3 General Policies Collars: All dogs are required to wear a cotton or nylon collar while at Puppy Haven. No leather or chain collars allowed. Leashes: All dogs must be brought in and out of the facility on a leash for their safety and others. Puppy Haven is not liable for leashes left in our lobby. Behavior: In order to maintain the safest playgroups possible, certain playroom etiquette is enforced and required. If a dog shows aggression towards a handler or dog, the staff will evaluate whether he/she will be dismissed from daycare. Other habits such as humping, stalking, obsessive barking, chasing and rough play will also be monitored and could result in dismissal. Hours of Operation: Monday-Thursday from 7am-7:30pm. Friday from 7am-7pm. Saturday from 9am-4pm. Sunday from 12pm-4pm. A $15 late fee will be charged for dogs left in the facility up to 15 minutes after closing. Dogs not picked up will be boarded and charged our nightly rate. Holiday hours vary and are listed on our website and posted in the store. Wellness: There are certain risks associated with allowing your dogs to play with other dogs. When dogs play together, the following are possible: bites, punctures, scratches, broken bones, bruises, transmission of disease, etc. All owners are responsible for their pet. If we notice a dog is sick, injured or has other ailments, we will immediately contact the owner and request that the dog be picked up at the earliest convenience either by the owner or by an emergency contact. Vaccinations: All dogs must be kept current on vaccinations. Puppy Haven allows a seven day window from the time of expiration. After that, no exceptions can be made. It is your responsibility to provide ongoing verification of current vaccinations. Temperament Test: All new dogs must pass a temperament test to ensure they fit in our open play environment. The test is a minimum of 4 hours, but you are welcome to leave your dog all day. The entire day is complimentary and reservations are required. We ask that dogs are dropped off between 7-10am Monday-Thursday or 9-10:30am on Saturdays. We do not perform temperament tests on Fridays or Sundays. Passing the temperament test does not ensure that a dog who exhibits changes in behavior in the future will not be dismissed. No daycare or boarding reservations can be made until the test is completed. Boarding: We ask that you provide food while your dog is boarding with us to not change their diet. Food for boarding must be divided into separate meals prior to drop off at Puppy Haven. All food must be sealed. Alternatively, house food is available for $4 per meal. Elevated cots and bedding for each boarding dog is provided. Personal bedding is discouraged. Boarding Cancelation Policy: Reservations may be canceled up to 48 hours prior to the day of the reservation and the deposit will be refunded in full. Deposits for cancelations made 48 hours or less prior to the date of the reservation will not be refunded, but left as a credit. Deposits for holiday reservations are left as a credit on your account if a reservation is canceled. Daycare: Daily daycare reservations are required following the Temperament Test. Cancelations need to be made 24 hours in advance. Excessive no shows may result in a fee. Grooming: All grooming pets must complete an Enrollment Application and provide proof of vaccinations. Baths are a $30 flat fee. Social Media Policy: Puppy Haven s Twitter, Facebook, Instagram, website and promotional items may include photos of your dog. 5 Veterinary Care and Expense Releases and Liability Waiver If in the sole discretion of Puppy Haven or any of its employees it is determined that my pet requires veterinary care while in the care of Puppy Haven, I hereby grant permission for Puppy Haven to seek medical attention for my pet. I understand that I am solely responsible for the payment of all veterinary or related charges incurred for the care and treatment of my pet. I hereby release Puppy Haven, its owners and employees from any responsibility, claims, damages, or debts related to the adequacy, quality of care or the professional judgment of the veterinarian or his or her staff related to any such veterinary care. This includes, but is not limited to, transportation to/from the veterinarian clinic and choice of veterinarian or animal hospital. If transportation of my pet is required in case of a medical emergency, I understand that Puppy Haven staff will try to ensure the safest trip possible. I acknowledge and accept that there are certain risks involved with pet transportation including injury resulting from auto collision (fault or no fault), escort in and out of Puppy Haven property, and other risks associated with the any vehicles owned by Puppy Haven employees that may be used to transport my pet. By my signature below, I agree to hold Puppy Haven, its employees and any parent or related company harmless from any and all liability arising from providing any such emergency veterinary care or medical service to my pet. I intend that this grant of permission is in full force and effect for any time after the below date that my pet is receiving any service from Puppy Haven, including but not limited to daycare, boarding, grooming or any other service offered by Puppy Haven. In consideration of my doing business with Puppy Haven, including but not limited to my allowing Puppy Haven to care for my pet, I hereby waive any claim of liability or responsibility against Puppy Haven that I may now have or that I may acquire in the future and agree to hold harmless and covenant not to sue Puppy Haven, its parent and agents, officers, subcontractors, employees, and any other related parties or entities from any and all liability for injuries to myself, my pets, or my personal property which may arise from or be associated with any of the services provided by Puppy Haven. I understand that there are certain risks involved with pet ownership, boarding, training, and care including, but not limited to, fights between pets, animal bites, and the transmission of disease. By my signature below, I acknowledge that these risks are inherent in pet care and boarding, and I hereby agree that I will not seek to hold Puppy Haven responsible for any injury that may occur to my dog or to any other personal property while on the premises on which Puppy Haven is doing business. This waiver of liability, covenant not to sue and release includes any injury to my pet or person that that might occur, no matter the cause, including illness that would have normally occurred had the pet not been at Puppy Haven.
