2 College Credit Plus (CCP) is Ohio s dual enrollment program, which provides an opportunity for students who are college-ready to earn transcripted college credit while in grades This guide provides information on Plain Local s CCP Program for and includes: College Credit Plus terminology Key information concerning CCP courses Information regarding transferability of college credits earned through CCP Information for CCP courses offered AT GlenOak High School Enrollment processes and forms Due dates All information included in this guide is also available on the GlenOak High School, Oakwood Middle School, and Plain Local homepages. 3 Key Information Regarding Plain Local s College Credit Plus (CCP) Courses Students may participate in CCP by enrolling in college courses that are taught at GlenOak High School or by enrolling in college courses directly through the university/college. Students wishing to take a CCP course either at GlenOak High School or directly from the college/university must turn in an Intent & Records Release Form by April 6th. Additionally, these students must complete any applicable college admissions applications, turn in all required CCP course enrollment forms and teacher recommendation forms, and take all applicable college admissions placement tests during Spring of the previous school year. The deadlines for the completion of all such applications, forms, and testing appear at the end of this document. Students must meet all of the college or university s normal admissions requirements that exist for traditional college students as well as any minimum placement tests scores that may exist for courses in which the student attempts to enroll. In most cases, this will involve COMPASS or ACT testing, however, specific test requirements for each course appear later in this document in the course descriptions section. The college/university makes the final determination as to whether or not a student is admitted to the college and then enrolled in the CCP course. Students are responsible for retuning textbooks to the instructor at the end of the course. Students failing to return a textbook will be fined the cost of the book and a financial hold will be placed on the transcript and diploma until the fine is paid in full. There presently are no fees for any CCP courses affiliated with public colleges or universities (private colleges or universities may charge a fee). However, students who earn a final course grade of F will be required to pay fees to Plain Local Schools to cover the tuition fees that Plain Local will be required to pay to the college/university for that particular course. Enrolling in a CCP course means that the student is committed to taking the course. Both parent and student should have a discussion regarding the seriousness of taking a college course and ramifications of the grade and credit earned by the student. Parents and students are encouraged to contact the student s high school counselor with any questions or concerns. Parents are required to attend the CCP Information meeting held at GlenOak High School, which will take place on February 26th at 6:00 PM in the GlenOak High School Theater or to acknowledge they have read and are aware of all information in this CCP guide. College Credit Plus students will: o be taught by GlenOak High School faculty who are adjunct college/university instructors or by instructors from the college/university in CCP courses that take place AT GlenOak High School o use textbook and course materials that are selected by the college/university o follow the college/university course objectives and learning outcomes o earn both high school credit and college credit o start (or continue if student previously took dual enrollment courses) their college transcript o earn a weighted grade equal to that of other advanced standing courses (AP Courses) in the same subject area that exist at GlenOak High School 3

4 Transferability of Credit Earned Through Plain Local s College Credit Plus Courses It is strongly recommended that students speak with admissions staff at colleges or universities they may be considering attending to check if the CCP courses they plan to take will fulfill a degree course or elective requirement. All credits earned from courses marked OTM or TAG will transfer to any Ohio Public College or University. o We recommend that students go to the college/university website and check the degree course requirements for the degree they are thinking of pursuing and also go to or These websites will provide information on what the CCP course will transfer to at the chosen university or college. For example, a student should learn if the English CCP course he/she takes at GlenOak High School through Stark State College will fulfill the English requirement at an Ohio public college or university such as Kent State or Ohio State. Later in this College Credit Plus Guide, there is a listing of all CCP courses offered AT GlenOak High School that includes the college course number, college course title, and potential college credit hours to be earned for all CCP courses offered here at GlenOak High School o Students will want to find out what courses they need to take at the college/university they plan to attend The college/university a student attends after high school graduation has the last and final word of what CCP courses are accepted for degree course requirements. It is the student s responsibility to contact the college admissions and verify. It is strongly recommended that students keep each course syllabus and obtain a college transcript at the end of each college academic term to verify that all the college credits completed at GlenOak High School appear on the college transcript. o Plain Local Schools does not assume any liability or responsibility for what appears on the student s college transcript. GlenOak High School counselors can assist in answering questions. 4

5 College Credit Plus TERMINOLOGY Dual Enrollment (DE) Courses that students can take that results in both college and high school credit being earned Institution of Higher Education (IHE)- Term referring to the college or university College Credit Plus- The new name the State of Ohio has given to any course or program where a student engages in dual enrollment coursework. It replaces old name of Post Secondary Education Option (PSEO) College Credit Plus (Dual Enrollment) AT The High School- Dual enrollment classes taken at the high school. College Credit Plus (Dual Enrollment) AT The College- Students apply to the college or university and if admitted, the student attends classes on the college/university campus and provides own transportation. In the case of a private college/university the student may be required to pay for tuition as determined by the IHE. There is a mandatory meeting held at GlenOak High School in January or February that students and parents must attend in order to be eligible for this option. Note: Post Secondary Enrollment Option (PSEO) is an old term still used by some colleges that also refers to dual enrollment courses taken at the college campus. Online Course- A CCP course that is taught and presented totally online. All lectures, homework assignments, tests and final exams will be taken online. There are no regular class meetings. Face To Face Course- The CCP course is taught by GlenOak High School faculty who are also adjunct professors and the class meets on a regular basis as part of the normal GlenOak High School class schedule. Test Scores- A copy of the student s ACT, SAT, or COMPASS scores is required when the student is enrolling for the first time in a dual enrollment course or taking a course with different test score requirements. ACT (American College Test)- A group of tests required or recommended by most colleges as part of the admission process. Students pay a fee to take the ACT, and it can be taken at GlenOak High School on various Saturdays throughout the year. SAT (Scholastic Aptitude Test)- A test of verbal, written, and mathematical abilities given by the College Entrance Examination Board through the year and required or recommended by some colleges as part of the admission process. SAT scores from on each section. Students pay a fee. The SAT is NOT offered at GlenOak High School. COMPASS- A comprehensive computer-adaptive testing system from ACT used to place students into appropriate college courses. It is free and can be taken at GlenOak High School or at Stark State College. High School Transcript- A student s official record of high school courses attempted, grades earned, and credit earned. It s generally required as part of the college application. The high school will provide a copy of the student s high school transcript to the college or university to comply with admissions requirements for dual enrollment courses. College Transcript A student s official record of college courses attempted and grades/credit hours earned. It s the student s responsibility to obtain it from the college, and it s recommended that a student does so at the end of each semester. Transcripted Credit College credit that is recognized on the student s college transcript upon successful completion of the college course. The high school student is enrolled at the college at the time the student is taking the college course. Intent & Records Release Form (IRR Form)- This form is required for enrollment dual enrollment classes. It tells GlenOak High School what dual enrollment courses a student is requesting to take and gives GlenOak and the college/university permission to share records such as student s high school transcript, test scores, student s college ID number, grades in college courses, and any other information that is necessary in order to enroll the student in to the college courses. Stark State Online Admissions Application An online application that must be completed by any student wishing to take a CCP course that s affiliated with Stark State College. This must be done prior to the student completing the Stark State Course Enrollment Form (see next definition). Stark State Class Registration Form- Stark State College requires students to submit a Fall and Spring class registration form. Students must complete the top portion including the Stark State ID number. Student and parent are required to sign the bottom of form. Middle section of the form (including the course information) will be completed by high school personnel. Mount Union CCP Application A paper application form that student and parent must complete if taking a dual enrollment course that s affiliated with the University of Mount Union Malone CCP Application A paper application form that student and parent must complete if taking a dual enrollment course that s affiliated with Malone University Teacher Recommendation Form A form that the University of Mount Union requires to be completed by the student s most recent math teacher for its Pre Calculus (Math 140) and its Introduction to the Engineering Profession (EGE 110) courses 5

6 INFORMATION ON CCP COURSES OFFERED AT GLENOAK HIGH SCHOOL GlenOak High School students have the opportunity to choose from 19 CCP courses that are taught AT GlenOak High School worth 70 college credit hours. Each course s delivery method is the traditional face-to-face model unless otherwise specified as an online course. The course descriptions that are provided come from the college or university that is affiliated with the course. The Ohio Transfer Module (OTM), which is a subset or a complete set of a public college's or university's general education requirement that represents a common body of knowledge and academic skills in English composition and oral communication, mathematics, arts and humanities, social and behavioral sciences, and natural sciences. Transfer Assurance Guides (TAG) courses include the Ohio Transfer Module courses and are pre-major/beginning major courses. All courses in TAGs are guaranteed to transfer and apply directly to the major requirements accordingly at any public college or university in Ohio. Courses that are marked as OTM or TAG courses are guaranteed to transfer to any public college or university in Ohio. Students looking to attend a nonpublic college or university in Ohio or any college outside of Ohio should contact the college of interest to determine credit transferability. NOTE: Due to the fact that a college or university could at any time change the requirements to be enrolled into a course or could make decisions that would affect the CCP courses that it offers at a high school, the following information pertaining to CCP courses that are taught AT GlenOak High School is subject to change at any time during the school year. ENGLISH LANGUAGE ARTS College Credit Plus Courses GLENOAK COURSE NAME: College Composition I CCP GlenOak Course Code: CCP001 High School Credits: 1 Course Length: Semester Recommended Grade Level: 11 th - 12 th Grade is weighted on HS transcript: Yes Stark State College Course Name: College Composition I Stark State College Course Code: ENG124 College Credit Hours: 3 Credit Transferability: OTM Prerequisites/Testing Requirements: Score at least 70 on COMPASS Writing or 18 on ACT English Course Description: ENG 124 emphasizes writing based on reading response with review of essay development, grammar, and punctuation. Emphasis is on the process of drafting, revising, and editing to achieve clarity. A research project requires APA or MLA documentation. GLENOAK COURSE NAME: Brit Literature: Med to 1785 CCP GlenOak Course Code: CCP002 High School Credits: 1 Course Length: Semester Recommended Grade Level: 11 th - 12 th Grade is weighted on HS transcript: Yes Stark State College Course Name: British Literature: Med To 1785 Stark State College Course Code: ENG233 College Credit Hours: 3 Credit Transferability: OTM & TAG Prerequisites/Testing Requirements: Successful completion of Stark State s ENG124 Course Description: ENG 233 covers British literature from the Middle Ages to the early modern period. Students will read, discuss, and write about works by British authors in their historical and cultural contexts. Principles of content selection, organization, audience analysis, and projection are studied. Union Course Code: Math 140 College Credit Hours: 4 Credit Transferability: Check with your college of interest Prerequisites/Testing Requirements: Successful completion of GlenOak s Algebra II or Honors Algebra II and score at least 51 on COMPASS Algebra Completed recommendation form from most recent math teacher Course Description: A study of topics needed to prepare a student for calculus including polynomial, exponential, and logarithmic functions, graphs, quadratic equations and inequalities, and selected topics from analytic geometry and trigonometry. 7

As provider of care the student studies communication principles, medical terminology, and math for medications. Under professional role behaviors, principles of critical thinking are utilized as a foundation for the nursing process. Involvement in professional nursing student organizations is required. In the coordinator of care behaviors, health care settings and the various roles of the nurse such as change agent, leader, and patient advocate are introduced. Mt. Union Course Name: Intro to the Engineering Profession Univ. Mt. Union Course Code: EGE 110 College Credit Hours: 2 Credit Transferability: Check with your college of interest Prerequisites/Testing Requirements: Best if enrolled in Algebra II or Honors Algebra II or higher level math class when enrolled in this course Completed recommendation form from most recent science teacher Course Description: A highly interactive seminar style course that explores all aspects of the engineering profession, including engineering disciplines, challenges, education, and employment; creativity and design; and the professional responsibilities of engineers. Student teams make presentations that discuss the relevance of course topics to current events. 10

11 EMERGENCY SERVICES College Credit Plus Courses GLENOAK COURSE NAME: EMERGENCY MEDICAL TECHNICIAN GlenOak Course Code: CCP017 High School Credits: 1 Course Length: All year Grade Level: 12 th Grade is weighted on HS transcript: No Stark State College Course Name: Emergency Medical Technician Stark State College Course Code: EMS121 College Credit Hours: 7 Credit Transferability: CTAG Prerequisites/Testing Requirements: Stark State requires acceptance into their Fire Science Program and must be 17 or in last year of high school Must complete both FBI and BCI background checks Course Description: Emergency Medical Technician (EMT) course provides theory and practical skills training for managing medical and traumatic situations at the EMT level and follows the objectives as listed in the Ohio Administrative Code. GLENOAK COURSE NAME: FIREFIGHTER 1 & 2 GlenOak Course Code: CCP018 High School Credits: 2 Course Length: All year Grade Level: 12 th Grade is weighted on HS transcript: No Stark State College Course Name: Firefighter 1 & 2 Stark State College Course Code: FST228 College Credit Hours: 11 Credit Transferability: CTAG Prerequisite/sTesting Requirements: Stark State requires acceptance into their Fire Science Program and must be 17 or in last year of high school Must complete both FBI and BCI background checks Course Description: This course meets the requirement in the Ohio Revised Code regarding the certification of a full-time paid Fire Fighter. Instruction includes topic areas taken from NFPA 1001 Fire Fighter 1 and 2. This is the highest level of Fire Fighter certification offered in the state of Ohio. Successful completion of this course is required to take the state certification test at Fire Fighter level 2. INFORMATION TECHNOLOGY College Credit Plus Courses GLENOAK COURSE NAME: COMPUTER APPLICATIONS FOR PROFESSIONALS GlenOak Course Code: CCP013 High School Credits: 1 Course Length: Semester Recommended Grade Level: 9 th 12 th Grade is weighted on HS transcript: No Stark State College Course Name: Computer Applications for Professionals Stark State College Course Code: ITD122 College Credit Hours: 3 Credit Transferability: TAG Prerequisites/Testing Requirements: Must attain minimum score on Stark State ITD122 Placement Test (paper and pencil test taken at GlenOak High School) and provide COMPASS Reading, COMPASS Writing, or ACT scores (no minimum score required) Course Description: Describes the components and peripherals of a computer/pc and how they function and communicate as a system. Principle topics covered are the Windows operating system, Internet applications, the Windows networking environment and a variety of software application packages used to solve scientific, business, and engineering technology problems. 11

12 BUSINESS & MARKETING College Credit Plus Courses GLENOAK COURSE NAME: MICROECONOMICS GlenOak Course Code: CCP014 High School Credits: 1 Course Length: Semester (offered 1 st semester only) Recommended Grade Level: 11 th 12 th Grade is weighted on HS transcript: No Stark State College Course Name: Microeconomics Stark State College Course Code: BUS221 College Credit Hours: 3 Credit Transferability: OTM & TAG Prerequisites/Testing Requirements: Preference given to: Score at least 18 on ACT English or 80 on COMPASS Reading Students who completed GlenOak s High School of Business Program and then to students who have completed High School of Business Economics course Course Description: An in-depth study of microeconomic concepts and principles such as supply and demand, price elasticity, production costs, different market structures, income distribution, marginal analysis, and other issues relating to global economics. Upon completion of this course, students should be able to demonstrate an understanding of these topics and be able to apply them to business. 14 Pathways To Earn 15 or 30 College Credit Hours The state of Ohio requires each school district to create a pathway for students to earn 15 college credit hours and a pathway for students to earn 30 college credit hours. 16 Application / Enrollment Procedures Overview For STARK STATE COLLEGE courses taught at GlenOak High School 1. Student and parent must complete and sign a GlenOak CCP Intent & Records Release Form (pages of this CCP Guide). This tells GlenOak which dual enrollment course(s) the student wishes to take and allows GlenOak to receive the grade the student earned from Stark State and allows GlenOak to share test score data and the student s high school transcript with Stark State College. 2. Students must access and complete the online Stark State Admissions Application by going to and then clicking on the link for College Credit Plus Admissions Application. Only first time Stark State students need to complete this online application. Returning Stark State Dual Enrollment students do not need to complete this online application because they have previously completed it. Once the online admissions application is completed, Stark State College will assign the student a Stark State Student Identification Number. Students must keep this number because it will be used to complete the Course Enrollment Form for all Stark State courses. 3. Student must complete one Stark State CCP Class Registration Form for each of the CCP courses in which they wish to enroll (pages of this CCP Guide has one blank class registration form for each Stark State course that is taught at GlenOak High School). You will need to complete Stark State s online application in order to get a Stark State ID number for top of form If neither Fall 2015 or Spring 2016 is already marked on your form, leave it blank. It will be marked after your student schedule is determined for the school year. Some of the middle section of the class registration form for each course has already been completed. Leave the rest of the middle section blank. Leave GPA blank. For Graduation Date, write May followed by your expected high school graduation year (ie, May 2017 ) Both student and parent must sign the bottom of the form 4. Student must earn the required ACT or COMPASS scores for the course(s) in which he/she wishes to enroll. If GlenOak does not already possess copies of ACT or COMPASS scores, then the student must include copies of these scores with the Intent & Records Release Form and Class Registration Forms which get turned in to the high school counselor s office. A returning Stark State dual enrollment student does not need to complete any additional testing unless the new course he/she is requesting has additional testing requirements that have not yet been satisfied. 16

17 For MALONE UNIVERSITY courses taught at GlenOak High School 1. Student and parent must complete and sign the GlenOak CCP Intent & Records Release Form (pages of this CCP Guide). This tells GlenOak which dual enrollment course(s) the student wishes to take and allows GlenOak to receive the grade the student earned from Malone University and allows GlenOak to share test score data and the student s high school transcript with Malone University. 2. Student must go online to and access and complete the Malone University CCP forms and documents which include: a. Application form i. includes a 250 word essay b. one teacher reference form c. one counselor reference form d. Malone Community Agreement form e. High school transcript i. GlenOak High School will submit this on your behalf if you are taking one of the CCP courses that meets at GlenOak High School otherwise, the student is responsible for obtaining and submitting his/her transcript 3. Student must earn the required ACT or SAT scores for the course(s) in which he/she wishes to enroll. If GlenOak does not already possess copies of ACT or SAT scores, then the student must include copies of these scores with the CCP Intent & Records Release Form and CCP Application Form which get turned in to the high school counselor. a. Students who need to take the ACT prior to the next national testing date may contact Malone University by calling to schedule an appointment to take the Residual ACT on campus at Malone University. The scores for this ACT session will only be reported to Malone University and cannot be used with any other institutions. 17

18 Application / Enrollment Procedures Overview (continued) For UNIVERSITY OF MOUNT UNION courses taught at GlenOak High School 1. Student and parent must complete and sign the GlenOak CCP Intent & Records Release Form (pages of this CCP Guide). This tells GlenOak which dual enrollment course(s) the student wishes to take and allows GlenOak to receive the grade the student earned from Mount Union and allows GlenOak to share test score data and the student s high school transcript with Mount Union. 2. Student must complete the University of Mount Union CCP Application and indicate the course(s) that the student is requesting to take (pages of this CCP Guide). 3. Students must have their current math or science teacher complete and submit to the student s counselor the University of Mount Union Teacher Recommendation Form (page 39 of this CCP Guide). Note that this form is specific to the course that the student is requesting to take (math teacher for Prealculus or science teacher for Introduction to the Engineering Profession). 4. Student must earn the required COMPASS scores for the course(s) in which he/she wishes to enroll. If GlenOak High School does not already possess copies of the COMPASS test scores, then the student must include copies of these scores with the CCP Intent & Records Release Form and CCP Application Form, which get turned in to the high school counselor. For KENT STATE UNIVERSITY courses taught at GlenOak High School 1. Student and parent must complete and sign the GlenOak CCP Intent & Records Release Form (pages of this CCP Guide). This tells GlenOak which dual enrollment course(s) the student wishes to take and allows GlenOak to receive the grade the student earned from Kent State Stark and allows GlenOak to share test score data and the student s high school transcript with Kent State Stark. 2. Students must go online to to access the following forms which all must be completed and submitted to your GlenOak High School counselor by April 22 nd unless otherwise noted: a. Online application for admission must be completed by May 1st b. Kent State CCP Permission/Authorization form paper form c. Counselor Recommendation form paper form d. Two Teacher Recommendation forms paper forms 3. Students must submit ACT or SAT scores to their GlenOak school counselor by the following dates so that GlenOak can provide the scores to Kent State Stark: a. April 22, 2015 if taking a Fall Semester course b. August 14, 2015 if taking a Spring Semester course 4. GlenOak High School will provide an official copy of the student s high school transcript to Kent State Stark. a. Note: If student is an out of district student whose home school is not GlenOak High School, the student will be responsible for providing an official copy of his/her high school transcript to the counselor at GlenOak by April 22 nd. 19 For CCP courses taught on the college campus / directly through the college 1. Student and parent must complete and sign the GlenOak CCP Intent & Records Release Form (pages of this CCP Guide). This tells GlenOak that the student intends to participate in CCP by enrolling in courses directly through a college/university and which dual enrollment course(s) you wish to take. It also allows GlenOak to receive the grades the student earns from the college / university. 2. Students are responsible for working directly with the college/university to complete all college admissions applications, class registration forms, other forms, and necessary testing. 3. Student must provide a copy of his/her college course schedule with course names, college credit hours, and course days/times to his/her school counselor by August 7, 2015 so that courses at the student s home school can be appropriately scheduled around the college courses. 19

20 COMPASS & ACT TESTING PROCEDURES What the COMPASS Test Is COMPASS is a free online test that many colleges use to determine college readiness in specific subjects, and each section (Reading, Writing, or Algebra) will take approximately 45 to 75 minutes, however, the COMPASS is not a timed test. The course descriptions section that begins on page 6 in this CCP Guide provide detailed information regarding what COMPASS tests are required for specific CCP courses, including minimum scores when applicable. Students must bring their Social Security number, which is required in order to log in to the test. It is also recommended that students bring a calculator if taking the COMPASS math test. COMPASS Testing for students who will take CCP courses at GlenOak High School Students presently in grades 9 through 11 who need to take the COMPASS test may do so at GlenOak High School during the school day on April 9 th or 10 th or after school at 2:30 on April 13 th, 14 th, 15 th, or 16 th. Students may sign up for time slots during any of these dates by going online at Students presently in 8 th grade at Oakwood who need to take the COMPASS test will be able to do so at Oakwood Middle School on during the school day during the week of March 23 rd Oakwood counselors will communicate to students a process for this. Students who are planning to take CCP courses at GlenOak High School but are not able to take the COMPASS test at their home school as referenced above may contact Stark State College at to schedule a time to go there and take the necessary COMPASS test This must occur prior to the April 22 nd deadline that is specified on the final page of this document. COMPASS Testing for students who will take CCP courses at the College Campus If a student is seeking to take CCP courses at a college/university and COMPASS is required, the student should contact that college/university and make the necessary testing arrangements through the college/university. Purpose of the ACT The ACT is a national test that many colleges/universities require students to take in order to be admitted and to determine if a student is college-ready in different subject areas. Oftentimes, there is a minimum overall score that is required in order to gain admittance into a college/university as well as minimum scores in each of the component parts, which are Mathematics, Reading, English, Science and Writing. Taking the ACT The ACT can be taken at GlenOak High School on various Saturday mornings throughout the year. Students can go online to to obtain the exact schedule for when ACT testing occurs and to register. It can also be taken at any time at Malone University or at Kent State University Stark if a student is seeking to take Malone University CCP courses (contact Malone admissions to set an appointment) or Kent State University Stark CCP courses (contact Kent State University Stark). 20
