2 Overview of REACH The Racial and Ethnic Approaches to Community Health (REACH) began in REACH is an important component of CDC's efforts to eliminate racial and ethnic health disparities in the United States. REACH partners use community-based, participatory approaches to identify, develop, and disseminate effective strategies for addressing health disparities across a wide range of health priority areas. Because the causes of racial and ethnic health disparities are complex and include individual, community, societal, cultural, and environmental factors, REACH communities and partners engage a variety of strategies in their work, from counseling and education to systems and environmental changes.

3 REACH Award History Program Funding Years Purpose # of Awards REACH REACH US Supported projects focused on a coalition-based approach targeting racial/ethnic minorities within six health priority areas: cardiovascular disease (CVD), immunizations, breast and cervical cancer screening and management, diabetes, HIV/AIDS, and infant mortality. Funded 18 Centers of Excellence in the Elimination of Disparities and 22 Action Communities. Used community-oriented participatory approaches to address racial and ethnic health disparities: CVD, diabetes mellitus, infant mortality, asthma, Hepatitis B, HIV/AIDS, adult immunization, and tuberculosis REACH National Organizations REACH CORE REACH * Enables national organizations to share evidence- and practice-based programs related to specific health disparity areas with their local affiliates and chapters. Funds communities to organize, implement, and evaluate evidence-based interventions that eliminate racial and ethnic health disparities in chronic diseases. This program supports the transition of communities from the analysis of intervention results to the use of these results in eliminating health disparities. Funds organizations to implement sustainable evidence- and practicebased strategies impacting health disparities REACH Demonstration Project Funds awardees to develop and implement strategies that reduce obesity and hypertension in populations experiencing health disparities. 2

4 REACH 2012 The REACH 2012 program focuses on changes in: weight, proper nutrition, physical activity, tobacco use, heart disease, diabetes, infant mortality, and asthma. New awardees will distribute at least 75% of funds to support community-based organizations across the country to apply evidence- and practice-based PSE-focused initiatives that reduce health disparities for those experiencing high burden of chronic disease and its risk factors. Approximately $21.9 million will fund the REACH program. Funding is from the Prevention and Public Health Fund (PPHF) established by the Affordable Care Act.

5 Examples of Awardee Activities Funding up to 15 local partners or communitybased organizations to reduce health disparities in racial and ethnic communities (90 local partners total) Documenting community changes and evaluating the impact of PSE changes Widely disseminating proven and innovative strategies and successes to expand the evidence base.
