2 Part I: Graduate School Application The Graduate School recommends beginning the application process a year before the intended semester of enrollment. Please submit the following application materials by mid-november to early December in order to be processed in time for the Department deadline of February 1. Review the UWM Graduate School Application Guidelines at: prospective/apply/ Graduate School Application Checklist Complete the University of Wisconsin System Graduate Admission Application Online Form: Graduate School Admission Application Fee Official Transcripts (must be originals for the Graduate School materials) One copy of the Reasons for Graduate Study Statement For international applicants ONLY: Proof of English Language Proficiency. 7 Admission Application Requirements Only one Digital Portfolio is necessary if you are submitting multiple applications (i.e. Teaching Assistantship, Project Assistantship, and Layton Fellowship). Digital Portfolio: Twenty images (max.) of recent work, formatted and labeled as follows: Formatting for Digital Portfolio: The entire Digital Portfolio must be saved as a single PDF document. Begin the Digital Portfolio with a 250-word artist statement (12 pt. font). Please note: the Digital Portfolio should be no larger than 20 MB total, and should be created using high quality compressed JPEG images no larger than 1024 pixels in width. Please include an on-screen caption (10 pt. font) below each image giving the Title, Date, Size, and Materials (please note detail images). Formatting for Video: Video, Net.art works, and video documentation of installation or performance must be represented in stills in the Digital Portfolio described above. Each video should be posted on Vimeo. For Vimeo directions, go to: For Vimeo compression guidelines, go to: If you prefer to keep your Vimeo files private, please provide passwords for the files. Please only provide links to Vimeo files that are represented by stills in the Digital portfolio. Each still should have descriptive captions (10 pt. font) with links to the Vimeo files in the Digital portfolio onscreen captions and on the image list. 7

This includes a review of any pending charges or convictions. The Wisconsin Fair Employment Act prevents employers from disqualifying applicants based on criminal history unless the position includes job responsibilities that are substantially related to the criminal history.
