2 Celebrating 10 Years of Health Care Advocacy 2014 was a benchmark in the success of rural health care advocacy in Missouri. The Health Care Collaborative of Rural Missouri has been actively advocating for rural health care needs in our region for a decade. To date, HCC has secured over 6 million dollars in local, state, and federal grant dollars. HCC employs over 30 staff, with two wholepatient full service clinics that served more than 3,500 patients in A special Thank You! is extended to the HCC executive board for their support., as we recognize our successes are made possible by the support of the board, funders, supporters, staff, and the community.

3 MISSION: To cultivate partnerships and deliver quality health care to strengthen rural communities. Our goals: Monitoring the availability and delivery of health care service Ensuring that all residents are informed about available health and wellness services Advocating for the individual, care provider and making an institutional commitment to the health of our citizens Holding Lafayette County accountable for its commitment to the health of our citizens Providing a unified voice for Lafayette County that will address health needs with agencies and governmental entities beyond our boundaries We will meet these goals and objectives through: The many voices that speak to us regarding their health needs The right and responsibility of quality health A systems approach to quality, holistic health care Accessible, equitable and quality health care services A commitment to health education to improve mind, body and spirit The promotion of personal and organizational health care achievements Collaborations with those who share our values and goals A pursuit of constructive change and innovation

4 General Membership The focus of the Health Care Collaborative is to develop and implement programs that are responsive to documented health needs of county residents, with specific health status indicators as benchmarks of progress. The HCC implements and supports countywide, and in some instance, regional programs that help create access to health education, awareness, prevention and treatment services. The HCC works directly with these service providers: Lafayette Regional Health Center (LRHC), Samuel U. Rogers Health Center (SURHC), Carroll County Memorial Hospital, I-70 Community Hospital, Fitzgibbon Hospital, Pathways Community Health, and the Lafayette County Health Department (LCHD). Our goals include: Monitoring the availability and delivery of health care service Ensuring that all residents are informed about available health and wellness services Advocating for the individual, care provider and making an institutional commitment to the health of our citizens Holding our region accountable for its commitment to the health of our citizens Providing a unified voice for the region that will address health needs with agencies and governmental entities beyond our boundaries. Our general membership is encouraged to attend meetings at the HCC office held at noon on the second Tuesday of every other month.

5 Programs HCC continues to expand its capacity to develop, implement and/or partner with programs that promote access to quality health care, physical activity and access to fresh fruits and vegetables. Here are some of the projects currently underway: ACA Health Insurance Marketplace HCC has been approved as a Certified Application Counselor (CAC) organization for the Health Insurance Marketplace. Additionally, HCC has launched a series of informational sessions to educate west central Missouri communities about the Marketplace and has also scheduled several enrollment programs throughout the region to help residents and businesses enroll in the Marketplace. Access to Primary Care Program (APCP) What is the Access to Primary Care Program (APCP)? APCP, funded by private donations from the Odessa community, is a collaboration between the HCC of Rural Missouri, Lafayette Regional Health Center, Odessa Medical Clinic, Red Cross Pharmacy, Neighborhood Clinic in Bates City and the Live Well Community Health Centers of Waverly and Concordia. What services does APCP cover? APCP covers office visits for people without insurance and co-pays for people with insurance if they meet eligibility requirements. The program covers primary care visits only and first-time prescription medications (no refills). Urgent care for things like colds, flu and ear infections are also provided. Treatment for chronic illnesses are not provided. Who qualifies for APCP? Primary care services are available for adults and children. Individuals must reside in the Odessa R-VII School District and must meet income criteria. To determine eligibility, patients must provide an insurance card, if insured, one form of identification (i.e. driver s license, state ID, birth certificate) for themselves and/or dependents, as well as show proof of income and proof of address. Services are provided by participating partners: Live Well Community Health Centers of Waverly and Concordia, Lafayette Regional Health Center, Odessa Medical Clinic, Neighborhood Clinic in Bates City and Red Cross Pharmacy.

6 Programs Advocacy Collaboration. Plans are in development for a multi-state RFP program with other networks through a collaboration with N2N Strategies. Passage of SB262. Statewide effort to pass SB262 which will prohibit health carriers from denying coverage for health care services delivered via telehealth if the same service is covered when delivered in person. Health Information Technology (HIT) Meaningful Use. HCC HIT network partners have attested to Stage 1 Meaningful Use and have prepared to attest to Stage 2 Meaningful Use. Telehealth redesign. HCC is in the process of redesigning its telehealth network to ensure access for network members, partners and continuing education.

7 Programs Access to Health Care Live Well Community Health Centers of Waverly and Concordia. The federally qualified health centers are owned and operated by HCC and consist of a growing staff. The Live Well Community Health Centers are a collaborative effort to provide health care for people regardless of their ability to pay and will serve both the uninsured and insured individuals receiving Medicaid, Medicare, or private insurance. Services include the following: Primary Care Behavioral Health Care Dental Care Well Woman Exams Wellness Programs Immunizations Odessa Outreach Project. The Access to Primary Care Program provides free primary care and prescription medications to low income, underserved residents in the Odessa R-VII School District. Services are funded by the Odessa Community. Diabetes and Depression. The Telehealth Outreach Diabetes and Depression (TODD) program was offered to adults diagnosed with diabetes and/or depression at no out-ofpocket costs.

8 AHEC The West Central Missouri Area Health Education Center, hosted by HCC, receives federal and other grant funding to address the shortage of healthcare workers in rural and underserved areas. AHEC partners with HCC to provide health career awareness to students, provide community-based training for health professions students and continuing education for providers as well as helping to recruit healthcare providers to the area. In 2014, AHEC hosted the University of Missouri School of Medicine SIM-VAN which provided hands-on training for LPN students from Lex-La-Ray Technical center and is providing online curriculum used in the Lexington School District s Health Occupation CNA class. AHEC assisted in recruiting two dentists for the new Live Well clinics in Waverly and Concordia. AHEC has provided school-based and after-school programming for area high school students interested in health careers. West Central AHEC serves four counties including Jackson, Johnson, Cass and Lafayette. Student Nursing Program Telehealth. Development is underway for provider-based tele-health video education through a partnership between HCC and AHEC (Area Health Education Centers). Telehealth continuing education was provided through MU Telehealth in early HOSA. AHEC/HCC are planning for a Health Occupations Students of America (HOSA) program in west central Missouri.

9 Programs West Central Missouri HealthTran The Health Care Collaborative of Rural Missouri received a planning grant to develop a new model of service delivery that includes a significant focus on eliminating the barriers that prevent people from accessing timely primary and preventive care, particularly transportation. HCC will enhance community and partner relationships to expand network participation to include rural public transportation. The activities proposed in this planning grant include identifying ways to encourage cross-organizational collaboration and leadership commitment, identifying opportunities for the network to better address regional and/or local population health needs, and identify and establishing ways to obtain regional and/or local community support/buy-in around the continued development of the network. Cultural Competency and Health Literacy HCC seeks to provide training for staff and others to create a more cohesive, inclusive, and competent health care industry. The HCC continuously works to improve their level of culturally competent and health literate programs and services brought with it a dedicated staff member to the cause. We realize that, by understanding a patient s values and beliefs, language, communications, actions, and socioeconomic status, we will play an important role in enabling them to achieve their best health outcomes. Having culturally competent providers and health home staff who coordinate, manage and offer services that are respectful and responsive to the cultural and linguistic needs of our diverse, rural, low income and underserved patients will have a positive effect not only on the patient but their family and community. HCC s focus on health literacy and cultural competency includes the training of staff and additional community presentations. A collaborative project with the Kansas Head Start program to conduct outreach for Parent Health Literacy classes has helped educate families on their children's health in the 2014 annual year.

10 Programs Rural Health Initiative The Lafayette County Rural Health Initiative team continued its effort to expand capacity and access to medical services to residents. The RHI Core Team focused on four working groups: a mapping group who surveyed 146 Network members to determine connections; community connector who developed a cadre of connectors to assist consumers with service referrals; an Affordable Care Act team to educate and enroll those residents of Lafayette County without at least minimal coverage health insurance; and a Theory of Change group that is working closely with the REACH Technical Assistance Team to develop a new way of accomplishing goals. In March 2014, the Health Care Collaborative of Rural Missouri opened its second Federally Qualified Health Center in Concordia which now provides primary care, dental services, and behavioral health services for everyone, whether they have private, public, or no insurance. If someone cannot pay, arrangements are made for payment plans, no one is turned away. These services were made possibly by funding from REACH and other entities. We recently received notification that our funding request for 2015 has been approved. Our Live Well Community Health Centers are planning an initiative to launch a new school-based clinic initiative. There are six public school districts in Lafayette County and in 2015 we hope to extend services to these schools through mobile outreach activities, face-to-face activities and virtual (telehealth) activities. Major activities for 2015 include an oral health outreach day for each district, which includes education, screenings, and care coordination. The RHI project will provide stipends to school personnel for their time dedication to the project.
