3 EMPLOYEE BENEFITS FROM CHUBB ACCIDENT & HEALTH Chubb BTA 360 SM (Business Travel Accident), Basic and Voluntary Accidental Death & Dismemberment (AD&D) are important components of any employee benefits program. PRODUCTS Chubb BTA 360 SM and 24-hour Basic and Voluntary Accidental Death & Dismemberment (AD&D) policies cover employees for accidental death or injury. BTA 360 SM can help employers fill coverage gaps, at home and abroad, by combining three components into one cost-efficient policy to meet the needs of their employees. Components include: Travel assistance coverages and services, which range from lost luggage and identity theft to medical and legal referrals, emergency transportation, and even dispatching medical specialists. Medical evacuation, repatriation and out-of-country medical coverages, which help to pay to secure competent emergency treatment for injured or sick employees, then bring them home. Traditional Accidental Death & Dismemberment coverage, which pays a lump sum for accidental loss of life, limb, sight, speech or hearing. AD&D policies are available on both a basic/employer-paid and voluntary basis. Basic insurance programs provide coverage 24 hours a day, 365 days a year, anywhere in the world. Chubb's voluntary program provides around-the-clock coverage for employees only or employees and their families, all through the convenience of payroll deduction. FLEXIBILITY Policies can be customized to meet the specific needs of your business and employees. You decide: Who is covered different classes of insured persons which may include non-employee directors, and employees spouses/domestic partners and dependent children. How much and what level of coverage each class receives. When each class is covered. Which benefits to choose over 30 benefits available. 4 ACCIDENT POLICY BENEFITS AVAILABLE Accidental Death & Dismemberment Accidental Medical Expense (BTA ONLY) Baggage Delay Brain Damage Burn Checked and Carry-On Baggage Checked Baggage Child Care Expense COBRA Premium Expense Coma Education Expense Enhanced Benefit for Dependent Child Enhanced In-Hospital Benefit Home Alteration and Vehicle Modification Pays a benefit if an accident results in loss of life, limb, sight, speech or hearing. Reimburses accident medical expenses if an accidental bodily injury causes an insured person to incur medical expenses. This benefit also reimburses medical expenses if an insured person incurs such medical expenses for care and treatment of a disease or illness for which symptoms first appear during a defined covered activity. Reimburses an insured person for the purchase of essential items needed, if such insured person s bags are delayed or misdirected by a common carrier. Pays a benefit if an accidental bodily injury results in brain damage to an insured person. Pays a benefit if an accidental bodily injury causes an insured person to suffer third degree burns. Reimburses an insured person for amounts actually paid, for direct physical loss or damage to suitcases and the personal property contained therein, if the loss or damage occurs when checked in or carried on to a common carrier. Reimburses an insured person for amounts actually paid, for direct physical loss or damage to suitcases and the personal property contained therein, if the loss or damage occurs while in the possession, care and control of the common carrier. If an insured person suffers accidental loss of life, this benefit will pay for child care costs for all children (up to the age of 13). If an insured person suffers an accidental loss of life, this benefit will pay the cost of the premium for the insured s dependent s election to continue group medical or dental insurance under a group plan provided through the policyholder. Pays a benefit if an accidental bodily injury causes an insured person to be in a coma. If an insured person suffers accidental loss of life, this benefit will reimburse actual incurred costs for his/her dependent children s tuition, fees, room and board, required books and course supplies billed by an institution of higher learning. Doubles the benefit amount for an eligible dependent child s accidental dismemberment. Pays a daily benefit for each day an insured person is in the hospital as the result of an accident. Reimburses the cost for home alteration or vehicle modification if an insured person suffers a covered loss due to an accidental bodily injury which results in a physician determining that a home alteration or vehicle modification is needed to accommodate a physical disability.

5 Home Health Care Kidnap and Ransom (BTA ONLY) Medical Evacuation and Repatriation Occupational Hepatitis Occupational HIV Paralysis Parent Care Permanent Total Disability Psychological Therapy If a covered loss due to an accidental bodily injury causes an insured person s confinement at home, after a hospital stay, then this benefit will reimburse the costs for medically necessary home health care. If an insured person resides in the United States and is kidnapped during business travel outside of the United States, this benefit will reimburse expenses for: a) payments made to a person or group for the insured person s release; b) any reward paid to an informant for information not otherwise available that leads to the arrest and conviction of person(s) responsible for such demand; c) reasonable fees and expenses for an attorney, a professional public relations consultant, a professional forensic analyst, a professional security consultant or for publicity expenses incurred as a result of the insured person s kidnapping; d) reasonable costs incurred by the insured person s immediate family member for temporary lodging, transportation and meals arising directly as a result of the kidnapping; and e) regular wages that are not paid due to the insured person s absence from work during the kidnapping. If an accidental bodily injury, disease or illness causes an insured person to require medical evacuation and/or repatriation, this coverage will pay the costs for such evacuation/repatriation. This benefit will also: a) guarantee payment of the charge made by a hospital prior to and as a condition of an insured person s admission for emergency medical treatment (Hospital Admission Guaranty); b) pay the costs for temporary lodging and meals incurred by an immediate family member who travels to be with a hospitalized insured person (Family Travel Expense); c) pay for an accompanying dependent child to return to his or her residence if the insured person is hospitalized while on a trip (Return of Dependent Child); d) reimburse medical expenses if an insured person who resides in the United States suffers an accidental bodily injury, disease or illness that results in medical expenses being paid while on business travel outside of the United States (Out of Country Medical Expense); and e) reimburse evacuation expenses if an insured person who resides in the United States is on a covered trip and the United States Department of State issues a travel warning advising U.S. citizens to depart a country while such insured person is traveling in that country (Political Evacuation). Pays a benefit if an occupational injury causes an insured person to become infected with hepatitis. Pays a benefit if an occupational injury causes an insured person to become infected with HIV. Pays a varying percentage of the principal sum if a covered accident causes the insured person to suffer quadriplegia, paraplegia, hemiplegia or uniplegia. Pays an additional benefit to an insured person s dependent parents or grandparents if an accidental bodily injury causes the death of the insured person. Pays a benefit amount if an accidental bodily injury causes an insured person to suffer a disability that continues for 365 days and is expected to continue without interruption and without expectation of recovery for the rest of the insured s life as determined by a physician. Reimburses expenses incurred for psychological therapy if an insured person suffers a covered loss resulting in a physician determining that psychological therapy is required.

Reimburses the cost for rehabilitation required as the result of a loss caused by an insured person s accidental bodily injury. Pays a benefit if an insured person suffers an accidental bodily injury resulting in a covered loss while operating or riding in a private passenger automobile and using a seat belt. Also pays an additional benefit if the insured suffers an accidental bodily injury as set forth above and such insured is positioned in a seat protected by a properly deployed occupant protection device. If an accidental bodily injury causes an insured to suffer a covered loss of life, this benefit will reimburse actual incurred costs for the spouse/domestic partner s tuition, fees, room and board, required books and course supplies at an institution of higher learning. Pays a benefit to an insured person s surviving spouse/domestic partner or dependent child if an accidental bodily injury causes the insured s covered loss of life. Pays a benefit if an accidental bodily injury causes an insured person to suffer a temporary total disability. Pays a varying percentage of the principal sum if an accidental bodily injury causes an insured person to suffer loss of use of: a) one hand or one foot, b) both hands or both feet or a combination, c) one arm or one leg, d) both arms or both legs or combination, or e) both arms and both legs. Reimburses the costs incurred for tuition, fees, room and board, books and course supplies billed by an institution of higher learning if an insured person suffers a covered loss due to an accidental bodily injury. ACCIDENT POLICY HAZARDS AVAILABLE 24-Hour Business & Pleasure 24-Hour Business Travel Extraordinary Commutation (BTA Only) Felonious Assault (BTA Only) Specified Trip Provides coverage 24 hours a day, 365 days a year, anywhere in the world. Provides 24-hour coverage while an insured person is traveling away from his or her regular place of employment on the business of the policyholder. Provides coverage while an insured person is commuting to or from work when a strike, major breakdown or catastrophe causes the discontinuance of service of one or more public transportation systems regularly used by such insured person. Provides coverage for exposure arising from and occurring due to a felonious assault. Options for coverage are a) only for assaults that occur on the premises of the policyholder, b) only for assaults that occur off the premises of the policyholder, or c) for assaults that occur on or off the premises of the policyholder. Provides 24-hour coverage while on a specific trip identified in the policy.

7 EXCLUSION BUY-BACKS AVAILABLE (For Basic AD&D, BTA and Voluntary AD&D) Employee Air Crews Travel in Employer-Owned/Leased/ Operated Aircraft War-Related Accidents For an additional premium, a policyholder may choose to remove the exclusion for accidents or losses that occur while an insured person is acting or training as a pilot or crew member. For an additional premium, a policyholder may choose to remove the exclusion for accidents or losses that occur while an insured person is riding in a policyholder owned, leased or operated aircraft. For an additional premium, a policyholder may choose to remove the exclusion for accidents or losses that occur as the result of a war. ADDITIONAL SERVICES AVAILABLE Identity Theft Services* Travel & Medical Assistance Services* Services assist an insured person with the cancellation and replacement of lost or stolen U.S. issued credit cards, help an insured establish a police report, and place fraud alerts with the three credit bureaus. Services include pre-travel country information; worldwide medical, medical search and legal referrals; medical monitoring and emergency travel arrangements. *These services are provided by independent service providers.
