3 SIGNATURES REQUIRED IN FOUR PLACES BELOW ENTRIES MUST BE SIGNED TO BE ACCEPTED CAREFULLY READ THIS AGREEMENT BEFORE SIGNING By entering this competition and signing this entry blank as the owner, lessee, trainer, manager, agent, coach, driver, handler and on behalf of myself and my representatives, employees and agents: I agree that I am subject to the Constitution and Rules of the competition. I represent that I am eligible to enter and/or participate under the rules and every horse I am entering is eligible as entered. I acknowledge that the rules applicable to the competition have been made available to me and, if applicable, my minor child, and that I am familiar with them. In consideration of the acceptance of this entry and of the holding of the competition and of the opportunity for me and/or my minor child to win prize money, ribbons or trophies, I agree to defend and hold the competition, the named facility, Hansen Dam Equestrian Center, any other organizations that have officially recognized this event and the directors, officers, sponsors, members, employees and agents thereof, harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of my or my minor child s horse while in or upon the competition premises or grounds or near any entrance thereto and I personally assume all responsibility and liability for any such claim, and I further agree to hold the aforementioned parties harmless from any claim for loss of my or my minor child s horse by disappearance, theft, damage or injury caused or alleged to be caused by the negligence of the competition organization or any of the other parties aforementioned or by the negligence of any other person or any other cause or causes. I hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expense including legal fees, by reason of the liability imposed by the law upon any of the aforementioned parties for damage because of bodily injury, including death at any time resulting there from, sustained by any persons, including myself and my minor child or on account of damage to property arising out of or in consequence of my or my minor child s participation in this competition, howsoever such injuries, death or damage to property may be caused, and whether or not the same may have been caused or may be alleged to have been caused by the negligence or the aforementioned parties or any other parties. All minors/juniors must be supervised by their parent/guardian or authorized agent at all times while at or on the show grounds, whether riding or on foot. The parent/guardian or authorized agent shall assume all responsibility to protect juniors from any injury, damage, harm and/or loss and shall indemnify, defend and hold the management harmless there from. I hereby represent and agree that in the event that the entries hereby made for and on behalf of an exhibitor under the age of 18 years that I am one of the parents of such minor, or duly appointed legal guardian of such minor and as such entitled to make this entry for and on behalf of such minor, and I further hereby represent and agree that in the event that the entry hereby made for and behalf of another person other than a minor under the age of 18 years, I have full authority and privilege from such other person to make such entry for and on behalf of such other person. I AGREE in consideration for my participation in this competition to the following: I AGREE that I choose to participate voluntarily in the competition with my horse, as a rider, driver, handler, lessee, owner, agent, trainer, or as a parent or guardian of a junior exhibitor. I am fully aware and acknowledge that horse sports and the competition involved in inherent dangerous risks of accident, loss, and serious bodily injury including broken bones, head injury, trauma, pain, suffering or death ( Harm ). I AGREE to release the competition from all claims for money damages or otherwise for any Harm to me or my horse and for any harm caused by me or my horse to others, even if the harm resulted directly or indirectly from the negligence of the competition. I AGREE to expressly assume all risks of Harm to myself and my horse including Harm resulting from the negligence of the competition. I AGREE to indemnify (that is, to pay any losses, damages, or costs incurred by) the competition and to hold them harmless with respect to claims for harm to me or my horse, and for claims made by others for any harm caused by me or my horse at the competition. I have read the CDS rules about protective equipment and I understand that I am entitled to wear protective equipment without penalty, and I acknowledge that the competition strongly encourages me that I do so. If I am a parent or guardian of a junior exhibitor, I consent to the child s participation and agree to all of the above provisions and agree to assume all of the obligations of this release on the child s behalf. I AGREE that the Competition as used above includes all of their officials, officers, directors, employees, agents, personnel, volunteers and affiliated organizations.
