Services for seniors needing long term care, such as in home care, adult foster care, assisted living, residential care facility and nursing home care. They also investigate elder abuse, and disabilities, regardless of economic status. AMERICAN RED CROSS Cove Avenue Red Cross provides relief to victims of disasters, Armed Forces emergency services, First Aid, CPR, water safety training, and disaster preparedness and international tracking services. They also coordinate Blood Donor programs. CENTER FOR HUMAN DEVELOPMENT, INC. (CHD) Cove Avenue CHD provides services related to alcohol & drug related issues, environmental health, mental health, public health, developmental health, Veteran s services and services for disabled community members. 4 CHILD WELFARE Gekeler Lane Union County Child Welfare provides child protection, foster care, adoption, parent training, family counseling, family sexual abuse treatment, and referrals for drug and alcohol treatment. They are located in the office of Department of Human Services. COMMUNITY CONNECTION OF NORTHEAST OREGON, INC. UNION COUNTY SENIOR PROGRAMS 1504 Albany (CCNO) CCNO provides Food Services, Meals on Wheels, In-home care, Disabled and Low-income Transportation, Energy Assistance, Summer Food Service Program and Weatherization programs. Other services include Kids Club child care, Food Bank, legal assistance information and referrals. The Housing Resource Center exists to help homeowners and prospective homeowners with a variety of services directly related to obtaining or retaining a primary residence. Administrative Office/Housing Resource Center/Weatherization Kids Club Senior Center Transportation DEPARTMENT OF HUMAN SERVICES (DHS) Self Sufficiency 1607 Gekeler Lane Department of Human Services provides medical assistance, cash assistance, SNAP (formerly food stamps) for families, the elderly and/or disabled persons who have limited income and resources. 5 Union County EMPLOYMENT DEPARTMENT /WORK SOURCE OREGON TRAINING & EMPLOYMENT CONSORTIUM (TEC) CHILD CARE RESOURCE & REFERRAL 1901 Adams Avenue The Employment Department provides local, regional and national job listings, unemployment insurance claim filing, Veterans placement assistance, career development and classes to help with job placement. Training and Employment Consortium administers the workforce investment act fund. Those that qualify for this program may receive assistance with job search, vocational assessment, academic and vocational skills and career counseling. Child resource and referral provides services to parents seeking childcare. EASTERN OREGON HEAD START One University Blvd La Grande, OR Head Start promotes school readiness by enhancing the social and cognitive development of children ages 3 to 5 through the provision of educational, health, nutritional, social and other services for low income families and children with disabilities. Hospice: Experienced and compassionate staff members provide hospice services for the terminally ill. Physical, social, spiritual, emotional care and support are key parts of the care GRH offers to patients and their families. Medicare and Medicaid may also cover hospice for those who quality. All services are free and confidential. SHELTER FROM THE STORM Island Ave Provides support services to victims of domestic and/or sexual violence. Services include shelter, 24-hour crisis line, victim advocacy, peer counseling and court support. SHRINERS S.W. 10 Northeast Oregon Public Transit E Penn Union County Northeast Oregon Public Transit provides high quality commuter, fixed route, deviated route, ADA Para- transit, demand response and intercity services in and between most communities in Northeast Oregon. They can assist veterans and their families with compensation/pensions, medical care, military records, grave markers and veteran home loans. 11 WOMENS INFANT AND CHILDREN (WIC) Cove Ave Union County Nutrition Program for Women, Infants, and Children, (WIC), focuses on the needs of pregnant and breast feeding low-income women and children. Food is provided for women and children for nutritional needs during this important time, and though coverage for women end soon after giving birth, benefits can be retained until six months after the birth of a child if the woman is breast feeding. 11 P a g e

Enrollment priority goes to children most in need, including: children living in poverty (100% of Federal Poverty Level); children with disabilities or serious health/mental health concerns; and children from families affected by mental health issues, child abuse or neglect, incarceration, parent illness/disability, domestic violence, substance abuse, homelessness, foster care and other serious concerns. BAKER COUNTY HEALTH DEPARTMENT Pocahontas Road Baker County Health Department provides immunizations, childhood vaccines, vital records, certified copies of birth/death certificates available up to 6 months after event. Exams are conducted for Sexually Transmitted Infection, HIV testing, case management, diabetes information, family planning, and home visits by a nurse for high risk pregnancies and young children, WIC nutrition program for pregnant or nursing mothers, infants and children up to P a g e

Programs include: Voucher Housing Program, Low rent public housing, family self-sufficiency program, homeownership program, transitional housing program, housing for seriously mentally ill, rent to own homeownership program and property management. Bible studies, and prayer are offered freely and confidentially to anyone experiencing a difficult circumstances. Heart of Grant County provides comprehensive family violence and sexual assault services, crisis counseling and referrals to other available services and support groups for adults and children. TB state supplied treatment at no charge after diagnosis, sexually transmitted infection exams, HIV testing, case management, diabetes information, family planning, and home visits by a nurse for high risk pregnancies and young children. JOHN DAY SENIOR CENTER NE Dayton John Day, Oregon The Senior Center provides food services to seniors. The center also houses Community Connection programs like; energy assistance, emergency utility shut offs, weatherization applications, legal assistance, options counseling, rental assistance and information and referrals. 25 Wallowa County COMMUNITY CONNECTION OF NORTHEAST OREGON, INC NW 1 ST Community Connection of Northeast Oregon provides in-home care, transportation, Meals-On- Wheels, congregate meal sites, tax aide, insurance counseling, and health screening to senior citizens, Low Income energy assistance, food bank services, emergency rental assistance, tax aide, weatherization and summer food service program to low-income families.
