1 Integrated Delivery of Rehabilitation Services Family Feedback Survey The Government of Ontario has asked all agencies across the province that work with children and youth with special needs to develop a new approach to the delivery of rehabilitation services (Physiotherapy, Occupational therapy, Speech-Language Pathology). Within the Ottawa area, a large group of agencies has come together to look at how best to accomplish this. Our goal is to make a proposal to the government that will reorganize how services are delivered to make services Easier for children and families to find and use... Better in meeting the needs of children and families... More continuous as the child grows, moves into and eventually out of school While the government is not providing any new funding at this time, we believe there are ways we can reorganize things to make better use of our current resources. To achieve our goal, we strongly believe that we need to hear directly from families about their experience with our services and about the changes we should consider to make it better. By completing the survey below you will help us do just that. Thank you! The questions in this section refer to your experience with OCCUPATIONAL THERAPY (OT) services in the Ottawa area. Has your child or youth used Occupational Therapy (OT) services at any age from 0 to 21 years old? o Yes o No When was your child/youth referred to these services? o Birth to school entry o Kindergarten (JK and SK) o Grades 1-3 (primary) o Grades 4-6 o Grades 7-8 o Grades 9-12 ACCESS TO INFORMATION AND SERVICES The following questions refer to ACCESS TO INFORMATION (how easily a family can find information about Occupational Therapy (OT) services) and ACCESS TO SERVICES (the ease of completing intake processes and starting Occupational Therapy (OT) services).
