1 Planning and Community Development Department 214 North Center Street Goldsboro, NC (919) October 1, 2015 Dear Prospective Bidder: The City of Goldsboro s Community Development Department, on behalf of Wayne County Public Schools through its Public Facilities & Improvement Program, will be accepting informal bids for improvements to the running track located at the Pat Best Athletic Field next to Dillard Middle School, 1101 South Devereaux Street, Goldsboro, NC Bids must be sealed and clearly marked IFB (Pat Best Athletic Field Improvements). Bids must be submitted to the Finance Department, City Hall Annex, P.O. Drawer A, City of Goldsboro, North Carolina on or before Friday, October 16, 2015 at 10:00 a.m. at which time, the bids will be publicly opened. ALL WORK INDICATED on any documents attached to the BID FORM is included in the scope of work and shall be assumed to be included in the contractor s bid price. The attached BID FORM must be used to submit a bid on this project. Any changes to these forms, missing information, or any other irregularities in the bid package, may be cause for rejection of the bid and disqualification of the bidding contractor. Please advise that you must return the entire bid package complete to be qualified in this bid process. Prior to submission of bid packages, all bidders are solely responsible for visiting and conducting a site visit/inspection of the running track located at the Pat Best Athletic Field next to Dillard Middle School to ensure a more qualified bid. The Contractor shall indemnify and save harmless the City of Goldsboro and its officers, agents, and employees from all suits, actions or claims of any character brought for any injury or damages received or sustained by any person, persons, or property by reason any act of the Contractor, its agents or employees, in the performance of the contract. The Contractor shall furnish a Certificate of Liability Insurance from an insurance company, licensed to do business in the State of North Carolina within ten (10) days after award of the contract.

6 TECHNICAL SPECIFICATIONS FOR IMPROVEMENTS TO PAT BEST ATHLETIC FIELD Dillard Middle School 1101 South Devereaux Street Goldsboro, NC INSURANCE AND WARRANTY FUND REQUIREMENTS: Contractor agrees to purchase at he or she own expense insurance coverage to satisfy the following minimum requirements. A certificate reflecting the following minimum coverage shall accompany this Contract: Workers Compensation Insurance -Limits of no less than $1,000,000 each accident, each employee and policy limit. Waivers of Indemnity are not recognized by the North Carolina Department of Insurance and will not be accepted by the City of Goldsboro. Commercial General Liability -Combined single limits of no less than $1,000,000 each occurrence and $2,000,000 aggregate. This insurance shall include Comprehensive Broad Form Coverage including contractual liability. City of Goldsboro is named additional insured as their interests may appear must be endorsed onto policy and listed on Certificate of Insurance. Commercial Automobile Liability -Limits of no less than $1,000,000 Combined Single Limit. Evidence of commercial automobile coverage is only necessary if vehicles are used in the provision of services under this Agreement and/or are brought on a City of Goldsboro site. City of Goldsboro is named additional insured as their interests may appear must be endorsed onto policy and listed on Certificate of Insurance. All insurance companies must be licensed in North Carolina and be acceptable to the City of Goldsboro s Risk Manager. Contractor shall be required to provide the City no less than thirty (30) day notice of cancellation, or any material change, to any insurance coverage required by this Contract. Technical Specification to include: $ Peel grass and soil from edges of pavement, remove grass from existing cracks, clean cracks out, spray poison in cracks crack seal with hot pour sealant and larger cracks fill in with asphalt, seal coat with 2 coats of asphalt sealer and repaint stripe lines. $ Small Infield asphalt pad: Peel grass and soil from edges of pavement and seal coat with 2 coats of asphalt sealer. $ Asphalt Binder Price per ton (As of October 1, 2015 per Construction-Prices.aspx If cement price fluctuates from this stated price at any time during the performance of the asphalt work, the contractor reserves the right to adjust the prices in Accordance with the NCDOT Monthly Terminal F.O.B. Asphalt Binder Prices through the process and approval of a change order.

8 MINORITY BUSINESS PARTICIPATION REQUIREMENTS Provide with the bid - Under GS (c) the undersigned bidder shall identity on its bid (Identification of Minority Business Participation Form) the minority businesses that it will use on the project with the total dollar value of the bids that will be performed by the minority businesses.
