2 1.0 INFORMATION FOR RFP 1.1 BACKGROUND The Municipality of Red Lake requests the submission of an RFP for the following Project. PROJECT: 1.2 LOCATION: Sanitary Storm Sewer Infiltration Study Work will be performed on sanitary sewer mains within the Municipality of Red Lake. 1.4 CONTRACT DESCRIPTION: Flush, clean, inspect and camera main sewer within the Municipality of Red Lake. A detailed report and video identifying areas of infiltration to be supplied. 1.5 INSURANCE: Successful bidder will be required to provide proof of registration with the Workplace Safety and Insurance Board, and proof of liability insurance (min. $2,000,000.00) before final awarding of the contract. Throughout the project, the Contractor will ensure that the work areas will be secure in the interests of safety and left in a clean, safe condition daily and at the end of the contract. The successful bidder will be responsible for the supply and use of Personal Protective Equipment for all employees on the project as required under the Occupational Health and Safety Act. 1.6 CONSTRUCTION TIMELINES: The project start date is anticipated to be early October PAYMENT: Payment for work will be on a price per meter basis and shall be quoted as such. 1.8 SUBMISSION OF RFP Two original copies of the RFP must be submitted no later than 12:00 noon Local Time on Monday, September 19, Municipality of Red Lake Municipal Office 2 Fifth Street P. 3 RFP Documents must be sealed in an envelope marked: Request for Proposal - Infiltration Study THE LOWEST OR ANY RFP NOT NECESSARILY ACCEPTED. THE MUNICIPALITY OF RED LAKE RESERVES THE RIGHT TO REJECT ANY OR ALL BIDS, TO WAIVE IRREGULARITIES AND INFORMALITIES THEREIN, AND TO AWARD THE RFP IN THE BEST INTEREST OF THE OWNER IN IT S SOLE UNFETTERED DISCRETION Withdrawal or Substitution of Submission A Bidder may withdraw or substitute all or part of it s submission at any time up to the official closing time. The last submission shall supersede and invalidate all prior submissions. Facsimile submissions will not be accepted Right to Cancel The Municipality of Red Lake is not bound to proceed with a RFP Award and may proceed at its sole discretion, as it determines following receipt of the submissions RFP Document The RFP Document consists of five parts. a) Name of Company or Individual Information - Page 1 b) Information For Bidder - Pages 2-4 c) RFP Unit Price - Page 4 d) Signature Page - Page 5 All five parts shall remain bound together and submitted in their entirely. Page 1, 5 and specification requests are to be completed by the Bidder. Reply on the Specifications may be completed on a separate page. 1.9 VALIDITY OF RFPs The Bidder agrees that this RFP and the prices quoted in the RFP Unit Price are valid and open for acceptance by the Municipality of Red Lake for a period of 90 calendar days from the date of opening RFPs INQUIRIES/SUPPLEMENTAL INFORMATION Any questions regarding the RFP can be sent via to Todd Olson - Operations Superintendent of the Municipality of Red Lake. The questions and answers will be forwarded to Bidders in the form of an addendum. The questions may be ed to; todd.olson@redlake.ca 3

4 No oral answers or interpretations from any source unless confirmed by addenda shall modify these document or be binding. Bidders may be required to provide proof of competency by offering references to similar past project accomplishments. All work will be performed by experienced and qualified workers. 2.0 RFP UNIT PRICE The Bidder hereby offers to supply all labour, equipment and material required to: Flush, clean, inspect and camera main sewer within the municipality of Red Lake. A detailed report and video identifying areas of infiltration to be supplied following completion of work. Work offered at a price per meter basis $ per m 4
