2 Hepatitis B Vaccinations for patients with Chronic Kidney Disease or receiving Dialysis Introduction Patients with chronic kidney disease are at increased risk of hepatitis B virus infection due to the increased exposure to blood and other body fluids because of the process of dialysis. Your renal unit will have many processes in place to reduce the risk of exposure to the blood borne virus hepatitis B, but this risk cannot be completely removed. It is for this reason that the Department of Health recommends that all patients with kidney failure be vaccinated against the virus. Ideally patients are vaccinated before they start dialysis; if you start dialysis as an emergency, we suggest this is completed as soon as possible. The renal team will discuss with you when they suggest you start the vaccination programme and your chronic kidney disease or dialysis nurse will be involved in the administration of the vaccine. Benefits of being vaccinated against Hepatitis B The vaccination will encourage the production of antibodies so that your body is prepared to fight the virus if it enters the blood stream. By doing this, vaccination also prevents disease spreading and causing risk to others. For your protection it is recommended that the antibodies (anti- HBs) your body makes are ideally greater than 100mIU/mL. However, some protection is given if your antibody levels are miu/ml. After your first course of injections, your antibody levels will be checked using a blood test. If they remain at less than 100mIU/ml you may be offered either a further course of injections or a single booster injection. You will then have your antibody levels checked every year and a further single booster injection offered if required to keep 2 PI14_1232_02 Hepatitis B vaccination patient passport
3 you protected. Unfortunately, because kidney failure affects the immune system, some people will never develop any antibodies. These people should remain protected by the safe way we deliver dialysis in the UK but may be more at risk of picking up the infection if receiving dialysis in certain areas abroad. How is the vaccination given? A course consisting of three or four routine injections (dependent on the vaccine brand used) will be administered into the upper part of your arm, over a number of months. Your nurse will give you the dates you will need to return for your injections. If you are already on dialysis, the injections will be performed at your dialysis unit on a dialysis day. If you have a latex allergy you may need a particular brand of vaccine so please let the staff caring for you aware. Does the vaccine have any side-effects? As with any vaccine or drug, there will be some people who have a reaction or suffer side-effects. However this is rare. The most common are tenderness, redness, pain or swelling at the site of the injection or mild fever. These will only last a few days at the most. If you are concerned please either contact your GP or renal team at UHB.
