Prior to drafting any contract for good or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, the fee for our basic services of funeral director and staff will be applicable to any arrangement you select. If legal or other requirements necessitate the purchase of any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Method of Payment: Payment for goods and services is expected prior to the services being rendered. For your convenience, we accept cash, check, credit card (Visa, MasterCard, Discover, and American Express). We also accept the use of Insurance Policies as guarantee of payment. If accepted an Insurance Assignment Processing Fee of 4% or a minimum of $100 will be charged. 2 I. Professional Services of Funeral Director and Staff $1995 This is the basic charge to each family which we serve for our professional expertise in assisting them. It includes, but is not limited to a proportionate share of the taxes, licenses, utilities, and business operating expenses necessary to serve the public. Also included in this charge is the consultation with the family, clerical and administrative services, preparation and filling of necessary documents and permits, consultation with cemetery, crematory, common carrier, or other outside organization, maintenance of our tributes on the website. There will be an additional charge for the coordination, direction and supervision of each service requested or required. Notice: This fee or a portion of this fee that pertains to the basic services is already included in our charges for: direct cremations, immediate burials, forwarding and receiving of remains. II. Embalming and Other Preparation of the Body Except in certain special cases, embalming is not required by law. Embalming may be necessary however if you select certain funeral arrangements such as a service with viewing. California Law does require refrigeration of an unembalmed body within 24 hours. If you do not want embalming, you have the right to choose an arrangement that does not require it, such as direct cremation or immediate burial. A. Embalming $495 B. Funeral Home Facilities, Staff and Equipment Our charges include coordinating, attending and directing services and participants; staff to supervise and coordinate church or memorial services; preparing, supervising, setup, and attendants for visitation periods; arranging floral pieces and distribution of memorial stationary; supervising and attending at graveside; parking assistance; additional supplies and equipment may be available upon request. 3 V. Automotive Equipment and Other Equipment Additional mileage fee in excess of a 30 mile local service radius is $3 per mile, each vehicle. A. Transfer of deceased to funeral home $295 B. Second Attendant $100 C. Use of Hearse/Coach and driver $495 D. Limousine or other passenger vehicle and driver (4 hours / each additional hour $150) $450 E. Flower van and driver $195 F. Merchandise Melrose Abbey Mortuary offers a variety of caskets, urns and alternative containers from which to select. Caskets many times appear similar although they may differ greatly in quality and construction. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are alternative containers made of fiberboard and other cremation containers, including containers made of fiberboard, and containers made of other composition materials. Sea Scattering Ceremonies Ceremonies at sea offer family and friends the opportunity to gather and say goodbye during a trip of approximately (2) two hours that departs off the coast of Orange County. The services can be non-denominational or if a more religious format is desired we can assist in coordinating a clergy person. 4 IX. Direct Cremation $1495 to $5790 State and local laws do not require a casket for direct cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without any interior dressings). A. Direct cremation with container provided by purchaser $1495 B. Direct cremation with minimum alternative container $1990 C. Direct cremation with casket selected from our funeral home $1990 to $5790 Direct Cremation is the minimum cremation service offered for disposition of remains. This service includes: Basic Services of Funeral Director and Staff, Transfer of deceased to funeral home (within 30 mile service radius), Refrigeration, Placing shrouded remains into cremation container selected (purchased separately) and Cremation Fee. No additional services are added to this arrangement. If additional services are desired, the itemized fees from the General Price List are utilized along with the fixed fee of Basic Services of Funeral Director & Staff. X. Immediate Burial $1590 to $10385 Immediate Burial is the minimum service offered for disposition of remains in a cemetery. This service includes: Basic Services of Funeral Director and Staff, Transfer of deceased to funeral home (within 30 mile service radius, Refrigeration, Placing shrouded remains into casket selected (purchased separately) and Delivery Only to local cemetery. This is a Non-Witnessed/Attended Interment. No additional services are added to this arrangement. If additional services are desired, the itemized fees from the General Price List are utilized along with the fixed fee of Basic Services of Funeral Director & Staff. A. Immediate Burial with container provided by the purchaser $1590 B. Immediate Burial with minimum casket $2585 C. Immediate Burial with casket selected from our funeral home $2585 to $10385 XI. Forwarding of Remains to another Funeral Home $2195 This service includes: Basic Service of Funeral Director and Staff, Transfer of deceased to funeral home (within 30 mile service radius), Embalming, Dressing/Shrouding and placing in casket or shipping container (purchased separately), transportation to local airport. No additional services are added to this arrangement. If additional services are desired, the itemized fees from the General Price List are utilized along with the fixed fee of Basic Services of Funeral Director & Staff. XII. Receiving of Remains from another Funeral Home $1995 This service includes: Basic Service of Funeral Director and Staff, Transfer of deceased to funeral home (within 30 mile service radius), and Deliver Only to local cemetery. This is a Non-Witnessed/Attended Interment. No additional services are added to this arrangement. If additional services are desired, the itemized fees from the General Price List are utilized along with the fixed fee of Basic Services of Funeral Director & Staff. XIII. Receiving of Remains from Cemetery (Disinterment) $1995 This service includes: Basic Service of Funeral Director and Staff, Transfer of deceased to funeral home (within 30 mile service radius), and Special Care of Remains. If additional services are desired, the itemized fees from the General Price List are utilized along with this fee which outlines the services covered. PG4

The following list demonstrates the savings available when you select from the packages shown below. No credit for deletions will be given. Merchandise selection such as a casket, floral arrangements, and printed material can be a very personal choice, therefore we do not include those items on your behalf. LOTUS CREMATION PACKAGE $5400-$6200 This is a special arrangement that includes professional services and merchandise for those that desire a very structured tradition. No credits for deletions are available. An additional discount of $800 will be given to those purchasing new cemetery property at Melrose Abbey Memorial Park.
