1 LAKE-SUMTER COMMUNITY COLLEGE NURSING DEPARTMENT INTRODUCTION Thank you for your interest in the nursing program at Lake-Sumter Community College. The program graduated its first class in It has developed to accommodate the ever-increasing demand for health care professionals in the workforce. Program graduates are eligible to sit for the NCLEX-RN, the state licensing exam required to become a registered nurse. LSCC graduates have traditionally experienced high pass rates on this exam. As a result the program enjoys an excellent reputation among local employers. The program is currently approved by the Florida State Board of Nursing (BON) as well as accredited by the Southern Association of Colleges and Schools (SACS) and the National League of Nursing Accrediting Commission (NLNAC)*. The program offers a choice of two tracks for completion. The Bridge track (55 Credits) provides a one year option for students who are currently licensed practical nurses (LPN s) or who are currently certified as paramedics in the state of Florida. Students who are not LPN s or paramedics must complete the two year General track (72 Credits). Completion of program prerequisite and nursing courses will culminate in an Associate of Science degree in Nursing. Additionally, the UCF ADN-BSN Concurrent Track provides students the opportunity to begin work toward the Bachelor s in Nursing Degree while enrolled in LSCC ADN program. Theory classes are offered on the Leesburg and South Lake campuses. Class days and times will vary as you progress through the program. In addition there will be on-campus skills lab requirements as well as off-campus clinical requirements that may occur on various days of the week. Further time will be invested in multi-media lab assignments, required community service, and student organization activities. Although the listed course credit total for some semesters is less than 12, participation in the program is considered to be fulltime. LSCC does not offer a part time nursing program. It is important to recognize the nursing program requires more commitment from you for non-class time requirements than you have encountered in your previous educational experience. The information contained in this packet is intended to acquaint you with the admission/selection process for the Nursing Program as well as provide supplemental information that may affect your application. Procedures included herein are subject to change. It is the student s responsibility to remain apprised of information and policy updates. Students must submit official transcripts to LSCC Admissions Department for credit transfer for courses not completed at LSCC. 7 INFORMATION SESSION As soon as students have an interest in nursing they must complete the following activities: Declare Pre-Nursing Major with College Admissions Department. Establish Nursing Department File at Mandatory Information Session. Attendance at an information session is mandatory in order to establish nursing department file and to make application to the department. Sessions are held on both the Leesburg and South Lake campuses as per the schedule indicated on the nursing program website at SELECT Future Students. SESSIONS ARE OPEN SEATING- NO APPOINTMENT IS NECESSARY. It is disruptive and disrespectful to presenters and fellow prospective nursing students to arrive late at sessions. Therefore, attendees are expected to be seated by the start time of the session. If you cannot arrive on time you should seriously consider attending an alternate session as attendees are responsible for all information presented. SELECTION PREFERENCE Florida community colleges are responsible to uphold the provisions of the Charters granted them by the state of Florida. Each college is assigned a service district in its Charter along with the provision that residents of that service district are to receive services prior to non-residents. The service district for Lake-Sumter Community College includes the counties of Lake and Sumter. Therefore, available seats are filled according to the following preference: Former LSCC non-completers who have been granted re-admittance into the program. Qualified applicants who are legal residents of Lake or Sumter Counties (residents). Qualified applicants who are not legal residents of Lake or Sumter Counties (nonresidents). Qualified transfer students according to waiting list position and seating availability. Address verification is confirmed through Background Report contents. Proof of Lake or Sumter County residence may be required. 5

8 SELECTION CRITERIA Applications which are complete and meet minimum selection requirements will be forwarded to the selection committee for review. Incomplete applications or those which do not meet minimum requirements will not be considered in the selection process. 9 MINIMUM SELECTION REQUIREMENTS for both tracks are as follows: GPA overall as determined by LSCC Admissions Department calculation. HESI- 1) score of 80 in math section 2) score of 76 in each of reading comprehension, vocabulary and general knowledge, grammar sections 3) achieve average language score of 80 [scores in reading comprehension, vocabulary and general knowledge, grammar sections are totaled, then divided by 3] 4) achieve cumulative score of 160 [total of math and average language scores]. SELECTION FORMULA- The selection formula used to determine which applicants will receive acceptance offers into the program consists of 1) base score 2) accumulation of extra points toward total score. 1) The base score for both tracks is calculated as follows: [(overall college GPA)(60%)][(HESI cumulative score/2)(40%)] 2) Extra points are accumulated as follows: GPA- 1) 3.0 to 3.49 earns 1 point 2) 3.50 to 4.0 earns 2 points. Maximum award of 2 points SCIENCE PREREQUISITE COURSES- A&P I, A&P II, Microbiology. For each of the listed courses completed at time of application 1) grade of B earns 1 point 2) grade of A earns 2 points. Maximum award of 6 points. (When multiple course attempts are present on transcript the most recent grade will be used in the award of extra points.) HIGH SCHOOL HEALTH OCCUPATIONS- Documented completion of this 3 course series earns 1 point for completers in Lake and Sumter high schools. Eligible students must present documentation of completion to Manager of Workforce Services, Tech Prep located in SSB 228 on Leesburg campus, , within 18 months of completion. The sum of accumulated extra points is added directly to the base score resulting in total score for application. 7

13 PHYSICAL EXPECTATIONS The Lake-Sumter Community College Nursing Program holds as its ultimate goal the production of graduates who will be capable of attaining RN licensure as well as meaningful employment as nurse generalists. In order to achieve this goal nursing students will be required to demonstrate competencies in theory courses as well as in clinical courses. In order for nursing students to progress through the clinical portions of the program they will be required to show competency in prescribed sets of tasks. These tasks are accepted to be among the regular duties of a generally practicing registered nurse. The following is a list of physical expectations necessary in order to accomplish the tasks needed for success in the program as well as subsequent employment as an RN. Prospective nursing students should review the list to determine if they are good candidates for the program and for a nursing career. + Walk and stand most of the work day. + Lift and carry 50 pounds without assistance. Push or pull 100 pounds. + Reach overhead, twist, bend, crouch, stoop, kneel, climb, balance. + Handle, finger, pinch, grip, hold objects and possess ability to keyboard. + Good field of vision including near/far acuity and good depth perception. + Track, see, and hear in an environment that is not totally distraction free. + Work extended periods without relief. Prospective nursing students who believe they have limitations in any of these areas and wish to apply to the nursing department must register with the Office of Students with Disabilities (OSD) located on the Leesburg Campus in the Student Services Building. 14 ADMISSION ASSESSMENT EXAM (HESI) What is the HESI?? The HESI (Health Education Systems Inc.) Admission Assessment exam is a computerized nursing entrance test designed to measure prospective nursing students readiness to succeed academically in nursing programs. The HESI consists of ten sections. As an LSCC nursing program applicant you will complete 6 sections. 4 sections will be scored*. 2 sections are for information purposes and for your future use within the program**. *Math contains basic operations, decimals, fractions, etc. Calculators are not allowed. Scratch paper will be provided for you. There are approximately 50 items in this section. *Reading Comprehension contains reading scenarios related to healthcare topics. There are approximately 38 items in this section. *Vocabulary and General Knowledge contains basic vocabulary used in the health field. There are approximately 50 items in this section. *Grammar contains basic grammar for correct English writing format. There are approximately 50 items in this section. **Learning Styles assesses preferred learning style. There are approximately 14 items in this section. **Personality Style assesses personality in relation to preferred learning style and offers printout of study tips based on the relationship. There are approximately 15 items in this section. What is the procedure for taking the HESI?? The test is offered on the Leesburg and South Lake campuses by The Learning Center according to posted schedule. Prior to sitting for the test you must a) be a student at LSCC or pay the administration fee b) pay the HESI fee of $32.00 to cashiering c) register for your preferred test day and time through The Learning Center s web page Total Testing Time allowed is 3 hours. Students work through the sections in any order and at their own pace during the 3 hour time period. (Average completion time is 2 to 2 ½ hours.) It is strongly recommended that you prepare before taking the test. The Admission Assessment Study Guide is available for purchase directly from HESI at Copies are also held on reserve at the Libraries and The Learning Center on Leesburg and South Lake campuses. The LSCC Learning Center provides excellent preparation activities. HESI exams taken at other institutions: A certified copy of scores sent directly from the institution to the LSCC nursing department will be accepted in lieu of testing at LSCC. Scores must meet validation requirements below. What happens after I complete the test?? The results of the HESI will be printed as soon as you complete the test. You will receive a copy of your scores. A second copy of your scores will automatically forward to the Nursing Department. HESI scores are valid for application to the Nursing Department for 2 years. If your scores will be older than 2 years on the nursing department application return deadline of March 1 st you will need to retake the test. Scoring is based on the results of the four sections indicated above. In addition to the minimum scores required on each section an average language score and cumulative score are calculated for the purpose of admission into the department. The current minimum passing scores required for application to the Nursing Department are published in the Information Packet (SELECTION CRITERIA) which can be found on the college website at What is the retesting procedure?? A maximum of two attempts at testing per calendar year will be permitted for the HESI. A minimum of 30 DAYS must elapse between attempts. The highest score achieved in each section from eligible tests at the time of application to the Nursing Department will be considered for selection. A one hour time limit is observed for each scored section that is retested not to exceed total testing time of 3 hours. Maximum Testing Allowance: Students are allowed a maximum of two attempts for each version of the HESI resulting in an overall maximum number of four attempts. Any partial or whole exam testing is considered as an attempt. When the scores for a testing attempt become two years old that attempt will no longer count toward the maximum. a-2

15 GPA FACTORS/GRADE AMNESTY/GRADE FORGIVENESS Please reference sections in the LSCC Catalog and Student Handbook or consult student advising personnel for complete details on information presented in this section. GPA FACTORS- Students with transfer work should pay particular attention to the calculation of overall college gpa. Examples of factors that affect calculation are trimester curriculums, plus/minus letter grades, junior/senior level courses. GRADE AMNESTY- Students with coursework that is less than stellar and more than 10 years old are eligible to apply grade amnesty for the purpose of increasing the gpa calculation for application to limited access programs only. Official overall college gpa will remain unchanged. It is important to remember that prerequisite courses included in the amnesty calculation will be blinded and therefore not eligible for consideration toward program requirements. (Note single exception: If MTB1370 is completed or in progress at time of application return deadline and MAT0024 is included in amnesty courses, MAT0024 will be recognized as current.) Academic advisors will assist students on advisability of employing grade amnesty. GRADE FORGIVENESS- Students with coursework that is less than stellar and less than 10 years old may be eligible to retake courses for grade forgiveness. Students should monitor gpa calculation when grade forgiveness is a factor. Manual request for the calculation is sometimes necessitated. (Ex: Courses with associated labs may have been listed under separate course numbers on first attempt and listed as single C courses on second attempt. 16 MATH- ADMISSION REQUIREMENT There are two components to the math requirement- 1) demonstration of full knowledge base of MAT0024 2) successful completion of MTB1370. Successful completion of MTB1370 does not demonstrate full knowledge base of MAT0028 even though MAT0028 is a prerequisite course to MTB1370. MTB1370 expands on only a portion of MAT0028, not the whole course. Examples that satisfy the math requirement are: CPT score placing you out of MAT0028 that is less than 2 years old and successful completion of MTB1370. (NOTE: If CPT score is expired, but was current at the time you first registered into MTB1370 it is acceptable with supporting documentation from LSCC Admissions Department.) Successful completion of MAT0028 or higher level comprehensive math course and successful completion of MTB1370. Documented registration for a higher level comprehensive math course (based on CPT testing results current at time of course registration) and successful completion of MTB1370. Consult LSCC Admissions Department for guidance on CPT testing after score expiration dates. POST- ADMISSION BACKGROUND CHECK REQUIREMENT Program admittance letters will contain instructions for completing the background check requirement. The following criteria must be met to prevent seat forfeiture: FDLE/FBI BACKGROUND REPORT- meets one of the following 1) shows no history 2) shows misdemeanor(s) and/or felony(s) with results of conviction, guilty plea, no contest plea, or adjudication withheld no less than 2 years or 5 years old respectively; and does not contain offenses as listed in Section of the Florida Statutes as pertains to BON licensing guidelines. NATIONWIDE REGISTRIES (Sexual Offender & Predator; Healthcare Fraud and Abuse; U.S. Patriot Act)- show no violations. 17 Criminal History and Discipline Information (as copied , from ) Individuals who are considering entering the nursing profession and who have a criminal history often ask about potential barriers to licensure following successful completion of an approved nursing program. While it would be nice to know this prior to making a decision to enter the program, obtaining that information is not possible under current Florida law. The licensure application requires disclosure of any criminal history and the disposition of all cases prior to board review. Entry into the nursing education program is the prospective student s decision based upon the knowledge that he/she may, or may not, be granted a nursing license. All the factors listed below should be taken into consideration prior to making a decision about a nursing career. Each nursing program makes independent decisions regarding admissions and may require a criminal background screening as part of that process. Clinical facilities may limit or prohibit students with criminal histories from participating in clinical experiences. Other options may not exist for the student to complete required clinical hours in order to obtain a nursing degree; thus, such a student may not be eligible for licensure in Florida. The Board of Nursing makes decisions about licensure based upon a number of questions on the application and on an individual basis. The application (or the background screening) that indicates a criminal history is considered a non-routine application and must be reviewed by the board staff and possibly referred to the Board of Nursing for action. Each application is reviewed on its own merit. The Board of Nursing has created guidelines for specific offenses to be cleared in the board office; however, the staff cannot make determinations in advance as laws and rules change over time. Violent crimes and repeat offenders are required to be presented to the Board of Nursing for review. Evidence of rehabilitation is important to the Board Members when making licensure decisions. There are no permanent barriers to licensure; each circumstance is reviewed on a case by case basis. Section , FS, was enacted through the legislative process in The Board is required to deny any application that meets the criteria of this statute. You may review this statute at The Board may issue a license under conditions such as probation, supervision, or additional education, or the Board may deny your application. If drugs or alcohol are a concern, the board may require the applicant to undergo an evaluation and to sign a contract with the Intervention Project for Nurses ( ). 18 DRUG TESTING Currently drug testing is not a requirement for application to the nursing department. The department maintains a zero tolerance policy for substance abuse. All active students in the program are required to meet this standard. Upon enrollment into the program students will participate in an ongoing drug screening process including 1) annual clinical pre-attendance 2) semi-annual random 3) reasonable suspicion ASSESSMENT TESTING Currently the nursing department does not employ the use of national assessment testing tools for the purpose of creating exit points in the curriculum. These tools are included in the exam schedule for courses, at mid curriculum, and near program completion. Results of the exams are weighted in order to contribute a percentage to overall course grading. Course syllabi will indicate the specific percentages assigned to the exams. Percentages assigned to the exams are minimal. No student will exit the program based solely on national assessment testing performance. a-6

19 TRANSFER POLICY Students who have begun a nursing program at an institution other than LSCC may request a transfer into the nursing program at LSCC. It is important to remember that each institution develops and follows its own curriculum as well as admission procedures. Therefore, requests for transfer are reviewed on a case by case basis in order to determine what LSCC coursework is needed in order for the student to complete the program. Students who have been out of their prior program for over 1 year will not be considered for transfer into the LSCC program. Students who have made multiple attempts at nursing programs other than LSCC and subsequently have multiple departures will not be considered for transfer into the LSCC program. Students will not be transferred into the last semester of either track of the LSCC program. The initial requirements that must be satisfied before a student will be considered for transfer are: Completion of all prerequisite courses as outlined in the nursing information packet. ( An overall college GPA of 2.5 or higher substantiated by official transcripts from all institutions on record in the LSCC Admissions office. Current and passing HESI scores as outlined in the nursing information packet. A negative FDLE/FBI background check that is no more than six months old. Upon satisfaction of the initial requirements, the student will need to supply copies of course descriptions and syllabi for all nursing courses completed as well as a letter of recommendation from the Dean/Director of the last nursing program attended. After review of the latter, the program director will determine and then notify the student of the LSCC nursing coursework needed to complete the program. The student will be added to the transfer waiting list for the next term and year in which that coursework will be offered. Student will gain admission according to position on waiting list and seat availability. There is no guaranteed seating for transfer requests. Upon acceptance into the program the student will need to provide further medical and immunization documentation as required for all incoming nursing students. a-7

20 FINANCIAL ASSISTANCE The nursing department does not directly offer or manage financial assistance programs. It is the responsibility of each student to seek opportunities to fund his/her education. The internet is an excellent resource tool to research nursing grant money and scholarships. Some local places to begin are: LSCC Financial Aid Located in the Student Services Building. Can assist with financial needs and answer questions. Scholarships, grants, long or short term loans, and part time employment are provided according to availability, eligibility and need. LSCC Foundation Located in the M Building. Provides numerous scholarships each semester. Donations from community organizations for nursing students are generally awarded each November. LSCC New Directions Program Located in the Student Services Building Features various state/grant funded programs for men and women to assist with survival needs. Available programs vary based on yearly funding. Nurse Scholars Human Resources Departments of Local Hospitals Many hospitals fund education expenses directly related to nursing program courses. In general, students who accept this type of financial assistance commit with the facility to work for a prescribed time period after they graduate from the program. a-8

21 UCF ADN-BSN CONCURRENT TRACK SUMMARY INFORMATION This track, offered through collaboration with the University of Central Florida, provides LSCC Nursing Program admitted students the opportunity to begin coursework toward the Bachelor of Science in Nursing while enrolled in LSCC ADN courses. Additional requirements must be satisfied in order to participate in the concurrent track. The following requirements are necessary to begin the Concurrent BSN program with the UCF College of Nursing: Completion of UCF General Education Program requirements OR an earned AA degree from a Florida public institution. Completion of Foreign Language (This is typically met through two years in high school or two semesters in college of the same consecutive language) Chemistry(preferred) or other approved science Statistics Cumulative GPA of 3.0 or higher (per UCF registrar) TEAS score of 78% or higher; the LSCC testing center will offer the TEAS test on a limited basis. Students interested in applying to the concurrent track should complete and attach the UCF ADN- BSN CONCURRENT TRACK INTEREST PAGE, which is available within the LSCC Nursing Program Application.
