1 Annual management review Healthy Working Lives Health and safety policy Has your health and safety policy been reviewed within the last year? Are directors and managers clear on their health and safety roles and responsibilities? Are all staff clear on their health and safety roles and responsibilities? Do you have a process for monitoring the effectiveness of your health and safety policy? Organisation Do you have a competent person(s) to give support on occupational health and safety issues? Do they have enough current knowledge, experience and training to do this? Do you have measures in place for communicating and consulting staff on health and safety matters? Do you display a valid employer s liability insurance certificate for staff? Talk to the Healthy Working Lives experts FREE on:

2 Do you have the health and safety law poster displayed and is the contact information still relevant? Are first aid arrangements adequate for the business needs? > first aid kit > appointed person(s) > trained first aider (Refer to the First aid section of your folder.) Do you have arrangements to check the competence of contractors? Do you have a system to make contractors and visitors aware of appropriate health and safety information? Have you reviewed individuals health and safety training needs this year? (Refer to the Training section of your folder.) Do you record staff health and safety training? (Refer to the template in the Training section of your folder.) Are there adequate measures in place to supervise staff? Planning and implementation Do you have a process for identifying hazards and carrying out risk assessments to eliminate or control risks? (Refer to the Risk assessment section of your folder.) Do your risk assessments consider staff members who are: > young persons? > new and expectant mothers? > disabled?

3 Do you have a process for monitoring and reviewing your management arrangements and workplace precautions? (Refer to the Monitoring and review section of your folder.) Do you have a process for checking progress on your annual action plan? (Refer to the Management annual action plan section of your folder.) Do you have a process to ensure adequate resources are being allocated to your action plan? Measuring performance Do you check that your emergency procedures are adequate? Do you check that your workplace health and safety monitoring programme is being carried out by those with delegated responsibility? (Refer to the Monitoring and review section of your folder.) Has your occupational health monitoring been carried out? Do you check that your work equipment health and safety monitoring programme is being carried out by those with delegated responsibility? (Refer to the Work Equipment section of your folder.) Do you have written schemes of examination for equipment that requires a statutory inspection? For example: > lifting equipment > pressure systems > local exhaust ventilation (LEV) Do you check that the above have been carried out at the recommended frequency and ensure action and repairs have been carried out in accordance with the inspection reports?

4 Do you check that the following have recently been checked by a competent person? > portable electrical equipment > fixed wiring Do you check that the gas supply and appliances have been inspected recently by a suitably qualified Gas Safe registered engineer? (Recommended annually) > Are all gas shut-off valves accessible? > Are all gas pipes identified? Have you recorded, investigated and reviewed all workplace incidents and taken action to prevent a recurrence where possible? Have you ensured all RIDDOR injuries, diseases and dangerous occurrences have been reported? Having reviewed the incident data available, are there any areas where trends show your controls may need to be improved? If the premises have been inspected by the Enforcing Authorities in the last year, have you addressed the issues raised? Welfare arrangements Are indoor working temperatures comfortable throughout the year? Do you have adequate arrangements for staff working in wet and/or excessive hot and cold conditions? Is there adequate ventilation throughout the workplace? Is there adequate lighting throughout the workplace? Is there enough space to allow staff to work safely and comfortably?

5 Are all floor surfaces in good condition and free from slip or trip dangers? Is drinking water available direct from the mains/bottles? Can staff eat meals, etc. without risking it being contaminated? Can staff make a hot drink at work? Can rest facilities be provided for new and expectant mothers, including a refrigerator for expressed milk? Do you provide space for staff to change and store clothing? Do you provide enough toilets for staff? Is the whole workplace kept in a clean and tidy condition? Have you made sure staff working off site have access to similar facilities?
