2 General: ADDENDUM NO. ONE TO THE PREQUALIFICATION DOCUMENTS June 24, 2016 The following changes, additions, or deletions shall be made to the following documents; all other conditions shall remain the same. I. DESIGN-BUILDER PREQUALIFICATION QUESTIONNAIRE 1. On page 10 of 40 under the first paragraph of LEED/Sustainability Experience add the following paragraph "Design-Build team capabilities should be identified in the comparable projects and key team resumes that are submitted." 2. On page 11 of 40 Organization and Management Plan add the following "3. The organizational chart submitted should have placeholders for subconsultants/ subcontractors at this time." II. CLARIFICATIONS Q1: The Advertisement for Design Builder Prequalification and the instructions in the Prequalification Questionnaire include elements not formalized in the Prequalification Questionnaire including: 1. Key Personnel: (architectural team and contractor team) common experience. 2. Organizational Chart of Key Participants 3. Management Plan 4. Knowledge of Design-Build Process 5. Quality Control Plan for Design Phase and Construction Phase 6. BIM Modeling 7. LEED/Sustainability Experience Since all items highlighted above are not questions within the questionnaire, nor VII. Required Completed Attachments as noted on page 40 of 40 of the questionnaire, should these items be submitted as attachments? A1: Submittal requirements for the above components are identified in the questionnaire with the exception of LEED/sustainability experience which should be identified in the comparable projects and the key team resumes that are submitted. 3 02: A2: 03: A3: 04: A4: 05: A5: 06: AS: Due to the project's scope of work the design-build team may include a parking consultant as one of the design firms in the prequalification package. If so, it is assumed that the Contractor and building Architect would not be required to submit a 1,200 (minimum) parking space parking structure project as one of the three comparable projects. Please confirm. No. Provide parking experience of architect and contractor. I believe this was clarified at the pre-bid meeting, but we want to confirm: Page 2 of the questionnaire states, "Only the names of the Architect(s) and Contractor(s) are to be provided in response to this Request for Prequalification." However, on page 11, it states, "Each team shall provide an organizational chart to describe how the Design-Builder intends to manage/deliver this project, including internal, subconsultants (design team members and consultants - note: a waterproofing consultant is required to review all enclosure, roofing details and mockups), and owner lines of communication and responsibilities." It is our understanding that we should include our parking designer but not submit subconsultant names at this time. No - Architects and Contractors only. The organizational chart submitted should have placeholders for subconsultants/subcontractors at this time. For the project questionnaire, if we are teamed with multiple architectural firms and a parking designer, should we list 3 projects for the contractor, 3 projects for each architectural firm (6 total), and 3 for the parking designer? Architect: Total of three (maximum) housing and parking projects. Contractor: Total of three (maximum) housing and parking projects. In the staff questionnaire, if we are teamed with multiple architectural firms and a parking designer, should we fill out the questionnaire noting the principal and project manager for each architectural firm and the parking designer (for a total of 6 pages)? Revised submittal requirements allowed shall be Executive Architect/ Architect Principal-in-Charge in association with Design Architect. Provide Resumes for: Principal Project Manager Project Designer We intend to submit a comparable project that will be completed for the Fall 2016 semester, prior to the date that the RFP will be due. Please confirm that this Is acceptable. No. All projects must be completed by the date of submittal. Page 2 of 4

4 Q7: I wanted to clarify who to address our submittal to. On the downloaded print out it says Rhonda Mitchell with two different addresses depending on how we deliver it. But then we have the Ad Prequalification's pdf that says this (to Carrie Robb): Prequalification Questionnaires will be received only at the following address: FACILITIES DESIGN & CONSTRUCTION N. Item D. Insurance (pg. 22), states that we are to provide certificates of insurance on the University's form. Can you clarify which form that is and/or where it can be obtained? Submit University's form (Accord) evidencing that Proposers shall maintain insurance in the stated amounts. Attached is a University sample form. Per the questions and discussion at the Prequalification Conference, it was asked if teams with two or more architects could split the three (3) Design Firm's comparable projects between the design and executive architect of record. Please confirm the architect of record will only be able to submit three (3) Design Firm comparable projects. Revised Submittal Requirements allowed shall be: Executive Architect in association with Design Architect: Total of three (maximum) housing and parking projects. Please confirm that the requirements of Section 5.B, specifically those related to Surety/Financial/Insurance, are required of the Contractor only. Yes. Per the question raised during the Prequalification Conference, it was noted that the Design Firm could submit resumes in addition to those identified in the Questionnaire, specifically for an Architectural Designer and an Interior Designer. Will the Questionnaire form be revised to reflect this? Or should Proposers alter the form to suit? See A5. Page 3 of 4

If we are the lead entity submitting on the project but will be submitting in association with another contractor, may we also list (3) three comparable projects for the associate firm as well? KEY PERSONNEL does not ask for a resume for a DESIGN MANAGER. We feel this is an important team member to include with the Key Personnel. If we included the qualifications for this member would we receive credit towards the possible points in this section? See A5. Regarding the Questionnaire Section IV.A - Design Build Experience, Design Firm Comparable Projects Form: Should the box that indicates the "Contractor's Office that Performed the Work", be changed to the "Design Firm's Office that Performed the Work"? Also, should there be an additional box to identify the Contractor's contact information? In this way, the Design Firm form is consistent with the Contractor form.
