Medica Medica Direct Short-Term has 30, 60, 90 day coverage with a range of deductibles. Coverage can begin the day application is received. Preferred One Short-term medical insurance coverage provides immediate health care coverage when you need it. 2 HEALTH INSURANCE INFORMATION Minnesota Health Information This website was created by the Governor s Health Cabinet and provides a range of information about the cost and quality of health care in the state. You can compare private insurance providers, learn how to be a more informed health care consumer, and access links to a variety of healthrelated websites. MN Department of Health The MDH website contains a wide array of health-related information, including: *Health Care and Coverage (Insurance) info: *Guide to MN s Public Health Care Programs: *Guide to Additional Health Care Resources: MN Insurance Help Line Information/help line staffed by retired insurance agents through the Insurance Federation of Minnesota. Monday-Friday 9:00 AM to 3:00 PM. You can call or ask a question online and they will call you back. Not limited to health insurance. U.S. Uninsured Help Line Available 24 hours a day, seven days a week, in multiple languages. Information Specialists can describe public and private options for which you may be eligible, connect you to representatives from various plans, and help find additional resources pertaining to your unique health insurance situation. Coverage for All A health coverage locator with four ways to help you find health coverage and determine eligibility for public and private health plans. The site contains links to applications for public programs and to private plan quotes. You can also take a health insurance eligibility quiz at to see a specific list of public and private health plans that you may qualify for. Portico Healthnet Helps connect people to free or low-cost insurance coverage in Dakota, Ramsey, and Washington counties. PUBLIC HEALTH PROGRAMS 2
3 Minnesota has three main publicly funded health care programs. Certain criteria must be met to qualify for public health care coverage. Medical Assistance (MA): Minnesota s Medicaid program. Available to low-income individuals and families who fit into one of the designated eligibility groups. General Assistance Medical Care (GAMC): Provides medical care to low-income Minnesotans who don t qualify for MA or other state and federal programs primarily serves adults without dependent children. Minnesota Care: A publicly subsidized program for residents who lack access to health care coverage. Premiums are based on sliding fee scale according to family size and income. Generally, you must be without health insurance for 4 months before you can be eligible for MinnesotaCare. Application takes approximately 30 days to process. Assured Access (type in Assured Access) To qualify for Assured Access, you must live in Hennepin County, meet certain income guidelines, and be ineligible for public assistance health care programs. Assured Access is NOT health insurance. You will receive an Assured Access card and a list of participating clinics; the clinics will determine your discount on a sliding fee scale and you will be billed the remaining portion. You may apply online, request an application by phone, or pick up an application in person. Applications are processed in approximately 4 weeks. Health Coverage Tax Credit Federal tax credit that can pay 65% of qualified health insurance premiums for eligible individuals. To receive the HCTC, you must be enrolled in a qualified health plan and meet certain requirements, such as being a TAA (Trade Adjustment Assistance) recipient. Minnesota Comprehensive Health Association MCHA is a nonprofit Minnesota corporation established by the Minnesota legislature. MCHA offers insurance for individuals who have been turned down by private providers due to pre-existing conditions, often known as the high risk pool. Sage Screening Free breast and cervical cancer screenings to uninsured and underinsured women who meet income guidelines. You can access a list of participating clinics online or call for more information. Veterans www1.va.gov/health/index.asp Information on health benefits available to veterans. DENTAL RESOURCES 3
4 MN Dental Association Minneapolis District Dental Society St. Paul District Dental Society Contact these organizations to locate lower cost dental care in your area. Premier Dental See website for rates and application. Denticare of Minnesota Prepaid dental plan that can save 50-70% off dental bill; NOT insurance. University of Minnesota Dental Clinic Comprehensive dental clinic for adults and children, 25-30% cheaper than private dentists. Accepts medical assistance and all insurance plans. Copay based on individual s insurance. Does not used sliding fee scale. Payment due at time of service. PRESCRIPTION ASSISTANCE Family Wize Free prescription drug discount program. No income, age, or eligibility requirements. Cards can be picked up at certain local United Way agencies. See website for list of pick-up locations and participating pharmacies. Merck Programs If you have no prescription drug coverage and cannot afford your medications, you may be eligible for free Merck medicines through the Patient Assistance Program. If you have no prescription drug coverage, regardless of your income, you may be eligible for discounted Merck medicines through the Prescription Discount Program. Minnesota Rx Connect Allows you to compare prescription medication prices at local pharmacies. Also gives information on ordering medication through a state program that purchases medications through Canada. Needy Meds 4
5 Provides information on assistance programs. Pfizer PFriends Provides savings on 88 Pfizer medicines at participating pharmacies. The program helps qualified patients without prescription coverage, regardless of age or income. Ramsey/Anoka County Prescription Drug Card Provided by your local county government and the National Association of Counties. No enrollment form, no membership fee. Cards can be used at participating retail pharmacies to save an average of 20% on prescription drugs. Cards can be picked up at Workforce Centers and libraries. Rx Hope Web-based patient assistance resource. Rx Outreach Provides generic versions of name brand drugs. Must meet income guidelines. The only cost is an administrative fee. Together RxAccess Most cardholders save 25-40% on brand name drugs. To qualify, you must: be ineligible for Medicare, have no prescription drug coverage, meet income requirements. CLINICS & ADDITIONAL HEALTH RELATED RESOURCES Bloomington Public Health Division Clinic fees are based on income and family size. If a person cannot pay the fee, most services are still available. Must live in Bloomington, Edina, or Richfield. Community University Health Care Center Community clinic available to everyone. Walk-in service for medical, dental, and mental health care. Pediatric and OB providers. If you don t have insurance, they can help you apply for public programs or set up a payment plan based on your income. Sliding fee scale for those who qualify. Southside Community Health Services
6 Same day appointments available, walk-ins welcome. Payments on a sliding fee scale based on income for those who qualify. Must present proof of income at appointment to be eligible for discount. St. Mary s Health Clinic People without private health insurance who are not eligible for government programs, who live in the seven-county metro area, and who meet income guidelines are eligible. Also serves those in transition between jobs. Free services, appointments needed. To determine eligibility, one should call and speak with an intake and scheduling coordinator. Healthy Minnesota Women General information on women s health. Medline Plus Website to help Minnesotans find health information. MN Senior Health Options Specially trained advocates to help coordinate senior health care benefits and community resources. Must be eligible for Medical Assistance and Medicare. Neighborhood Health Care Network Network for the Minneapolis-St. Paul metro area. You can access a clinic directory, search for clinics in your area, and find information for men, women, insured and uninsured persons. 6
