1 Employment Opportunities The London InterCommunity Health Centre s mission is to provide inclusive and equitable health and social services to those who experience barriers to care. We are a growing organization that is part of a strong network of 75 community health centres across the province. Last year, our interdisciplinary staff team of 85 cared for more than 6,000 clients from across the city. Our team supports the physical and mental health care needs of our clients while assisting with issues related to the social determinants of health. On March 7, 2014 the programs and services of the Centre of Hope Family Health Team will transition to the Health Centre. Clients may continue to receive quality primary health care services at the Centre of Hope s Wellington Street location by staff employed by the London InterCommunity Health Centre. These services are funded by the Ontario Ministry of Health and Long Term Care through an agreement with the South West Local Health Integration Network. The Health Centre is currently hiring a number of staff to provide care primarily at the Centre of Hope site (work at other locations may be required). Full-time Medical Secretary (2 positions) As a front line ambassador to the Health Centre, the Medical Secretary plays a key role in maintaining a welcoming tone in the reception area. The Medical Secretary will greet all callers, clients and visitors, take messages, and provide accurate information regarding internal and external programs and resources. The Medical Secretary must be resourceful, and capable of troubleshooting a range of requests while prioritizing urgency. The Medical Secretary role provides clerical and administrative support for an interdisciplinary team of registered health care providers. The Medical Secretary will be required to open and close the building at times, following relevant security procedures. Fax: (519) Community Mental Health Nurse (Dundas Street location) Within the context of a client and family centered care model, the Mental Health Nurse will provide a high level of clinical assessments and recommendations for care to clients who are seriously affected by psychiatric conditions and substance use. As part of an interdisciplinary team and working within a Rehabilitation Model you will also provide psycho education and case management to clients, their family and other support systems. The Mental Health Nurse is required to work with people with severe chronic illness, addictions, poverty, homelessness or risk of homelessness as well as those from multicultural communities. 4 Systems Navigator As a Systems Navigator you will provide intake to determine eligibility for Health Centre services based on defined criteria. You will inform new and existing clients about the Health Centre s programs and services and assist clients with navigating internal and external health systems. Additionally, you will act as a resource to clients, colleagues and community residents. This position requires the ability to engage with people experiencing complex chronic illness, mental health conditions, addictions, poverty, homelessness or who are from a variety of cultural backgrounds.
