5 TENA Assessment Guide Common Causes of Persistent and Long-Term Urinary Incontinence TYPES DESCRIPTION SYMPTOMS Urge or Overactive Bladder (UUI or OAB) Involuntary and uninhibited bladder contractions (commonly referred to as overactive bladder), causing bladder pressure to exceed urethral pressure sufficiently enough to cause urine loss. Bladder contractions are due to an unstable bladder or detrusor over-activity that may be associated with central nervous system disorders, e.g., stroke, Parkinsonʼs disease. Intense and sudden urge to pass urine Usually little warning time so unable to delay or postpone voiding after sensation of bladder fullness (urge) is perceived Associated with other symptoms such as frequency (voiding more than 8 times in a 24 hour period, or nocturia awakening more than once to void) Urine loss on way to bathroom Moderate to large amounts (several milliliters) of leakage Timing of urine loss can be unpredictable Stress Urinary Incontinence (SUI) Urethral sphincter dysfunction due to relaxation and weakness of the pelvic floor muscles and reduction in urethral resistance. As women age, many develop intrinsic urethral sphincter dysfunction. Small amount of urine leakage occurs with physical activities, exercise or any action which increases with intra-abdominal pressure, e.g., coughing, laughing, sneezing, walking, exercise. Mixed Overflow A combination of both bladder and urethral dysfunction, causing stress and urge incontinence. A hypotonic or acontractile detrusor secondary to neurologic problems such as diabetic bladder neuropathy, multiple sclerosis or spinal cord injury. The cause of the urine leakage is that the bladder does not contract adequately. Another cause is urethral obstruction due to urethral stricture, enlarged prostate, pelvic organ prolapse. Combination of both urge and stress UI symptoms Palpable or percussable bladder Elevated PVR (more than ml in elderly persons) Interrupted urinary flow (start and stop voiding) Post-void dribbling Continual leakage of small amounts of urine
