2 This manual covers operation and installation procedures for the following products: CTSIB Utility Software, v3.x Biodex CTSIB Utility Software provides three primary functions: 1. Archiving and management of specific four-condition CTSIB tests commonly used in concussion management. 2. To provide a means for earlier Biodex Balance Systems to create a CTSIB progress report. 3. To enable portability of test data so that results can be viewed on a Windows-based computer. DISCLAIMER: The information provided is not intended to be a substitute for professional medical advice but as a guideline to assessing athletes following a concussion. 4 1 Introduction The Biodex CTSIB Utility Software is a PC-based program where the athlete s baseline balance test data is entered and stored. In the event an athlete suffers a concussion, subsequent balance test data is entered into the program. The software can then provide a Summary Report illustrating the athlete s relative performance of the post-injury balance tests to the pre-injury baseline. Concussion management best practices for baseline assessment includes cognitive testing, balance testing and a graded symptoms checklist. Biodex Balance Assessment is a single component of the recommended three, performed on either the Balance System SD or portable BioSway balance assessment devices. 5 2 Download and Install the Software 2.1 Download The Biodex CTSIB Utility Software can be downloaded from the Biodex website. 2.2 Installation of Software Follow the steps below: Download the software and then click on the setup.exe file. A pop-up window will display and begin to prepare the installation sequence. Click on the <Next> button and proceed according to the instructions given on screens. 7 3 Getting Started 3.1 Sign-in and Setting Password The initial screen is where User Name and Password are established. To begin, enter the software defaults: User Name: Biodex Password: Biodex Figure 3.1 To establish your own Password, click the <Reset Password> button. See Figure 3.1. 8 3.2 Main Menu Once the Password is established, sign in and the MAIN MENU will appear. See Figure 3.3. Figure Preferences It is best to setup the Preferences first. Preferences are intended to save time and keystroke entries when entering test results. For example, it is likely a group has been tested with common data (e.g., school name, sport, team). Preferences eliminate the need to repeat data entry for each athlete in the same sport for the same test, etc. Figure Instruction Manual, CTSIB Utility Software Biodex Medical Systems, Inc. 2013
9 To Set Preferences (See Figure 3.4): Select the Preference to set Simply select from the pull down menu OR enter a New Preference. Click on <Save> button. For the new Preference to be the default, go to the list, select and check to Make Default and click on <Save> button. Preferences can also be Deleted from the list. Click <Save> button to record all changes. See Figure 3.5. Important consideration regarding Preference Defaults and User Name: All defaults are saved per User Name. If multiple people use the program and have different sport responsibilities, or use different Cognitive software, then creating unique default Preferences and thereby unique User Names are recommended. 10 Figure 3.6 Example of Default Settings: Northwest Field House was entered as the New Facility and the <Make Default> box was checked. As long as Preference defaults are selected to be used, this will be the Facility of record. This can easily be changed. The same is true for the other Preferences. (See Figures 3.6 to 3.10.) 11 Figure 3.8 Figure 3.9 Post-Injury Sessions: Post-injury testing is recommended every 24 hours. Sometimes this is not possible. Should that be the case, either be more specific for the follow-up test by entering the specific session, or selecting something sequential; Test 1, Test 2, etc., following injury and with the test date, noting the exact date of testing post injury. 12 The Baseline % is a value that is entered for an intended upper limit range, indicating where the post-injury scores should return for the specific condition. The default value is set at 15%, which means the post-injury score should return to within 85% of the baseline value.* See Figure Figure 3.10 *See Section Rationale for 15% Baseline. 12 Instruction Manual, CTSIB Utility Software Biodex Medical Systems, Inc. 2013
13 3.4 Setting Up Additional User Names It is possible to set up unique users. Preference defaults are saved per user; creating multiple user accounts is recommended when unique default settings are also desired. To set up Unique Users: A hidden menu must be invoked, to do this: 1. Click the mouse twice on the upper right corner of the Preferences main screen, then 2. Click the mouse twice on the upper left corner. 3. A screen will appear with the default Biodex user. (See Figure 3.12) SECOND: Click mouse 2x upper left corner, after clicking on the upper right corner. FIRST: Click mouse 2x upper right corner. 14 3.4.1 Enter New User To Enter New User information, follow the steps below: 1. Click on the <New User> button shown in Figure System will forward to a New User screen/form with required fields to create a new user. See Figure Enter the desired Defaults, User Name and Password for the New User. From this screen, click <Back> to leave this operation and return and reference existing Preferences. 4. Once all the information is entered correctly, click the <Save> button. 5. Upon successful operation, the system will display the newly created user on next screen. 15 3.4.2 View User Information To View user information, follow the steps below: 1. Select any User from the list 2. Click the <Confirm> button. See Figure System will display the existing details of the user 4. Make the required changes, or it can be left as it is 5. Click the <Save> button. 16 3.4.3 Delete User To Delete user, follow the steps below: 1. Select any User from the list. 2. Press <Confirm> button. See Figure System will display the existing details of the user. 4. Click the <Delete> button. 5. System will prompt for confirmation of deletion. 6. By clicking the <Ok> button, system will remove the user from that list or click <Cancel> button to cancel the delete operation. 17 3.5 Entering Athlete Balance Test Results 1. Select <Enter Athlete Test Data> from the main menu. See Figure If New Athlete, select <New Athlete>. To illustrate, instructions will proceed as if New Athletes completed baseline balance testing for the Football team. If this was a follow-up test: Select the Athlete from the list. To see all athletes, enter the asterisk symbol (*). Lists can be filtered by clicking on the first letter of the last name. Lists can be sorted by clicking on the column heading. Figure 3.19 NOTE: When <Use Preference Default> check box is selected, all that remains to be entered are the Athlete s name and ID or alternate ID and any notes. The default Preference fields are pre-populated. Notes entered are saved in the Athlete s record and will be printed on the Summary Report. 18 3.5.1 Manually Entering the Test Data From the Athlete s CTSIB report, simply enter the SWAY INDEX scores for each condition. Do this for all athletes tested. 19 If this was a follow-up test: Select the Athlete from the list. To see all athletes, enter the asterisk symbol (*). Lists can be filtered by clicking on the first letter of the last name. Lists can be sorted by clicking on the column heading. Our Athlete is Big Bob. Select <Big Bob> as well as <Session Type> and <Test Date> for each subsequent test. Click the <Confirm> button. 21 3.5.2 Importing Data Files from USB Thumb Drive The Balance System SD and Portable BioSway with the new 12 Win CE display running 2.X Biodex Balance Software can export Patient CTSIB test files to a USB thumb drive. These test files can then be imported into versions ranging from 2.X to 3.X CTSIB Utility Software. Figure Press <Import File Data> button from the Enter Athlete Test Data screen. See Figure Select the <BioData> folder to import the.bio file(s). See Figure Select files from the BioData folder on the USB flash drive. 22 3. Choose Manual or Automatic Merge. In a Manual Merge individual test records are selected and can be reviewed prior to saving. Athlete test record can also be skipped. Specific ID values can be added. In an Automatic Merge all tests are imported and ID value remains 0. The ID can be assigned and populated later. See Figure Figure In a Manual Import a specific ID can be added and other fields edited and saved at the time of entry. 23 5. If the specific test exists, a message will appear. You can skip the record and go onto the next. This ensures that tests are not duplicated. See Figure Figure Automatic Data Import Select the <Automatic Merge> button 7. On the next screen, system will display some basic summary information of the.bio file selected from the list. 8. 24 9. The status of data transfer will be shown. See Figure Figure Click on the <Back> button to return to the imported list. 11. You can select another.bio file from the imported list and perform another import through the Manual or Automatic Merge option. 25 3.6 Manage Athlete Click on the <Manage Athlete> button from the main menu. Figure 3.33 Manage Athlete is where the Athlete s records can be edited if needed. 26 3.6.1 Test Data Update To Update/Edit any Athlete test data, follow the steps mentioned below: 1. Select any Athlete Test Data from the list that we want to update 2. Click the <Edit Test> Button 3. On the next screen, system will display the existing athlete test data 4. Make the changes as needed 5. Click on the <Save> button to Save whatever changed is made 6. 28 3.6.2 Delete Athlete Information Delete is used to Delete an Athlete from the database, but only after all tests have been deleted. To delete Athlete information, follow the steps below: 1. Use Edit Athlete for the particular test to be deleted 2. Click on the <Delete> button on that screen 3. To delete the Athlete, all tests must first be deleted, otherwise system will display an error message stating to delete all tests 4. Once all the test data are deleted, by clicking the delete button now will display a confirmation message before the final delete. 29 3.6.3 Delete Test Data To Delete tests, follow the steps below: 1. Use Edit Test for the particular test to be deleted 2. Click on the <Delete> button on that screen 3. System will display a confirmation message before the final delete of the test 4. 30 3.7 View and Print Reports To check Athlete s progress: Select <View and Print Reports>. Figure 3.40 To preview or print reports, follow the steps below: 1. Select any Athlete from the list 2. 32 4 Appendix 4.1 Backing Up Data Figure 4.1 Microsoft Access Database is the database program. The program data is stored as an.mdb file. It is suggested that you back-up your data files simply by clicking on the <BackUp Data> button. The file can then be copied to another storage device. See Figure 4.1. 33 4.2 Rationale for 15% Baseline Geurts, et al, showed a test variability difference between controls and TBI patients of 18%. Guskiewicz, et al, noted an average difference of 6% between baseline variability and day 5 post injury. Lehman, et al, in the paper Quantitative Evaluation of Sway as an Indicator of Functional Balance in Post Traumatic Brain Injury noted a standard error of measure for normative values of 17% Considering Guskiewicz was using values considerably larger than what are presented with CTSIB results, test variability between 6% and 18% appears reasonable.
