2 Medically Indigent Program (MISP) Provided Contact Information Nason Street, Suite 102 Moreno Valley, CA Eligibility Requirements This program covers acute illnesses and assistance for the health needs to prevent disability for adults years. The goal of the program is to reduce costly hospitalization and increase the ability to work. MISP is NOT insurance. It is for adults who suffer traumas or have other emergent needs. Acute illnesses Medical care to prevent disability Prescriptions that are on the Medi-Cal formulary are covered. Dental is available through authorized providers listed in the MISP brochure. Riverside County resident for at least 30 days, ages 21-64, must not have access to health insurance or health plan and not a resident of a skilled nursing home or institution for the mentally disabled. Proof of Riverside County residency for at least 30 days Complete an MISP application Adult between the ages Must not be insured or have access to a health plan Must be able to show proof of income Must show proof of identity Must not have a valid Visitor s visa Must not be in Riverside County for the sole purpose of rehabilitation Must not be institutionalized in a Skilled Nursing Home (SNF) or Institution for the Mentally Disabled (IMD) Call Persons who meet the eligibility may be granted up to twelve months of eligibility. They must report changes in income, living circumstances or if they apply or become eligible for other health benefits. Failure to do so can result in revocation of eligibility

3 to the date of change in status. The County of Riverside may review eligibility status on a random basis during the twelve month period. MISP can only be accessed after you have exhausted all other coverage options, including Medi-Cal, Social Security Disability (SSD) and Supplemental Security Income (SSI). MISP requires that you apply for all programs that you may qualify for as a condition of your continued MISP eligibility. Clinic hours vary by week based on provider schedule and availability. The clinic is closed on Sunday and most Mondays. Basic Dental is available. Fees are on a sliding scale based on proof of income. Call and set an appointment. There is a $50 charge for the first appointment.
