1 Employment Pathway Plan (Voluntary) Name: JACK GREEN Job Seeker ID: On the date this plan was signed, I discussed with DEMONSTRATION ORGANISATION FOR S.PROVIDER TRAINING the activities I agreed to undertake and the appointments I need to attend to assist me in finding a job and prepare for work. I am aware that if I can t attend my appointments or activities, I must contact my employment services provider (or Centrelink if I don t have a provider) in advance. These activities and appointments have been included in this Employment Pathway Plan, which has been negotiated under the Social Security Act Goals - these are my employment goals. The activities and appointments in this Plan will help me reach these goals. Short-term: I want to keep my job at ABC as a Data Entry clerk working part-time for 15 hours per week. Long-term: I want to work more hours, to earn a better wage and to visit family in Europe. Employment - this section lists all forms of paid employment I am doing or intend to undertake. I agree to undertake 30 hours per fortnight of Part-time work from 15/07/2013 to 12/01/2014 and will report my earnings to Centrelink. Jack is keen to work with ABC in his Data Entry role, DES Provider and Jack have investigated transport options and created contingency plans for any work-related issues. DES Provider arranged workplace assessment on 12/07/13. Voluntary I agree to get in touch with Bevan on if I am having any issues during my probation period. from 15/07/2013 to 12/01/2014. Jack and Bevan have agreed to a placement support plan: DES Provider and Jack will debrief at the end of each work day for the first week, minimum of fortnightly reviews following, stand-by strategy for transport issues outlined (taxi fares). Voluntary Appointments - this section lists the appointments and interviews I will attend. I will contact my employment service provider or Centrelink in advance when I know I am unable to attend. 2 I agree to attend fortnightly Disability Employment Services appointments with DES Provider. Now that Jack is working it is not practical for him to attend weekly so we have agreed to meet fortnightly in person and to alternate those contacts between DES Provider office and Jack's workplace (or nearby cafe) during his 6 month probation. Voluntary Employment services provider assistance - this section lists the ways in which my employment service provider will assist me to meet my employment goals. Transport and Licensing Assistance - DES Provider has given Jack 5 x cab vouchers that he can use to get to and from work if transport becomes problematic. DES Provider is also available to drive Jack to work if he needs that. Provider Services Assistance - DES Provider has created a placement support plan that Jack has agreed to. This includes regular contact schedule, contingency plans and strategies for Jack and his employer ABC. Work Related Clothing and Presentation Assistance - Jack needs more than one suit for his new job so DES Provider will provide an additional $200 towards work clothing to enable Jack present confidently and professionally in his first few weeks. Wage Subsidy Assistance - DES Provider will assist ABC to access $1,650 from the Wage Subsidy Scheme to help offset the costs of hiring and training Jack. Jack is aware that a small subsidy is being paid and is okay with the reasons DES Provider and ABC outlined. Page 2 of 6

I understand that my Employment Pathway Plan includes activities that I have agreed to do (voluntary activities). These do not affect my income support. My provider has explained how my personal information and privacy will be protected. 5 Information You Need to Know About your Employment Pathway Plan Your Employment Pathway Plan has been tailored to your individual needs and sets out the things you have agreed to do to gain sustainable employment. It covers your individual pathway to employment, including the assistance you will receive while you are looking for work. The Goals section lists the employment goals you have decided on with the help of your employment services provider (or Centrelink). The activities and appointments listed in the other sections of your Employment Pathway Plan will help you reach these goals. The Appointments section records the appointments you have agreed to attend with your provider or Centrelink. Some appointments will be marked as compulsory (appointments you need to attend to get your income support payments) and others may be marked as voluntary (appointments you should attend to improve your chances of finding employment). The other sections of the Employment Pathway Plan record the activities you have agreed to do to reach your employment goals. These may include: activities to improve your job search skills; training or educational courses; work experience activities and activities that will help you to overcome any personal barriers to employment you have. Again, some activities will be marked as compulsory (activities you need to participate in to get your income support payments) and others may be marked as voluntary (activities you should participate in to improve your chances of finding employment). If you have an employment services provider, the Employment services provider Assistance section will list specific ways your provider will support you to gain sustainable employment. For example, your provider may be able to provide help for you to travel to job interviews or jobs, or may be able to help you to prepare job applications. Your Employment Pathway Plan replaces any previous plans or agreements you have had and starts from the day you sign it. It is very important to note that you may not be paid income support if you don t enter into an Employment Pathway Plan when asked to do so. If you can t agree on an Employment Pathway Plan you can call the Customer Service Line on What happens if I don t do what is listed in my Employment Pathway Plan? No Show No Pay - If you don t attend a compulsory activity listed in your Employment Pathway Plan without having a reasonable excuse, or behave inappropriately at an activity, you might get a No Show, No Pay failure. If this happens you can lose a day s income support, for each day that you fail to attend or participate properly in your activity. You may also get a No Show No Pay penalty and lose a day s income support if you don t attend an arranged job interview without a reasonable excuse or if you attend a job interview but deliberately act in a way that results in a job offer not being made. Attending appointments - If you don t attend a compulsory appointment then your income support may be stopped and Centrelink will contact you to arrange a new appointment. You will usually be back paid once you agree to attend a new appointment. If you don t attend the new appointment, your income support may be stopped again. This time, if you don t have a reasonable excuse, you will lose your income support until you do attend an appointment. You will not get any back payment for the time you missed. Comprehensive Compliance Assessments - If you don t attend activities for three days or miss three appointments over a six-month period, Centrelink will conduct a Comprehensive Compliance Assessment. During this assessment Centrelink will look at why you have not met your requirements. If the Comprehensive Compliance Assessment determines that you have been intentionally not meeting your obligations, you may lose your income support for up to eight weeks (a serious failure). Serious Failures - If you refuse to accept a suitable job that is offered to you, you may lose your income support for up to eight weeks. If, after a Comprehensive Compliance Assessment, you are intentionally not meeting your obligations, you may lose your income support for up to eight weeks. Your payment can be reinstated if you agree to undertake a Compliance Activity. This will generally be 25 hours a week of fulltime Work for the Dole for eight weeks, but may include other similar activities if appropriate. What happens if I leave a suitable job or I am dismissed from a job due to misconduct? If you leave paid work without a reasonable excuse, or are dismissed because of misconduct, you will generally not be eligible to receive income support for eight weeks. How do I avoid penalties? To avoid penalties you should go to all appointments that Centrelink or your provider arranges for you or tell them in advance if you can t make it. Make sure to read all mail from Centrelink and your provider and get back to them if they ask you to do something. What should I do if I can t do the things I have agreed to? If you can t do an activity listed in your Employment Pathway Plan, or can t attend an appointment that has been arranged for you, contact your employment services provider (or Centrelink if you do not have a provider) as soon as possible. If you do so your provider may make another time to attend your activity or appointment. If you don t contact beforehand when you are able to do so, your income support payment may be Page 5 of 6
6 reduced, or stopped, even if you have a good reason for not being able to attend. If there are good reasons why you are unable to look for work or undertake the activities listed in your Employment Pathway Plan, you should discuss these with your provider or Centrelink. If your personal circumstances are affecting your ability to undertake the activities listed in your Employment Pathway Plan, please discuss these with Centrelink or your employment services provider. What happens if I get some paid work? If you or your partner has been successful in finding work then you need to tell Centrelink about any income you or your partner have received, in the same fortnight you worked. This will help Centrelink to make sure you are paid the correct amount of income support. If you are overpaid you may need to pay the money back to Centrelink. Your local Centrelink office can help you to declare your income correctly. What if I disagree with a decision that has been made? If you disagree with a decision Centrelink has made about your Employment Pathway Plan, or have concerns about the service you have received, you should contact a Centrelink Customer Relations Representative as soon as possible on If you disagree with a decision your provider has made about your Employment Pathway Plan, or have concerns about the service you have received, you can contact your employment services provider to discuss your concerns or you can call the Department of Education, Employment and Workplace Relations Customer Service Line on , who will investigate your concerns. If you are not satisfied with the response you receive, you may take the matter further by contacting the Ombudsman s Office on Privacy You ll be pleased to know that your privacy is protected by law. Your personal information can only be disclosed where Commonwealth law allows it, or in special circumstances, or when you give permission. You can get more information about privacy and the way in which your personal information can be used from: your employment service provider; Centrelink; the Department of Education Employment and Workplace Relations or the Privacy Commissioner (who can be contacted through Page 6 of 6
