2 Dear schoolchild. We kindly ask you to help us with a project on schoolchildren's eating habits. The study is carried out in nine European countries. More than 14,000 schoolchildren take part in the study. We kindly ask you to answer the questionnaire. Please do not write your name on the questionnaire. The questionnaires are completely anonymous. When you have answered the questionnaire we kindly ask you to put it in the envelope, close the envelope and return the closed envelope to your teacher. No one at your school will be given access to your answers. Although we sincerely hope that you are willing to help us, we want to stress that your participation in the study is voluntary. If you don t want to answer the questionnaire, please return it unanswered in the envelope. Thank you for your help! How to complete the questionnaire Most of the questions can be answered by ticking just one box against your answer. In a few questions you may tick more than one box.
