5 Sponsor Church Feeding Site Center Camp or Playground
6 Types of organizations that can sponsor the Program A unit of local, county, municipal, State, or Federal government (e.g. park, library, township, city) A public or private non-profit school food authority A public or private non-profit residential private camp A public or private non-profit college or university participating in the National Youth Sports Program (NYSP) Any other type of private non-profit organization
7 » Must provide a year-round service to community» Must demonstrate financial and administrative capability» Must not have been declared seriously deficient» Must serve in areas where poor economic conditions exist» Must accept an agreement with the Illinois State Board of Education (ISBE)
8 » Must exercise management control over sites New sponsors may operate only 1 or 2 sites» Management responsibilities cannot be delegated below the sponsor level» Have a valid Data Universal Numbering System (DUNS) number

10 » Attend a full-day training with the state agency» Train all staff throughout the operation of the Program» Maintain all records for at least three years + costs incurred, Disbursements, meal counts» Absorb any costs that are incurred when there is not enough reimbursement.» Submit online Sponsor application, site application(s) and budget.
11 » Decide sites and locations» Notify: local media of your intention to operate the Program area grassroots and minority organizations local health department of food preparation and serving sites (must attach a signed copy to the online application)»decide how meals will be prepared Self-prep Contract with a school food authority Contract with a food service management company (FSMC)»Monitor sites for compliance
