If enrollment falls below this number, the class may be cancelled, and a prorated credit will be applied. A thirty day WRITTEN notice is required if your child no longer wishes to be enrolled in a class. No refunds will be given for prepaid classes. Classes, times, performances and instructors are subject to change. Choose to save by paying tuition in full at the beginning of the semester, or pay via each month. These are the only forms of payment accepted. Upon registration there is an annual registration fee of $50 due per student. We offer a tuition discount if entire semester cost is paid on or before the first day of class. NOTE: The discounted price does not apply to private voice/dance lessons, and is non-refundable.

4 Monday Schedule Classes Begin on August 15 th Select Show Choir 4:30-6:25 PM Grades 9-12 By Audition Only BY AUDITION ONLY. Auditions will be held Tuesday, August 9 th from 4:30-7:30 at ACT: S. 48 th Street, Suite B-101, Phoenix, AZ Join us Sat, August 6 th from 1:00 3:00 at ACT for an audition workshop! Cost is only $20! (The Workshop is NOT Mandatory) Select is a one-year commitment (Fall and Spring Semesters). Select is a HIGH LEVEL show choir open to grades 9-12 by audition only. Focus is on harmonies and dance in a fast paced, high commitment level group. All community performances and all rehearsals are MANDATORY. This is a two-semester (Fall and Spring) commitment class. This year the group will be traveling to Hawaii for AMAZING performance and educational opportunities tentatively from March 5-9, Students will room with each other, 4 students per room. Parents may join us, but will room with other adults. Classes will end in March of 2017 Tuition: $446 per semester or $416 if paid in full at registration and again in January. * ADDITIONAL COSTUME & TRAVEL FEES ARE THE PARENT S RESPONSIBILITY. High School Studio Series 6:30 PM 8:00 PM Grades 9-12 Grades 9-12 please join us as we produce Footloose for our Studio Series. This class is a two-semester class commitment (Fall and Spring). Final Parent Showcase will be performed at a professional venue in April. Tuition: $435 per semester or $405 if paid in full by first day of class. * ADDITIONAL SHOE COSTS ARE THE PARENT S RESPONSIBILITY. * PARENT SHOWCASE TICKETS $15 EACH.

5 Tuesday Classes Classes begin on Tuesday August 16 Treble Makers Show Choir 4:30 PM 6:25 PM Grades 5-9 By Audition Only BY AUDITION ONLY. Auditions will be held on Tuesday, August 9 th from 4:30-7:30 at ACT: S. 48 th Street, Suite B-101, Phoenix, AZ Join us Sat, August 6 th from 1:00 3:00 at ACT for an audition workshop! (The Workshop is NOT Mandatory) Treble Makers is a one year commitment (Fall and Spring Semesters) Treble Makers is a middle and high school show choir This is a two-semester (Fall and Spring) commitment class. This Choir performs in the community and travels as well. We focus on 3 part harmonies, blending, performance and dance. This year the group will be traveling to HOLLYWOOD, CA. Our tentative dates of travel for this trip are from June 4 to June 8, All students will room with other students, four per room and WILL NOT be permitted to room with a parent. If parents would like to join us on the trip, they may, but will be rooming with other adults. Please keep this in mind when auditioning! Classes will run through May, until the trip date in June Tuition: $520 per semester or $490 if paid in full at registration and again in January. * ADDITIONAL COSTUME & TRAVEL FEES ARE THE PARENT S RESPONSIBILITY. Grades 6-9 Studio Series 6:30 PM 8:00 PM Grades 6-9 please join us as we produce Camp Rock for our Studio Series. Please note that this class is a two-semester class commitment (Fall and Spring). 6 Wednesday Schedule Classes Begin on August 17 th Studio Series (Production Based) 4:00 PM 4:55 PM Grades K-3 Studio Series classes are designed to allow your child to participate in an age-appropriate musical theatre show. All scripts, music and costumes are included in the cost of the class (with the possible exception of some footwear). The class will produce a musical of Sleeping Beauty, to be performed for parents at a professional venue in April. Tuition: $360 per semester or $330 if paid in full by first day of class. * SAVE BY COMBINING THIS CLASS WITH WEDNESDAY S ALL KEYED UP CLASS. * PARENT SHOWCASE TICKETS $15 EACH. All Keyed Up Show Choir 5:00 PM 5:55 PM Grades K-5 Does your child love to sing and dance? Are they natural performers? Would you like to foster and support that love and talent? Then All Keyed Up Show Choir is for you! Open to grades K 5 will give your child a real show choir experience. This group may perform at local community events during the semester. There may be solo opportunities in both singing and dancing. It s a MUST TAKE for those children who strive to get into our upper level middle and high school show choirs. No matter what your child s aspirations, ALL KEYED UP will feed their desire to sing, dance and perform in a nurturing and FUN group environment! 10 I agree to pay tuition in full at the beginning of the the semester OR pay the monthly payment on (keyword: Ahwatukee)the first week of class each month. I agree to pay a $10 per week late fee. If tuition plus applicable late fees are NOT paid in full by the third week of the month, I understand that my child WILL NOT be admitted into class, will be removed from the class roster and will be replaced by someone on the waiting list for that class. I understand that there is a $25 fee returned payments. I understand that tuition is based on a semester basis and have reviewed the payment options. All classes are TWO semesters long a one year commitment. Unless paid in full, tuition must be paid via (keyword: Ahwatukee) the first week of class each month, regardless of the number of classes per month. I understand that a 30 day WRITTEN notice is required for dropping a class. If this notice is not received, your account will continue to be billed and collection will occur until the notice is received. If a class is cancelled due to lack of participation, every attempt will be made to reassign your child to another class or your money for the month of cancellation will be refunded. I understand that it is the responsibility of the parent, guardian, or car-pool partner to check the parent information area and my personal s for ACT information. Flyers may occasionally be sent home with my child and it is my responsibility to check to be sure I am up to date on communications. Updates will be ed to me and I will be responsible for notifying ACT of any phone number or address changes. I understand that there may be additional fees for chorus/choir t-shirts, sweatshirts, dance shoes, costumes, showcase tickets, travel, etc. depending on the class my child is enrolled in. I understand that my child may not be dropped off earlier than 10 minutes prior to class start time, or picked up later than 10 minutes after class end time. Each additional minute will be billed at a rate of $1 per minute, unless prior arrangements have been made with the front desk staff.
