Distribute Appedix 2.1. Ask learers ot to write aythig util they are istructed to do so. Poit out that these are two examples of forms that require similar iformatio as the database etry form used by the HR perso. 3. Have learers work i pairs to compare the forms to idetify similarities ad differeces. 4. Debrief as a group. Poit out that although forms may look very differet from each other, the strategies used to complete the forms are the same. Describe the steps learers ca take to become familiar with a ew form: Sca the form for headigs. Skim the form to idetify areas that should be completed. Review the headigs to idetify what is required. Decide if the field requires etries i specific formats, i.e. block letters. 5. Activity 3: Askig Questios to Complete a Form 1. Draw learers attetio to the istructio at the top of the Marste Isurace form o Appedix Teach or review how to formulate questios if ecessary. Braistorm as a group the questios the supervisor could ask to collect the iformatio to fill i the form, as well as the questios or requests the supervisor ca pose to address commuicatio problems.
