1 JOB POSTING Personal Support Worker (PSW) LOCATION: Huron and Perth HOURS OF WORK: fulltime, part time and casual REPORTS TO: Supervisor (applicable program area) DATE: November 1, 2013 Expression of Interest We are seeking interested internal applicants for fulltime and part time positions across various locations, and for various programs, within Huron and Perth. The creation of these positions has been necessitated by a number of culminating factors increased difficulty scheduling PSWs for available service requests, increase in referrals, enhanced program hours of service, and the need for flexibility of workers to travel across Huron and Perth. Our desire is to create a group of PSWs that are highly trained, flexible and adaptable to the changing and evolving needs of our funders, partners, clients and families. These positions will be eligible for group benefits, RRSP program, paid sick/personal days, scheduled vacation time, applicable travel time, and professional development and training courses. We are also considering premiums for workers that fill these positions and meet a defined set of criteria such as completing cross-training requirements for all types of PSW positions across the agency, fulfilling difficult service requests, regularly travel significant distances to meet referrals demand, and other yet to be determined criteria. Immediate Openings: full time, part time, and casual positions are as follows: PSW Adult Day Program (ADP)Respite Locations: Wingham 1 st and 3 rd weeks, Goderich 2 and 4 th weeks (Monthly Schedule) Could apply for one or both locations. Hours: Thursday and Friday 4pm-11pm (3 positions) Thursday and Friday 11pm-8am (4 positions) Must have/agree to complete U-first, Gentle Persuasive Approach and Montesorri Approach Could be a home help worker if experienced with personal care Could be other certificate such as SSW, DSW, college level Rec and Leisure, Activation, Restorative Care Preference for PSWs for overnight roles PSW Community/In Home Seeking 8 fulltime and 12 part time Every other weekend mandatory PSW eshift (Palliative and Pediatric Care) Seeking 4 fulltime and 4 part time All hours are overnight, weekends included Meet all the requirements of the eshift PSW Position Description (see below) Page 1 of 5

2 GENERAL PSW POSITION Description The PSW assists clients with normal activities of daily living that the client is unable to perform unassisted. Under the direction of the Supervisor, the PSW will perform duties as outlined in the service plan for the client(s). The duties vary according to the individual clients and families receiving service. The PSW must work within a goal-directed, outcome based and Client Driven Care philosophy. 1. Client Care: a. Bathing, grooming, dressing, general hygiene, ambulation assistance, feeding b. May assist with specialized care under the direction of a regulated health care professional and only after receiving specific training for the specific client from a regulated health care professional c. General day to day housekeeping of the home d. Laundry either in the home or at nearest facility e. Prepare meals with respect to diet, budget and being conscientious about storing meals in advance f. Respite relief to caregiver by offering supervision and companionship to client g. Working with client to establish routines h. Escort client to doctor appointments, shopping or banking i. Support to client(s) through their presence and accessibility 2. Teaching: a. Encourage a healthy lifestyle b. Act as a role model in performing housekeeping skills, personal care, child care and nutrition c. Demonstrate methods of housekeeping, personal care, child care and nutrition d. To motivate clients towards self-sufficiency and continued independence 3. Recording and reporting: a. Take initial information on client and document on service plan b. Complete Safety Assessment on first visit and twice a year c. Complete daily report sheets and other documentation as required in client s home after each visit and submit the reports to the Supervisor at the end of each month or when client is discharged d. Report changes in the status of the clients and/or family with regards to emotional, social, financial, physical or environmental aspects to the Supervisor immediately e. Collaborate with the client and Supervisor to develop a client goal directed, outcome based service plan and schedule of duties as per the Client Driven Care Philosophy 4. Seek guidance from the Supervisor whenever in doubt. 5. Respect and maintain the confidentiality of the client s and/or families being serviced and with respect to Agency business. 6. Be reliable and arrive at assignments on time. 7. Notify the office of any illness, emergencies or delays immediately. 8. Maintain healthy & safe working practice and report any health and safety concerns. 9. Follow ONE CARE code of ethics, legal responsibilities and professionalism and health & safety policies. 10. Maintain a positive image of ONE CARE in the community. Page 2 of 5

3 11. Understand and follow the Client Bill of Rights. 12. Upgrade skills and knowledge through appropriate agency and community approved educational programs. 13. Knowledgeable of ONE CARE programs and services at selected sites in order to educate the clients by enabling them to meet their needs from a holistic and wellness perspective and remain in the community. 14. Participate in ONE CARE Continuous Quality Improvement activities. 15. Familiar with the CCAC contract with regards to the Personal Support and Homemaking tasks. 16. Participate in the Client Driven Care Research initiatives of the SWCCAC and other future opportunities and initiatives related to quality improvement. 17. Accountable for being knowledgeable about the health and safety program and safe working procedures. 18. Maintain a safe work environment and report concerns to Supervisor. 19. Attend mandatory training and meetings. 20. Accountable for being knowledgeable about Agency Policy and Procedures. 21. Perform other tasks as assigned. 1. Current PSW Certificate, HSW working towards PSW Certificate or comparable, e.g. RPN. 2. Current Emergency First Aid and CPR level C. 3. Demonstrated problem solving, ability to assess situations, set priorities and decision making skills. 4. Demonstrated willingness and ability to be adaptable, build relationships, focus on consumers needs and communicate effectively at all levels of the Agency and community. 5. Excellent interpersonal, communication and time management skills. 6. Valid Ontario driver s license. ESHIFT PSW POSITION Description The eshift Personal Support Worker will assist in the care provided to clients receiving shift palliative care in their own homes. This will be achieved through a holistic approach meeting the physical, emotional, psychological and spiritual needs of the clients and their families. Some examples are providing or assisting with personal care, assist with preparing and giving food and fluids, as requested by the client/family, observing and reporting changes in client condition and functioning, helping clients determine levels of pain and symptom distress using ESAS ratings and reporting them to the Delegating Nurse. This position requires the epsw to be an integral part of the palliative care team which includes the Visiting Nurses and the Delegating Nurses. 1. Assist in the administration of medications and record these medications in a communication book for the client and family as well as in the electronic client record. 2. Assist in personal hygiene and basic care. 3. Provide attendant care services including transferring, lifting, bathing, feeding, and personal care. 4. Maintain electronic records through communicating findings via an iphone. Page 3 of 5

4 5. Implement established programs as directed. 6. Maintains a professional manner at all times in the performance of duties. 7. Protect and promote the welfare, dignity, and individual rights of our clients at all times. 8. Adhere to the goals, policies, and aims of RCCP in all professional activities. 9. Provide basic health and personal care for each client. 10. Provide each client with meaningful companionship, observation, safety and conservation. 11. Ensure the client s surroundings and personal effects are clean, tidy and serviceable. 12. Employ safe work practices at all times. 13. Supervise clients exercise routines, set-up and provide leisure activities for clients, and accompany clients on outside recreational activities if appropriate. 14. Perform other duties related to client care. 15. Take clients blood pressure, temperature, pulse and respirations. 16. Report or record fluid intake and output. 17. Observe or monitor clients status and document client care in electronic chart via iphone. 18. Collect specimens such as urine, feces or sputum. 19. Administer suppositories, colonic irrigations and enemas as approved client specific delegated acts. 20. Perform other procedures as directed by visiting nurses and delegating nurses. 21. Make client s bed and maintain client s environment. 22. Maintain inventory of supplies. 23. Clean equipment. 24. Assemble setup and operate job-related equipment. 25. Assist clients in and out of taxis, ambulances, cars or helicopters. 26. Deliver and collect linen, rubbish, infectious waste, specimens and equipment. 1. Graduate of an accredited school for Personal Support Worker. 2. Current CPR Certification required 3. Required 50 hour enhanced eshift PSW training program 4. Therapeutic verbal and written therapeutic communication skills in English 5. Ability to work well with others in a team approach, and adapt to changing situations. 6. Physically capable of performing assigned duties within a flexible work schedule inside a 24 hours a day, 7 day a week operation. 7. Knowledge of and abilities in transferring, lifting, and other attendant care duties. 8. Knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation and evaluation of nursing care. 9. Must be familiar with laws, regulations, and guidelines governing health care provision. 10. Ability to rotate shifts. 11. Excellent teamwork and team building skills 12. High degree of resourcefulness, flexibility, and adaptability. 13. Able to effectively communicate both verbally and in writing. 14. Politically and culturally sensitive. 15. High level of critical and logical thinking, analysis, and/or reasoning to identify underlying principles, reasons, or facts. 16. Strong morals and ethics, along with a commitment to staff privacy. 17. Valid Ontario driver s license. Page 4 of 5

5 ADP RESPITE PSW POSITION Description We are seeking an ADP-Respite PSW who, under the direction of the ADP Supervisor, will be responsible for service delivery in the Adult Day Program setting including program planning and implementation and direct client care, including overnight care. 1. Plans, implements and participates in a variety of therapeutic, social and recreational activities for clients. 2. Assists clients in performing activities of daily living including bathing and toileting. 3. Maintains accurate and confidential client records. 4. Works cooperatively and communicates effectively with team members including staff, volunteers and caregivers. 5. Ensures the rights and privacy of clients, volunteers and staff are maintained. 1. Diploma or certificate in Recreation, Activation, Gerontology or related field or equivalent combination of education and experience. 2. Experience in an Adult Day Program setting preferred. 3. Excellent communication skills both written and verbal. 4. Knowledge of the physical, social and psychological aspects of the aging process. 5. Emergency First Aid and CPR certificate. 6. Gentle Persuasive Approach and Montessori training completed.
