1 Free or Low Cost Health Care for Immigrants- Davidson County Bridges to Care Call to Register Phone: (615) Hours to call: Mon - Fri 8 am to 4:30 pm 311, 23 rd Ave. N., Nashville, TN Bridges to Care links uninsured residents of Davidson County to a network of lowcost community clinics that charge for service s based on the patient s ability to pay. Through Bridges to Care, you can find a clinic that is best for you based on location, hours, language, and charges. The Bridges to Care network includes an array of primary care providers and dental care providers, as well as referrals to specialty care, access to prescription medications, and transportation to medical appointments. Clínica Hispana La Paz 4053 Nolensville Road ( ) Medical services for the whole family, serving the Hispanic Community in Nashville. Bilingual nursing staff. Pediatrics, gynecology, lab tests, work and school physicals, general medicine, allergy shots, work injuries, minor surgeries, minor emergencies. They charge fixed rates for services (not on a sliding scale) but their fees are lower than most private practice fees. Walk-ins welcome Hours: Mon-Sat 8:30am 7pm; Sunday 11:30am 6pm Faith Family Medical Clinic Phone: Address: st Ave N Nashville, TN Appointment required Services are available to individuals who are either working at least 20 hours a week or are receiving unemployment or are full-time students, as well as their families. Individuals must not have any form of health insurance. Fees are between $15 and $40, based on patient s income. Proof income, or proof of being a full-time student, is required. Family medicine; diagnostic lab testing; Physical therapy; Individual and/or family counseling; Basic X-rays, CAT scans, and MRIs can be done for a reduced fee. No Walk-ins. Hours: Mon, Tu, Thu 7:30am 5:00pm; Wed and Fri 7:30am 4pm Matthew Walker Comprehensive Health Center Phone: Address: th Avenue North Nashville, TN The Center focuses on primary and preventive medical and dental services to the underserved and uninsured. Transportation is provided. Services are provided to uninsured patients in north, northwest, and south Davidson County. Fees are based on a sliding fee scale. Documentation of income is required (check stubs, monthly statements from SSI or DHS, or letter from head of household stating dependency are examples). Call for current fee schedule. In emergency situations the fee may be waived.

The cost of the visit is $40 but patients who cannot afford this can be charged on a sliding scale based on household income, but no one is turned away if unable to pay. They provide healthcare services to the underserved and uninsured population. Call re: dental program. They are working on having dental, but not in place as of 11/2007. Eligibility is based on proof of household size and income. Fees are $25 per visit. Patients have two weeks to verify proof of income for initial visit. After that, patient is charged 100% of medical services rendered for entire medical visit.
