind If you fall behind in your hours (due to illness etc.) you will be required to make those hours up, either during the school holidays (excluding the Christmas holidays when the office is shut) or once you have completed your classes. If you do not keep up with your hours, your trainer will be unable to complete their observations. Please note that this will result in you not completing your program. You will also be required to fill in an extension to your enrolment form. This will ensure that your funding continues, and you will not be withdrawn from the program. If you continually avoid completing your placement hours then you will be required to defer from the course as you would not be meeting the course requirements. If you do not contact the office, to inform them of your intentions, you enrolment will be cancelled. Please note that it is not up to your trainer to chase you up. If cancellation occurs, you will be required to re-enrol which comes at a cost. Unacceptable Behavior Please note that the following is defined as unacceptable behaviour: - Rudeness to staff, other students, clients, residents, children and trainers - Failure to comply with service Occupational Health and Safety requirements - Breach of duty of care - Using mobile phones, mp3 players during shifts - Breaking privacy and confidentiality. This could be either by disclosing private information including names of students, staff, clients, residents, children, or trainers, the name of any facility or centre, or discussing any incident that may have occurred there. - In the possession of banned substances, or appearing under the influence of banned drugs - Not responding to requests from the Director addressing any issues - Failure to comply with FYI Training s policies and procedures - Lack of notification to Director and FYI Training in regards to change of shifts - Continuously leaving placement early or arriving late - Taking photos of staff, students, clients, or children without having written consent Consent forms can be found on our website under downloads - Discussing details of staff, students, clients, children or incidents on any form of social media Any students displaying unacceptable behaviour will need to have a meeting with the practical Placement Coordinator and the training manager to discuss the issue and find a resolution. If a student is found with or on banned substances (i.e. intoxicated, under the influence of drugs), as stated previously, you will be asked to leave and be required to report to FYI Training head office immediately, an can result in the cancellation of your enrolment.

18 P a g e 17 Once Placement Has Finished You are required to submit the following evidence, upon the completion of your placement hours; - A log book signed off by the Director/Supervisor stating that you have completed the required amount of hours for your program - 2 observations signed off by your trainer, submitted and on file - Reflective journal (for applicable courses) Please ensure that you notify your Director and/or Supervisor that you are due to complete your program, so that they are aware of the date that you will be finishing with them. This will also be a good opportunity for you to thank them. You will then be required to return your log book and employability skills checklist to the office, and for children s services your journal too. If these are not returned to the office then you will not receive your certificate as these are your evidence that you attended placement and completed your hours. If you return your log book to the office and you have completed less hours than required then you will be told that you need to return to placement to complete those hours. Once you hand in this paperwork your student file will be audited by the office staff.) If there are issues with your assessments or placement paperwork then the office staff will contact you, this will extend the amount of time before you will receive your certificate. Please allow up to 4 weeks for the issuing of your certificate.
