2 Employees' Rights and Responsibilities The Occupational Safety and Health Act of 1970 (OSHA) states that "each employer shall furnish to each of his employees employment and a place of employment which is free from recognized hazards that are causing or likely to cause death or physical harm to his employees." [Section 5 (a)(1) - generally known as the General Duty Clause] To that end this plan was developed and implemented on August 29, 2005, by our office as a means to ensure a safe work environment for our employees and to communicate appropriate actions to be taken by employees in the event of a fire or other emergency that might occur in our office. It is imperative that ALL EMPLOYEES know the proper steps to follow in the event of an emergency. In order that employees are familiar with this information, ALL EMPLOYEES must do the following: 1. Read and understand the components of the Emergency Action Plan as stated in this document. 2. Contact the Emergency Action Plan Manager if there are any questions concerning the Emergency Action Plan. 3. Verify to the employer that you have read and that you understand the Emergency Action Plan by signing the Employee Confirmation Form. The Emergency Action Plan Manager for this office is: The OSHA Deputy Coordinators for this office are Melissa Scurlock, Debbie Joyner, and Sue Wilson. A copy of the Emergency Action Plan may be accessed from your computer terminal. Written copies are available in the office of in Room 475 of the Hamilton Eye Institute. 3 Posters detailing employee information published by OSHA (OSHA Poster #3165) and TOSHA (Tennessee Department of Labor #337132) are located on the Workplace Safety and Health Resources Website and on the bulletin board or other conspicuous location in each office. You MUST know the location of these posters and be familiar with their contents. THESE POSTERS MUST NOT BE REMOVED FOR ANY REASON. The Emergency Action Plan that has been developed for our office is designed to detail the procedures to be followed by the employee in case of a fire or other emergency that might occur in the office. The separate elements of the plan are: A. Definitions B. Means of Reporting Fire or Other Emergency C. Emergency Evacuation Plan D. Specific Emergency Situations E. Rescue and First Aid F. Portable Fire Extinguishers G. Training Requirements H. Record Keeping Requirements [29 CFR ] The Hamilton Eye Institute at The University of Tennessee Page 4 of 22
5 A. Definitions 1. Emergency Action Plan - a plan for a workplace, or parts thereof, describing what procedures the employer and employees must take to ensure employee safety from fire or other emergencies. [29 CFR (i)] 2. Emergency Escape Route - the route that employees are directed to follow in the event they are required to evacuate the workplace or seek a designated refuge area. [29 CFR (j)] 3. Means of Egress - a continuous and unobstructed way of exit travel from any point in a building or structure to a public way. This includes intervening room spaces, doorways, hallways, corridors, passageways, balconies, ramps, stairs, enclosures, lobbies, escalators, horizontal exits, courts and yards. 6 B. Means of Reporting Fire or Other Emergency [29 CFR ] 1. Any employee who discovers a fire, smoke, or chemical spill with large amount of fumes shall immediately REMOVE any patient and/or other employee from the room and CLOSE the door to the room. The employee should then go to the nearest phone and notify Campus Security by dialing and informing the dispatcher as to the nature and location of the emergency. [29 CFR (b)(4)] 2. Emergency Telephone Numbers, printed on bright red-orange paper, are posted at several conspicuous locations in the office and by clicking on the red T icon on your computer desktop. [29 CFR (b)(4)] 3. The employee should then activate any local fire alarms available on their individual floor. Pulling the handle in this box will activate the local fire alarm siren and flashing strobe lights, alerting all employees and patients of the need for evacuation of the building. Local FIRE ALARM PULL BOXES are located in the hallways of the Second, Third, and Fourth floors on the left hand side of the doors that EXIT to the East Stairs and West Stairs. An additional FIRE ALARM PULL BOX is located on your left in the stairwell of the West Stairs on the First Floor as you approach the first exit door. Another FIRE ALARM PULL BOX is located in the lobby near the elevators of the 930 Building. The locations of these FIRE ALARM PULL BOXES are also shown on the evacuation diagrams for each floor. They are identified by the letter F. 4. Medical emergencies should be reported to the physician present in the office or, if no physician is present at the time of the emergency, to Campus Security at and informing the dispatcher as to the nature and location of the emergency and the need for an ambulance. [29 CFR (b)(4)] 5. In this office the FIRST AID KIT is kept in the Procedure Rooms on the Second and Fourth Floors on the countertop. 7 C. Emergency Evacuation Plan EMERGENCY ACTION PLAN 1. Since each floor has its own master evacuation plan, employees should know and understand the most direct evacuation route from their work area to the outside. In the event of a fire, smoke, vapors from a chemical spill, or other emergency that requires evacuation of the building, the employee will move quickly and calmly along the prescribed route. Patients should be escorted from the office along the same route. DO NOT USE THE ELEVATOR OR ALLOW PATIENTS TO USE THE ELEVATOR. 2. Evacuation diagrams are located on the wall on all floors of the building. Each employee should be familiar with the location of and the directions shown on these diagrams. Copies of these diagrams are also found in each employee's OSHA Handbook. Large Green EXIT signs (Red in the West Stairs First Floor exit route) in the ceiling identify the direction to the stair exits. Follow these exit signs to the nearest exit stairway. There are two stairs available for emergency exit, The East Stairs and the West Stairs. The East Stairs exit on the First Floor onto the driveway on the East side of the building. Watch for Emergency Vehicles using this driveway. From there you should move to the parking lot on the East side of the HEI and check in with the OSHA Deputy Coordinator. Keep away from the street area or fireplugs so as not to interfere with emergency personnel and equipment. The West Stairs exit into the lobby on the First Floor of the Hamilton Eye Institute. From the lobby proceed through the revolving door or other doors onto the Madison Avenue side of the HEI. Proceed eastward (to the LEFT as you exit the door), assemble in the parking lot on the East side of the HEI, and check in with the OSHA Deputy Coordinator. Keep away from the street area or fireplugs so as not to interfere with emergency personnel and equipment. 3. After evacuation of the building ALL EMPLOYEES will check in with their OSHA Deputy Coordinator so that it can be verified that all employees have safely evacuated the building. The OSHA Deputy Coordinator for the Hamilton Eye Institute will be at the assembly location holding a BLAZE- ORANGE sign. Employees must check in with her/him IMMEDIATELY after evacuation. 4. Evacuation Drills will be held on an annual basis. Documentation will be kept in the Emergency Action Plan Manual. The Hamilton Eye Institute at The University of Tennessee Page 7 of 22

14 a. All places of employment, passageways, storerooms and service rooms shall be kept orderly and in a sanitary condition. [29 CFR (a)(1)] b. The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition... [29 CFR (a)(2)] c. To facilitate cleaning, every floor, working place, and passageway shall be kept free from protruding nails, splinters, holes or loose boards. [29 CFR (a)(3)] d....aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. [29 CFR (b)(1)] e. Exits shall be so located and exit access be so arranged that exits are readily accessible at all times...[29 CFR (f)(1)] f. All exits shall discharge directly to the street, or to a yard, court, or other open space that gives safe access to a public way. [29 CFR (f)(6)] g. Means of egress shall be continuously maintained free of all obstruction or impediments to full instant use in the case of fire or other emergency. [29 CFR (k)(2)] h. Exits shall be marked by a readily visible sign...[29 CFR (q)(1)] The Hamilton Eye Institute at The University of Tennessee Page 14 of 22
15 D. Specific Emergency Situations [29 CFR Appendix] 1. If the emergency is a chemical spill, the employee will put on proper personal protective equipment and begin containment procedures. Personal protective equipment is available in the following locations: Eye Protection (goggles, etc) Gloves Procedure Room Examining Rooms The SDS (Safety Data Sheet) for the chemical involved should be consulted for additional precautions or special clean-up techniques. These sheets are located in the SDS Notebooks with the OSHA written plans in Room 475 and on the departmental OSHA website. 2. In the event of an earthquake, employees should NOT try to evacuate the building or move to other floors in the building. They should stand in a inside doorway or crouch under a desk or table away from windows, glass dividers, book cases, cabinets, or mirrors. Hold onto the desk while the earthquake is in progress. Maintain this position until the shaking stops. If no desk or table is available, move to an inside doorway and protect your head with your arms or other object. Be prepared for aftershocks. DO NOT ATTEMPT TO USE THE ELEVATOR. Move away from the building after the quake has subsided. Avoid power lines, exposed electrical wires, bridges, overpasses, or any other structure that might fall. 16 3. In the event of a tornado alarm (outside sirens continuously sounding), employees should move away from the outside rooms and go to a center hallway or room away from windows. An interior area on the bottom level of the building is preferable. Stairwells or interior bathrooms are alternative areas as long as there are no windows. Employees should stand away from windows, glass dividers, bookcases, cabinets, or mirrors. Employees should remain in this location until the "All Clear" is sounded. 4. Due to the climate in our area, a blizzard emergency is not reasonably anticipated. 5. Due to the topography of the office building location and the location of the office on the Second, Third, and Fourth Floors, a flood emergency in the office is not reasonably anticipated. 6. Due to the geographic location of our office, a hurricane emergency is not reasonably anticipated. Thunderstorms or tornadoes may sometimes follow the inland path of a hurricane, and precautions for these emergencies should be followed for this event. 7. Due to the small quantity and nature of chemicals present in this office, a major toxic chemical spill in the office in not reasonably anticipated. 17 E. Rescue or First Aid [29 CFR ] 1. In the event of an injury to one of the employees, one of the office physicians should be informed immediately, and the employee examined to determine the nature and extent of the injury. Appropriate immediate treatment should be rendered, and the employee referred to a secondary treatment facility if required. If no physician is in the office, then a Technician will render first aid, transport to the nearest Emergency Department, or, if necessary, call for emergency transportation (ambulance) (Call Campus Security ). 2. In the event that chemicals, blood products or other potentially infectious materials are splashed in the eyes of the employee, the eyes should be irrigated promptly and copiously with one of the eyewash fountains located in the Procedure Rooms on the Second and Fourth Floors. [29 CFR (c)] 3. The First Aid Kits are located in Procedure Rooms on the Second and Fourth Floors on the counter tops. F. Portable Fire Extinguishers [29 CFR ] 1. There are portable fire extinguishers located in the hallways on all floors. Their locations are indicated on the Evacuation Plans with a letter X. However, during a fire emergency employees should activate the fire alarm at the pull boxes and immediately evacuate the building as described in paragraph I, C of the Emergency Action Plan. Therefore, no maintenance or training is required for employee use of fire extinguishers. [29 CFR (b)] The Hamilton Eye Institute at The University of Tennessee Page 17 of 22
18 G. Training Requirements [29 CFR (a)(5) 1. A copy of the Emergency Action Plan and Fire Prevention Plan will be given to ALL CURRENT EMPLOYEES AND NEW EMPLOYEES WHEN THEY ARE HIRED (including part-time and temporary employees). After the employee has read and understands the Emergency Action Plan, he/she will sign the Employee Confirmation Form. These forms will be filed with other OSHA training documentation. 2. ALL EMPLOYEES will attend a Training Session during which the Emergency Action Plan and Fire Prevention Plan will be explained, the various procedures will be detailed, and an opportunity to ask questions will be afforded. This training session will cover the following elements: a. Location of a copy of 29 CFR 1910; b. Explanation of Emergency Action Plan and the location of the written plan; c. Means of reporting fire or other office emergencies; d. Location of emergency medical tray; e. Location of oxygen tank; f. Locations of emergency evacuation diagrams; g. Procedure for verifying complete evacuation of all employees; h. Procedure for chemical spills, earthquake, tornado, or other emergency in the office; i. Location of First Aid Kit; j. Explanation of Fire Prevention Plan and the location of the written plan; k. An opportunity to ask questions. The Hamilton Eye Institute at The University of Tennessee Page 18 of 22
19 3. This training session will be given at the implementation of the Emergency Action Plan and will be repeated on an annual basis or whenever changes are made in the plan. Annual training sessions will address the entire plan, while special sessions will emphasize only the changes made in the plan. 4. New employees will be instructed on the details of the Emergency Action Plan as part of their Orientation Procedure. 5. A record of these training sessions including the date, time, employees attending, person conducting the training, and the contents of the training session will be maintained in the Emergency Action Plan book. 20 H. Record Keeping Requirements 1. The following records will be kept in order to document the implementation of the Emergency Action Plan, the confirmation that the employees have read and understand the plan, the initial training of the employees has been accomplished, and the continued instruction of employees in the content of the Emergency Action Plan has been completed: a. Employee Confirmation Form b. Training Session Record 2. These records will be kept in the OSHA Coordinator s office with the other OSHA Training documentation for a period of 5 years unless changes in the OSHA regulations in the future require the records to be kept for a longer period of time.
