1 SEMINOLE RIDGE FOOTBALL CHEERLEADING CONSTITUTION/REQUIREMENTS I. Purpose: The primary purpose of the cheerleading squads is to support Seminole Ridge High School activities and sports. II. Tryouts A. Clinic Information 1. What to bring: a. The Athletic Accident Insurance Form (attached) and $10 fee (Check or Money Order made payable to Seminole Ridge High School) must be brought to the April 10, try out clinic. If the form and money are not presented at registration, the student will not be permitted to try out. Everyone trying out must pay $10. b. Once a student has made the squad, the $70 balance must be paid. 2. See below for important dates and the selection process. Football Cheerleading o Varsity will be made up of 10 th, 11 th and 12 th graders o Junior Varsity will be made up of 9 th, 10 th and 11 th graders Saturday, April 10, 2010 o 2-3:30pm Try-out material (2 cheers & a dance) will be taught to all girls interested in trying out. o SRHS Gym o What to wear: t-shirt, shorts and athletic shoes Tuesday, April 20, 2010 All Grades o What to wear: All girls will wear red cheer shorts, white t-shirt with sleeves & no writing on it, hair up and no jewelry! Remember this is your first chance to make a good impression. o 3:00 4:00pm Try-outs for current 9 th, 10 th and 11 th graders in SRHS gym o 4:30 5:30pm Try-outs for current 8 th graders in SRHS gym Wednesday, April 21, 2010 o 3:00pm will be sent out to announce the squads Thursday, April 22, 2010 o 2:48pm uniform fitting in the auditorium for varsity and jv (current 9 th, 10 th, & 11 th graders) o 4:30pm or as soon as you get out of school uniform fitting in the auditorium for current 8 th graders who made the JV squad o 5:30-6:30pm Mandatory Parent Meeting for Varsity and JV Football Cheer Squad meeting in the auditorium. o We will be completing paperwork, please bring health insurance information. o You will be required to pay a minimum of $175 for the clothing order from Varsity at this time. o You will also be required to pay the $50 deposit for cheer camp at this time. B. Eligibility Requirements 1. Applicants currently in the 9 th, 10 th or 11 th grade must have a cumulative 2.0 grade point average at the time of the try-out and must maintain a 2.0 to remain eligible. Incoming 8 th graders are all academically eligible, but must maintain a cumulative 2.0 grade point average after the 1 st 9 weeks. 2. After a student has been selected to a squad, an Athletic Eligibility packet must be complete and returned before the student may attend cheer camp. This packet will be available at the parent meeting: a. Physical Exam physicals are good for one year. Student athletes must be given a new physical each year. b. Copy of birth certificate, incoming 8 th graders and new cheerleaders c. Notarized medical consent form d. Last page of Athletic Eligibility packet must be notarized as well. 3. The Varsity football squad will be made up of 10 th, 11 th & 12 th graders. The JV football squad will be made up of 9 th, 10 th & 11 th graders. 4. The try-out being held on April 20, will be for football for the varsity squad for the junior varsity squad. o o A separate try-out for basketball cheerleading will be held after school starts in the fall. Girls may try out for whichever squad they chose or all of them.

2 C. Judging Process 1. The girls will be divided into groups of 3 or 4. The girls will be able to choose their own groups. Eighth graders will be grouped together. 2. There will be three stations set up in the gym. At the first station the group will demonstrate the first cheer and their toe touch, at the second station the group will demonstrate the second cheer and at the last station the group will demonstrate the dance. All stations will be operating at the same time. Once a group has completed all three stations, they are free to go. 3. Below is a sample score sheet stating what and how girls will be judged during the try-outs on April 20. Station 1 & 2 Motions Toe Touch (station 1) Knowledge of cheer Spirit Smile Station 3 Motions Knowledge of dance Rhythm Smile Appearance Deductions -5 wrong shirt -5 wrong shorts -5 hair not up -5 wearing jewelry 4. Judges will be area cheer coaches, former cheerleaders, and uniform representatives and Mrs. Dickmann (Varsity Cheer Coach). III. Practices A. The Varsity squad, will be enrolled in Eurhythmics class. They will be placed in the class by the guidance department after they have been selected to the squad. Practice will be held during the class. The class will be for 1 st semester only. Be sure to select a class for 2 nd semester. B Junior Varsity will practice at a time and location on campus to be determined by the JV coach, usually Mondays & IV. Wednesdays from 3:00-4:30pm. Summer Camp A. The JV & Varsity will attend a camp at Seminole Ridge July 26-28, from 8:30am 4:30pm. Attendance is MANDATORY! B. All members will attend the camp. Camp and the time spent working together in crucial to team building. C. The cost is $105 V. Expected Behaviors & Standards A. If selected for the team, you will be a representative of Seminole Ridge High School and will act accordingly. B. While attending games, home or away, as well as cheerleading camp, you are bound by all rules and regulations of the student handbook. C. See Article VIII, for the exact Discipline Suspension and Dismissal Policy D. If you quit a squad, you will not be able to try out again.

3 VI. Discipline Suspension and Dismissal Policy A. Suspension from the squad will result if cheerleader drops below a cumulative grade point average of 2.0. She will however remain in the Eurthymics class. B. Removal from the squad will result if a member of the school faculty, staff or administration, observes a cheerleader in possession of or using tobacco, alcohol or drugs. C. Removal from the squad will result from the following behavioral infractions: 1. Insubordination to coach. 2. Suspension from school (out of school suspension) for any reason. D. Demerit System 1. Each cheerleader will begin the season with 100 points. A cheerleader will be removed from the squad if her point total falls below 60 points. 2. Varsity only Eurthymics Class a. Each football game will count as 100 points as part of your Eurthymics grade. It will be considered the performance/test component of what we do in class. b. Ways to lose points from your performance/test grade will be the following: -2 points for every minute late to a game -2 points for every minute late returning from 1/2 Time -10 points for forgetting poms for game -10 points for not wearing hair as instructed to game. -10 points for not coming dressed & ready to go for the game (including hair); with an additional - 2 points for every minute it takes you to get ready -5 points for each incidence of wearing jewelry (practice or game) -40 points for missing a game (excused or unexcused) -5 points for bra straps showing (including clear straps) -5 points wearing wrong color bloomers 3. Varsity only general demerits -40 points for missing a game (excused or unexcused) 2. Junior Varsity Ways to lose points a. -2 points for every minute late to a game or other team event b. -2 points for every minute late returning from 1/2 Time c. -1 point for every minute late to practice/class d. -10 points for forgetting poms for game e. -10 points for not wearing hair as instructed to game. f. -10 points for not coming dressed & ready to go for the game (including hair); with an additional - 2 points for every minute it takes you to get ready g. -5 points for each incidence of wearing jewelry or chewing gum (practice or game) h. -20 points for missing practice (excused or unexcused) i. -40 points for missing a game (excused or unexcused) VII. VIII. j. -5 points for bra straps showing (including clear straps) Costs A. UCA Cheer Camp $105 B. Camps clothes and other cheer supplies - $300 estimated Fundraising A. All fundraising will be done using individual accounts. All members will participate in all fundraising activities. If you choose not participate, you will be charged the amount you failed to raise. It is understood some people are able to pay out of pocket for expenses. However, the fundraising we do benefits the entire team. The squads have expenses during the course of the year and this is the only way we can cover those costs. B. Fundraiser available to defray costs. 1. Kids Summer Cheer Camp June 7-10, 2010 at Seminole Ridge PARTICIPATION IS MANDATORY! You will receive $10 for each child you refer. Getting referrals is optional, but an excellent way to make easy money. 2. ESPN the magazine fundraiser Sell a 2 year subscription of 52 issues for $40 and you ll receive $30 to defray your costs Example: Sell 2 subscriptions and earn $60 X. Safety A. Stunting or tumbling without the supervision of the coach is strictly prohibited. B. The squad will follow accepted cheerleading safety guidelines, which are available for review upon request.

4 Interscholastic Athletic Accident Insurance STUDENT NAME: (Please Print) DATE OF BIRTH: / / INTIAL SPORT: Other Sports: Effective August 2, 2004, all high school interscholastic athletes and cheerleaders will be required to contribute $80 toward the cost of interscholastic athletic accident insurance. At the time of try-outs, only $10 is required. If the student makes the team, the balance of $70 will be due. This annual (school year) contribution will be used to help offset the School District s cost in providing quality accident insurance for our athletes. According to School Board Bulletin #Sp-1615-FO/EBRM, this contribution of $50 per student athlete/cheerleader will be collected PRIOR TO participation in any interscholastic athletic activity (including try-outs). This fee in NON-REFUNDABLE, and, once paid, will continue to cover the athlete in additional sports within the given school year period. Please refer to the attached sheet for specifics of the Interscholastic Athletic Accident Insurance coverage. Please return this sheet with all requested information and your $10 payment attached. If using a check, make it payable to Seminole Ridge Community High School. 5 INFORMATION PERTAINING TO ATHLETIC INSURANCE Policy Features of the Basic Athletic Accident Insurance Coverage is provided by Monumental Life Insurance Company and is administered by Bollinger Inc. Who is covered: All registered and enrolled interscholastic athletes and cheerleaders, which includes all interscholastic sports and football. Benefits are payable for covered expenses that are not recoverable from another health care plan. If the insured is not covered by another health care plan, the excess provision shall not apply, and the benefits are payable as primary. If you are not covered by another health care plan, you are strongly encouraged to select a health care provider from the CCN directory. Your athletic director has a copy of this directory or you can access it via the Internet at Claim Forms: This form must be completed by the parent and the school. You need to submit the form to Bollinger Inc. within 30 days of the accident, even if you are still waiting for an Explanation of Benefits from your primary carrier. Claim forms may be obtained from the athletic department of your High School. Incurred Period: Covered claims must be incurred within 730 days from the date of the accident for the Basic Coverage. Initial Treatment: Injury must be treated within 30 days after it occurs. Exclusions: 1. Injuries which are not caused by an accident; 2. Injury sustained as a result of practice or play in senior high interscholastic tackle football, unless the premium for such coverage had been paid; 3. Re-injury or complications of a condition for which medical advice or treatment was recommended by a physician or received from a physician within a two-year period preceding the effective date of the coverage of the Insured; 4. Treatment performed by a family member or person retained by the School; 5. Injury due to: acts of war; suicide or intentionally self-inflicted injury, while sane or insane; violating or attempting to violate the law, the taking part in any illegal occupation; fighting or brawling except in self defense, or loss in consequence of being legally intoxicated as defined by the laws of the state in which the loss occurs; or under the influence of any drugs or narcotic unless administered by or on the advice of a physician; 6. Medical expenses for which the Insured is entitled to benefits under any (1) Worker s Compensation Act; or (2) mandatory no-fault automobile insurance contract; 7. Expenses for which there is no obligations to pay; 8. Treatment or loss resulting from hernia, regardless of cause, Osgood Schlatter s disease, or osteochondritis; 9. Injury sustained as a result of operating, riding in or upon, or alighting form a two, three or four-wheeled recreational motor vehicle or snowmobile; 10. Any expense for which benefits are payable under Catastrophic Accident Insurance Program of the State High School Interscholastic Activities Association; 11. Bacterial infections, sickness or disease or any kind such as strep throat or tonsillitis, heat exhaustion, sunburn, frostbite, fainting, allergic reactions, except those which occur as a result of accidental ingestion of pus forming infections which occur through an accidental cut or wound; 12. Vegetation poisoning such as poison ivy or poison sumac, or ptomaine poisoning; 13. Expense incurred for treatment of temporomandibular joint dysfunction and associated myofacial pain; 14. Private air travel, to include ballooning or ultra-light aircraft; parachuting; hang-gliding; bobsledding; travel in or upon a snowmobile; ATV (all terrain vehicle); or any two or three-wheeled motor vehicle; 15. The repair or replacement of prescriptions of eye glasses, contact lens or hearing aids; 16. Experimental procedures; 17. Serving in the armed forces of any country or international authority.

6 APPLICATION FOR SEMINOLE RIDGE COMMUNITY HIGH SCHOOL FOOTBALL CHEERLEADING TRY-OUTS My child has my permission to be a cheerleader at Seminole Ridge Community High School. I understand that she must abide by the rules and regulations set forth by the coach of Seminole Ridge Community High School and be present for all practices and games. I have read the Cheerleading Constitution/Regulations and understand that the violation of any of these may lead to temporary or permanent suspension from the squad. I understand that this form, along with a copy of the January 2010 report card or print out showing cumulative grade point average, must be completed and returned to Mrs. Dickmann in room 6101, placed in her mailbox, or received in the mail (current 8 th graders only) on or before April 9, 2010, or my child will not be allowed to try out. I understand that I must attend a mandatory meeting on April 22, in the auditorium from 5:30-6:30pm. I understand that at that time I will be required to pay $175 for cheer clothes ordered from Varsity, as well as a $50 deposit for cheer camp. I will arrange for my daughter to be at the uniform/camp clothes fitting on April 22 at 2:48pm for current 9 th, 10 th and 11 th graders and 4:30pm for current 8 th graders. I understand the selection process and that my daughter will be evaluated by qualified people and we agree to abide by the decision of the judges. I understand all costs involved as stated in the Constitution/Regulations. I understand by the very nature of the activity, cheerleading and gymnastics carry a risk of physical injury. No matter how careful the participant and coach are, how many spotters are used, or what landing surface is used, the risk cannot be eliminated. I understand these risks and will not hold Seminole Ridge Community High School or any of its personnel responsible in the case of accident or injury at any time.
