We provide services in more than 50 locations, ranging from the inner-city and suburbs to regional and remote parts of Australia. Our vision is for full citizenship for all people living with a mental illness in Australian society. Our mission is to improve mental health and wellbeing in local communities across the country. We believe that recovery is an individual process and that with the right kind of support everyone can live a life based on their own strengths, values and goals for the future. We use an approach called the Collaborative Recovery Model (CRM) to support people through our services. The CRM assists individuals to identify their personal strengths and values, to set goals and then helps them make progress towards achieving them. 2 PIR Consortium and position overview Central Adelaide and Adelaide Hills Partners in Recovery was established by a consortium. Under a new structure Primary Health Networks replaced the Medicare Locals on 30 June Neami National was nominated as the lead agency. The ultimate objective of PIR is to improve the system response and outcomes for people with severe and persistent mental illness by: Facilitating better coordination of services; Strengthening partnerships with clinical and community support organisations; Improving referral pathways and advocating for system change to meet individual client needs; and Promoting a community based recovery model to underpin all clinical and community support services. The Support Facilitator is responsible for facilitating the engagement of services and support for individuals who are eligible for PIR. The primary role of the position is to support clients to develop a PIR support plan based on each client's individual needs using a recovery oriented approach. The Support Facilitators actively engage with clients to coordinate existing services and supports to address the needs of people who meet the eligibility of the PIR program. This is primarily a facilitation and coordination role to seek solutions to complex systems in a resourceful manner. Conditions and period of employment The Partners in Recovery program will run until the 30 June Successful applicants will be employed directly by the agencies in the PIR consortium, not Neami. The agencies are: Life Without Barriers Mental Illness Fellowship South Australia Mind Australia Mission Australia Uniting Care Wesley Bowden Uniting Care Wesley Port Adelaide The base salary will be in the range $62,244 - $65,010 pa (pro rata), to be negotiated with the employing agency. Criminal record checks are mandatory for all new appointments. Working with Children check required before commencement of work (employee responsibility). Accountability The Support Facilitator (PIR) is accountable to a line manager within the host agency and also has a reporting relationship to the Partners in Recovery (PIR) Coordinator(s) employed by Neami as the lead agency. 2 of 8

4 Section C: key responsibilities Provide a coordinated service to consumers Operating in a way that is consistent with the principles of recovery Developing productive and supportive relationships with clients and their existing support network Undertaking ongoing review of the needs of the PIR clients Developing an Action Plan with clients of PIR and conducting regular reviews of the Action Plan in consultation and collaboration with clients, service providers, carers and families, as appropriate, The PIR Action Plan will guide the necessary engagement and integration of required services Developing and maintaining local partnerships and relationships Identifying and connecting a PIR client to ongoing case-management early in the implementation of the PIR Action Plan Being a point of contact for PIR clients, their families and carers when service arrangements are not working or the client becomes disconnected from required support Engage with potential PIR consumers and their support networks Utilise different strategies to support engagement with consumers disconnected from the service system Assess individual consumer needs and collaboratively develop a PIR Action Plan that is implemented, monitored and reviewed regularly Engage with and work collaboratively with the existing care coordination activities that may already be in place Involve carers, family and friends as identified by the consumer in the development and implementation of the Action Plan Facilitate the engagement of services for PIR consumers, including primary health care services. Utilise conflict resolutions skills and participate in cross program/service coordination to resolve systemic issues Advocate for the consumer as needed or requested Contribute to service mapping and gap analysis in your area Participate fully as a team member Using the team approach to support work, co-operate closely with other team members and service providers to ensure continuity of care and provision of a comprehensive service Actively participate in reflective practice through team meetings, decision-making processes, service planning sessions, supervision and staff development activities Assist the team and broader PIR network, to further develop best practice, review and develop policies, and project submissions Assist the team to regularly evaluate the effectiveness of the service in consultation with consumers 4 of 8

5 Develop and foster partnerships Providing support for local sector development activities, including promotional, communication and networking activities Contributing to ongoing service mapping, gap analysis and systems enablers and barriers Developing and managing effective and strategic partnerships with identified stakeholders to ensure comprehensive, cohesive and seamless integration of complete wrap around services are provided to PIR clients Engaging with stakeholders and partner agencies to promote the service, increase awareness about mental health issues, and identify potential gaps in services to facilitate sector development and training opportunities Facilitating better coordination of service systems to meet the needs of the PIR client Building service pathways and networks of services and supports as needed; wherever possible, securing access to existing services and supports Represent Neami and the PIR program on relevant committees, advisory groups and other events as required Build awareness of the PIR program s benefits with key stakeholders Develop collaborative working relationships with other Neami staff, PIR staff and external stakeholders Establish and maintain sustainable service pathways, partnerships and support networks to meet consumer needs Involve carers, family and friends in the development of Action Plans where possible Promote higher level systemic change by building and strengthening community partnerships to encourage innovative and collective solutions Maintain records and resources Maintaining the necessary reporting and information provision to PIR organisation management to ensure effective administration, evaluation and governance Collect, collate and maintain data on all consumer interactions, ensuring that the data is accurate, up to date and complies with reporting guidelines Collect information on local community services and supports Share this information with other PIR Support Facilitators and Neami National staff Support Neami efforts in reducing our impact on the environment and work towards a sustainable future 5 of 8

About this role: The successful applicant for the Support Facilitator position will bring solid experience in the human services environment. In particular, they will demonstrate an understanding of how best to access, advocate for, and maintain services for consumers living with a mental illness who have complex needs. This experience will be complimented by a keen and dedicated commitment to supporting recovery and improving outcomes for people living with mental illness in our community. The following competency criteria will inform our selection decision: Highly developed interpersonal skills Excellent verbal and written communication skills Proven ability to analyse problems and formulate solutions in a clear, innovative and resourceful manner and evidence of a holistic approach Ability to think systemically and act strategically Demonstrated ability to work effectively with a range of different service providers (including government and non-government) to access services and/or coordinate service delivery Ability to work independently when required, taking appropriate initiative but also participate as an integral member of a team. Demonstrated professional and personal commitment to upholding and facilitating the rights of people living with complex mental health and other needs Understanding and commitment to recovery-oriented principles in mental healthcare Demonstrated experience in working collaboratively with multiple stakeholders to develop appropriate Action Plans Facilitating collaborative care planning Experience in advocating for services for and/or working with people with complex needs Experience in care coordination for people living with complex needs 6 of 8

7 Experience engaging with clients to offer and seek solutions to complex problems in a resourceful manner using the additional services and support available to enhance recovery outcomes Familiarity with and experience in liaising with a range of primary health services Knowledge and/or experience of the SA mental health system Degree level qualifications Experience working with Aboriginal and Torres Strait Islander people Knowledge of relevant State and Commonwealth Acts and Legislation relevant to people living with mental illness and/or disability Knowledge of supported decision making for people who may live with a level of mental incapacity as defined by the Guardianship and Administration Act 1993 Knowledge of National Mental Health Standards In addition you will need: Computer literacy Current Australian driver s license Tertiary qualifications in Mental Health, Disability or Community Services desirable Demonstrated skills in working with clients with complex needs, and capacity to see consumers at the core of their focus. 7 of 8

We were founded in 1986 by a group of concerned residents wanting to support people in their local community. Today, we support over 7,000 Australians in New South Wales, Queensland, South Australia, Victoria and Western Australia across a variety of short and longer term mental health support and housing services. Our service model is based on the University of Wollongong s Collaborative Recovery Model, which underpins all our services and provides for consistency and measurable outcomes. We also incorporate the Optimal Health Program from St Vincent s Frameworks for Health, our own Physical Health Prompt and other complementary tools and approaches as needed. 2. Learning and innovation We have grown and developed a lot in the last three years, with a doubling of the number of people we support and in the staff who support them. This growth and development can be directly attributed to the quality of our services and the culture of learning and innovation that drives us as an organisation. We believe in learning new things, considering new perspectives and testing out new ideas as we find better ways to do things. We are always looking ahead to ensure we can take advantage of new opportunities and respond to any risks or issues that are facing us and our sector. 3.
