1 Community Health Snapshot: Healthcare Initiative Consortium
2 Definition and Purpose! The Healthcare Initiative Consortium is a group of healthcare leaders gathering Hall County data on some of the top health indicators.! This group is working toward community health improvement on the local level using meaningful and specific data, as well as ongoing communication.
3 Members! Last year, this group provided a community snapshot on adult health indicators. For this information, go to: Group includes representatives from:! Physician practices: " The Longstreet Clinic " Northeast Georgia Medical Center " Northeast Georgia Physicians Group Primary Care Clinic at the Hall County Health Department! Brenau University College of Health Sciences! Good News Clinics! The work of this group dovetails with Vision 2030 and the Greater Hall Chamber of Commerce s Healthcare Committee.
4 Obesity: Findings from Adult Survey! Regardless of insurance status, between 75 & 80% of the adult sample (2011 data, patients 18 and older) are overweight or obese, which is worse than state and national data.! Georgia data show 64.8% of adults were overweight, with a Body Mass Index of 25 or greater. Nationally, more than one-third of U.S. adults (35.7%) are obese.
5 Significant Findings: Obesity (Adults)! Recent research shows that obesity rates have doubled among adults and tripled among children in the U.S. These trends will affect life expectancy and obesity-related diseases like diabetes and heart disease.

Reinforces the need for community to embrace healthy lifestyles, encourage activity walking trails, green space, access to healthy foods, education, school lunch menus, tying into the goals of Vision 2030.

32 Reading Readiness of Our Children! Only 1 in 3 Georgia 3 rd Graders can read proficiently! Children who cannot read by 3 rd Grade are 4 times more likely to drop out of high school.! By age 5 a typical middle class child recognizes 22 letters of the alphabet, compared to 9 for children from low income families.! 61% of low income children have no children s books at home. * The Get Georgia Reading Campaign for Grade Level Reading.
33 UW s Read Learn Succeed Initiative As early as 18 months, low income children begin to fall behind in vocabulary development and other skills critical for school success. Parents play an enormous role, as do daycare providers, pediatricians, preschool programs and the broader community.
34 Reach Out and Read Model! Program geared to engaging community pediatricians and providers with parents and caregivers! Between the ages of 6 months and 5 years children are seen 10 times for an well or preventive visit.! Significant amount of counseling and anticipatory guidance around developmental milestones included in these visits.! Goal of the program is for the pediatrician to provide an age appropriate book at each visit and to engage parents in the importance of reading.
