72 SB SCE Proposal Request Notice LANGUAGE FOR SCE PROFESSIONAL SERVICES PROPOSAL REQUEST Your firm/and other firms with Small Business Design Contracts are being invited via this notice to propose services for survey cost estimate (SCE) for the following project: The project scope shall be to evaluate and prepare a Survey Cost Estimate. Survey Cost Estimate Professional Services Proposal Request Project No. DGS Description Location City, County, PA (Zip Code) Agency/Institution Representative at Site: Name Telephone No. If you elect to propose services to perform the survey cost estimate, complete the 150 Form. 73 (Version ) SB Professional Design Project Request Notice LANGUAGE FOR PROFESSIONAL DESIGN SERVICES PROPOSAL REQUEST Your firm/and other firms with Small Business Design Contracts are being invited via this notice to propose Professional Design services for the following project: Professional Design Services Proposal Request Project No. DGS Description Location City, County, PA (Zip Code) If you elect to propose to perform Professional Design services, complete the 150 form. If you are selected, we will send you a work order for Professional Services under your Small Business Design Contract via within 4 weeks from the date of this notice. In accordance with terms of your Small Business Design Contract ( the Contract ) with the Department, your firm will be compensated $26, for this Work based upon a fixed fee percentage of 15% of the Total Base Construction Amount.
