Licensed Lead Abatement Contractors for the above-listed address. Please find attached a scope of work. This work shall be funded and contracted under the WCDP Lead Safe Westchester Program. The Westchester County Health Department issued a Lead Paint Violation Notice for this property. You will need to contact Mr. Eric Steinnagel at (914) , to discuss the scope of work, method of abatement, and devise a work plan. The Health Department will be conducting final dust wipes for clearance when the job has been completed. Once you have met with Mr. Steinnagel, and discussed the scope, your Bid Proposal shall be submitted to: M. 3 SECTION LEAD PAINT ABATEMENT 1. Certification Documents and Insurance Certificates A. The Contractor shall make available, upon request, copies of certificates documenting the successful completion of an E.P.A. approved Lead Abatement Course for any worker who will participate on the project. Certificates shall show the date of course completion. B. The Contractor shall provide the owner and a representative of the Department of Health with current certificates of insurance. 1. Records The Contractor shall submit to the Westchester County Department of Health, a detailed written summary of the technique used to comply with the regulations, which follow in this specification. 2. Testing The Contractor shall be responsible for testing all workers as described in U.S. Department of Labor Occupational Safety and Health Administration (O.S.H.A.) Document 3126, April 1991, Working with Lead in the Construction Industry and 29 Code of the Federal Register (CFR) Lead, revised as of July 1, 1989, U. S. Government Printing Office. For wipe testing, the Contractor shall employ the services of a licensed independent testing laboratory. Original testing result documents shall be given to the owner and copies of these documents shall be given to a representative of the Department of Health and posted on the site. Each test report shall contain the following information: A. Area size of each sample component. B. Material type of each component sampled. All testing shall be made in compliance with the Interim Guidelines for Hazard Identification and Abatement in Public and Indian Housing, HUD, May Testing shall be done as follows, and shall be represented as a separate line item cost on the bid proposal sheet: A. Pre-abatement tests- Wipe tests shall be performed on a window sill, window well, and floor surface, of each room where abatement is to take place prior to abatement set-up and mobilization procedures. B. Post-abatement tests- One wipe test shall be taken on each surface as described in pre-abatement tests after the abatement is complete, waste is removed, and wet-wipe clean-up is complete. Each wipe sample shall cover a one square foot minimum area (or as close as possible in the case of window sills and wells. The length and width of the window sill, and well, tests shall be documented in inches). The Contractor shall notify the Westchester County Department of Health after the wipe tests have been scheduled, but before they are taken. Acceptable sample results shall not exceed the following values: A. 40 micrograms per square foot for floor samples. B. 250 micrograms per square foot for window sill samples. C. 400 micrograms per square foot for window well samples

4 Surface dust sampling shall take place no sooner than 24 hours after the completion of post abatement clean-up activities. Wipe samples conducted outside the containment area (exterior) shall be made within 10 feet of the containment area. If any of the residual lead dust level results exceeds the appropriate clearance criteria, the area shall be cleaned again and retested until acceptable results are achieved. The number of tests conducted on this project shall be in accordance with HUD guidelines. The Contractor shall provide a line item cost for TCLP testing to be conducted in accordance with HUD publication Lead Based Paint: Interim Guidelines for Hazard Identification and Abatement in Public and Indian Housing; September 1990, Chapter 11, Waste Disposal, The Contractor shall present test result documentation to the owner and a representative of the Department of Health prior to waste disposal. The Contractor shall provide line item cost breakdowns for the disposal of materials labeled industrial and hazardous. 3. Worker Protection The Contractor shall adhere to worker protection practices as described in O.S.H.A. Document 3126, April 1991, Working with Lead in the Construction Industry, and 29 CFR Lead, revised as of July 1, 1989, U. S. Government Printing Office. Protection practices shall include, but are not limited to: 4. Set-up A. Selection and usage of appropriate respirator. B. Provision and usage of protective work clothing. C. Housekeeping of work site surfaces. D. Vacuuming. E. Change room. F. Lunchroom. G. Shower. H. Lavatory. I. Medical surveillance. J. Biological monitoring. K. Medical examinations. L. Record keeping and reporting. The Contractor shall adhere to set-up practices as described in Volume 52 Federal Register, Page 1890, Part 35, January 15, 1987, and as amended in 53 FR20789, June 6, 1988; 56 FR920, January 9, 1991; 56 FR15172, April 15, 1991 and 24 CFR35.24B. Set-up practices shall include: A. Isolation of work areas including proper signage. B. Barriers denying access to site. C. Pre-cleaning of abatement area including use of HEPA vacuum. D. Preparation of work area including the sealing of vents, windows, doors, and non-abated areas with two layers of six-mil plastic, securely fastened. E. Airlock preparation. F. Decontamination areas

Clean-up and Disposal The Contractor shall adhere to clean-up procedures and disposal requirements described in documents cited in Section 4, Set-up, and in the Interim Guidelines for Hazard Identification and Abatement in Public and Indian Housing, HUD, May Clean-up and Disposal practices shall include, but are not limited to: A. The maintenance and use of HEPA vacuum unit during each clean-up pass. B. The appropriate mixture and application of a phosphate containing solution during each clean-up pass. C. The appropriate packaging, labeling and disposal of all abated waste and clean-up materials. All identification and disposal of waste items shall be made in compliance with appropriate Environmental Protection Agency and Resource Conservation and Recovery Act regulations. The Contractor shall remove the lead paint in the areas described below. The contractor shall provide a line item cost for each room listed below and detail in his/her bid the work to be performed in each room for a complete and thorough job. Any bid submitted without this information shall be disqualified from the bidding process. PLEASE NOTE: ALL AREAS THAT HAVE BEEN SCRAPED, STRIPPED, OR REPLACED, ARE TO BE VIEWED BY THE HEALTH DEPARTMENT PRIOR TO BEING PRIMED AND PAINTED TO ENSURE THEY HAVE BEEN PROPERLY DONE. IN ADDITION, ALL SCRAPED WOOD SHALL BE MADE SMOOTH PRIOR TO BEING PRIMED. The items listed below may be either stripped and repainted, removed and replaced, or in the case of a wall or ceiling it shall be sheetrocked, but will be left up to the discretion of the Health Department. Areas to be addressed are as follows: 120 Beech Street, Yonkers, NY FRONT PORCH- Overhang Assembly Upper Entablature (above front exterior windows) All debris associated with the above installations shall be removed from the premises and disposed of properly. The Health Department shall be notified when cleanup begins to arrange for Final Dust Wipes. -6-
