1 Community Health Needs Implementation Strategy FY FY2015

2 FY2013-FY2015 Community Health Needs Implementation Strategy Overview Located in Talladega, Alabama, Citizens Baptist Medical Center is part of Baptist Health System, a faith-based, not-for-profit health care system consisting of four hospitals and a network of health centers serving north and central Alabama. With 122 beds, Citizens Baptist Medical Center is a major provider of health care services in Talladega County and the surrounding area. Citizens is dedicated to the Baptist Health System s faith-based ministry, focusing on whole-body wellness for all patients. This report includes the FY2013-FY2015 plans for Citizens Baptist Medical Center to address the prioritized health needs identified in the Community Health Needs Assessment report. The full Community Health Needs Assessment report is provided in a separate companion document. Target Areas and Population As described in the Community Health Needs Assessment report, Citizens Baptist Medical Center serves the broader Talladega County region, which includes a population of approximately 41,000. The majority of the population is White (61.7%), followed by African American (33.8%), and Hispanic (2.4%). Current estimates project an increase in number of older adults, with those aged 65+ years expected to grow 15.9% over the next five years. Identified Health Priorities The FY 2013 Community Health Needs Assessment for Citizens Baptist Medical Center was conducted using both statistical data on the current health status of the community as well as interviews from individuals who provided key insights into the broader health interests of those served by Citizens Baptist Medical Center. The resulting health needs were complied and ranked using a system that incorporated comparative methods with those health needs meeting high or medium criteria deemed as of greatest priority. Subsequently, 6 health needs were identified as meeting high or medium criteria and are listed with a brief description below: 1. Cardiovascular Disease: Heart disease is the leading cause of death and cardiovascular disease was commonly mentioned as a health concern by individuals in the community. 2. Diabetes: Diabetes was commonly mentioned by members of the community as a contributing factor to the overall perception of poor health. CBMC Version 1 - June 2013 Page 2 of 6

Health Care: Members of the community expressed concerns over the perception of limited primary care resources including long wait times and over use of emergency services. 4. Maternal and Child Health: The infant mortality and low birth rates in Talladega County are higher than HP 2020 goals. 5. Nutrition: Members of the community mentioned a lack of knowledge regarding health education including healthy eating as a concern. 6. Overweight/Obesity: More than one-third of adults in Talladega County reported a BMI greater than or equal to 30. Lack of physical activity and obesity is a major contributing factor to other chronic conditions and was mentioned as a major concern by members of the community. Development of Implementation Strategy Following a comprehensive review of the Community Health Needs Assessment report, the Citizens Baptist Medical Center leadership team was responsible for determining the best approach to address the identified health priorities. The resulting Implementation Strategy links specific activities to achieving targeted outcomes for each priority s specified goals and objectives. The activities in the Implementation Strategy including existing programs and partnerships as well as new activities aimed at enhancing the benefits Citizens Baptist Medical Center provides to the community. Additionally, rationale was provided for any health priorities that will not be address at this time (if applicable). Implementation Strategy FY FY2015 The following Implementation Strategy details the plans for Citizens Baptist Medical Center to address the health priorities over the next 3 years: CBMC Version 1 - June 2013 Page 3 of 6

4 FY FY2015 Community Health Needs Implementation Strategy Health Priority Cardiovascular Disease Diabetes Goals, Objectives, Strategies, and Partnerships To improve community awareness of the signs and symptoms related to cardiac events (heart attack, heart failure, etc) and to promote participation in initiatives aimed the early detection and prevention of cardiovascular disease Baptist Health System Workforce Wellness program; Walgreens; Coosa Valley Electric Cooperative; Investigate potential opportunities for additional partnerships To promote healthy lifestyle choices as a method of reducing the risk factors associated with diabetes Baptist Health System Workforce Wellness program; Lincoln Elementary School Activities Performance Outcomes Support the Baptist Health System Workforce Wellness program initiatives for Hospital employees and community partners aimed at identifying and reducing potential cardiovascular disease risk factors for Hospital employees and community partners including - Blood pressure screenings, smoking cessation classes, discounted memberships to Weight Watchers, enhanced access to gym/physical fitness activities, and health eating/nutritional education Continue to support and participate in the Walgreens Million Heart Campaign Promoting and conduct cardiovascular/heart health screening at Talladega store location Provide educational material on the importance of healthy living and the potential benefits associated with reducing risk factors associated with cardiovascular disease Continue to partner with Coosa Valley Electric Cooperative to support the CHAPS program providing free health screenings throughout the community Investigate potential opportunities to partner with community organizations on initiatives and programs aimed at improving cardiovascular health in the community including Additional educational sessions for high risk populations; Partner with nursing homes and/or community/civic organizations on educational activities focused on cardiovascular disease prevention Potential outreach program aligned with AQAF initiatives promoting quality improvement initiatives for cardiovascular disease Continue to provide nutritional counseling and education to all newly diagnosed diabetes patients Continue to partner with the Lincoln Elementary School and provide ongoing support and equipment for their Project Fit America Program which encourages student to complete the Presidential Physical Fitness Test requirements for the achieve physical fitness Support the Baptist Health System Workforce Wellness program which provides Hospital employees and community partners with health screenings and access to healthy living initiatives including Glucose screening, smoking cessation classes, discounted memberships to Weight Watchers, access to Hospital gym, and health eating/nutritional education participating in cardiovascular health related screenings and activities through the Baptist Health System Workforce Wellness program screened during Million Heart Campaign Educational material provided during Million Heart Campaign Number of health screenings provided through CHAPS program participating in diabetes related screenings and prevention activities through the Baptist Health System Workforce Wellness program Resources provided to the Lincoln Elementary for Project Fit American Program Increased community awareness of the signs and symptoms of cardiac events and the importance of cardiovascular health Increase in educational activities aimed at reducing the burden of diabetes in the community CBMC Version 1 - June 2013 Page 4 of 6

5 Health Priority Goals, Objectives, Strategies, and Partnerships Activities Performance Outcomes Health Care To support active initiatives aimed at improving access to health care services in the community Baptist Health Centers Support efforts to expand access to primary care by promoting Hospitalist program to community physicians Provide collateral material to local physicians Promote and increase awareness of the health care services offered through the Baptist Health Centers Rural Health Clinic (RHC) Financial assistance/reduced rates for underinsured and uninsured patients Investigate other opportunities and partnership that will continue to expand access to health care the community Resources used to promote Hospitalist program Number of patients utilizing Baptist Health Centers Rural Health Clinic resources Improved communication regarding available resources and access to health care services in the community Maternal and Child Health Citizens Baptist Medical Center will not directly address the expressed needs relating to Maternal and Child Health Limited resources will prevent Citizens Baptist Medical Center from addressing this health priority. Additionally, the following existing programs provided by the Talladega Health Department are currently being provided which address Maternal and Child Health: Assistance obtaining WIC (supplemental nutrition) and education on child nutrition; Family planning services including education on infant care Not directly addressed at this time Support the Talladega County Health Department s efforts to improve maternal and infant health Overweight/ Obesity To support activities that promote physical activity and the importance of healthy lifestyle choices in the community Baptist Health System Workforce Wellness program; Talladega County Nutritional Service Board; Investigate potential partnership with local Schools Support the Baptist Health System Workforce Wellness program initiatives for Hospital employees and community partners aimed at identifying potential health risk factors and encouraging physical activity including Glucose screening, BMI testing, smoking cessation classes, discounted memberships to Weight Watchers, access to Hospital gym, and health eating/nutritional education Continue to partner with the Lincoln Elementary School and provide ongoing support of the Project Fit America Program and their efforts for students to complete the requirements for the Presidential Physician Fitness Test Continue to sponsor Shocco Springs 5K run & Lincoln Elementary 5K run participating in Baptist Health System Workforce Wellness screening and activities to improve nutrition and physical activity Number participants in sponsored races Number of school teams sponsored Increased physical activity among residents Talladega County Partner with the Talladega County Nutritional Service Board to provide assistance developing nutritional guidance to promote overall healthy eating habits among school aged children in the community Investigate additional opportunities to provide support to local schools by sponsoring team sport activities CBMC Version 1 - June 2013 Page 5 of 6

6 FY2013-FY2015 Community Health Needs Implementation Strategy Ongoing Monitoring To ensure the health priorities are being addressed, the leadership team will review and update the Implementation Strategy in an ongoing manner beginning with its formal adoption through FY2015. Updates to the plan may include revisions to existing activities as well as additional programs and partnerships that further the goals to address each health priority. The next scheduled Community Health Needs Assessment will be conducted for FY2016. Approval The Community Health Needs Assessment report and FY2013-FY2015 Implementation Strategy were reviewed and approved by the Baptist Health System Quality and Missions Committee. Any changes to the Implementation Strategy will be provided for review to the Quality and Missions Committee and incorporated into this report annually. Copies of the Citizens Baptist Medical Center Community Health Needs Assessment and Implementation Strategy are available to the public online and at the Hospital by request.
