1 CASE STUDY-POLICY ANALYSIS OF NUTRITION IMPACT OF AGRICULTURE AND FOOD SYSTEMS IN SIERRA LEONE UN NETWORK MEETING NAIROBI, 2013

2 INTRODUCTION In Sierra Leone, two-thirds of the population depends on agriculture (FAO, 2013) accounting for the country s 46% GDP (CAADP, 2011) Hunger and malnutrition levels have not changed significantly since 2000(six districts report levels above 70%-CFSVA2011. Food insecurity coupled with poor household nutrition and health practices results in unacceptable malnutrition among children with only two districts and the western area recording tolerable/acceptable levels of less than 25%. Agricultural activities in Sierra Leone have high propensity to influence nutrition outcomes Therefore, there is a need to better understand how agricultural and food systems impact nutrition outcomes in Sierra Leone.

3 Nutrition profile INTRODUCTION CONT. Low Birth Weight Infant 24%(MICS 2005) Underweight Prevalence 18.7% (SMART 2010) Stunting Prevalence % (SMART 2010) Wasting Prevalence 6.9% (SMART 2010) Exclusive breast feeding rate 32%(MICS 2010) Continued breast feeding rate at 12-15months is 84% and at months is 48% (MICS 2010) 76% of children aged 6-59 months have some level of anaemia (DHS 2008) 46% Women of child bearing age are anaemic(dhs 2008) >95% children 6-59 months receive Vit. A-Mass Distribution June % Children months de-wormed Mass Distribution June 2013 (During the maternal and child health week)

4 INTRODUCTION CONT. The broad objective of the case study was to review how food and agricultural policies are intended to have an impact on nutrition outcomes in Sierra Leone Specific objective were to: To perform nutrition and food security situational analysis To analyse the nutrition sensitivity of the specific agriculture and food policies/ action plans/programmes To describe the relevant policy processes and alignments, including an analysis of supporting factors and constraints To identify opportunities on how to make the selected policies/action plans/programmes more nutrition-sensitive

5 Study Methodology Desk Review of Secondary Data/ Reports/Documents (2) In-dept interviews with Key informants in agriculture and nutrition (3) Meetings with stakeholders/ Institutions/Organisations/Agencies. Relevant national food and agriculture policies analysed were: National Sustainable Agriculture Development Plan ( ) National Food and Nutrition Security Policy Implementation Plan ( ) Health and women advancement policies (gender) were analysed as secondary policies

6 Key Findings: National Food & Nutrition Security Implementation Plan Policy statement: All relevant organizations should integrate nutrition considerations in programmes and activities. Nutrition sensitivity: Calls for all actors including those in agriculture to act on nutrition in a multi-sectoral way. However, the relevance of this policy is not direct to achieving nutrition outcomes as there is no guarantee that the sectors will make commitments to integrating nutrition outcomes. Proposed ways of improving nutrition sensitivity: There should be policy/strategy that requires that all relevant sectors (notably agriculture) to have nutrition or nutrition-based goals or objectives in their programming/intervention plans.

7 Key Findings: NFNS Plan Policy Statement: Small-scale farmers should be provided with effective support to improve domestic food production. Nutrition sensitivity: Crop diversification, food storage and diversification ensures that food is available for all the population groups in Sierra Leone Proposed way of improving nutrition sensitivity Nutrition education to farmers and develop dietary guidelines Dietary guidelines should be prescribed in the policy to guarantee that the general population are advised to consume diversified foods

8 Key Findings: The National Sustainable Agriculture Development Plan (NSADP, ) Policy Statement: Commodity diversification: Promotion of smallholder commercialisation schemes Nutrition sensitivity: Crop diversification promotes availability of diverse food for consumption including the micronutrient rich foods Gender considerations: Women play an important role in nutrition and their involvement in agriculture can lead to improved nutrition status due to their caring roles Proposed way of improving nutrition sensitivity: Dietary guidelines need to be developed and used so that crop diversification can lead to dietary diversification Engender agriculture: Most agricultural tools are male friendly while women are still using mortar and pestle for rice processing which are rudimentary and laborious. Time saving technologies should be introduced to allow women more time for caring for the family and engage in other economic activities

9 Key Findings: The National Sustainable Agriculture Development Plan (NSADP, ) Policy statement: Agriculture (and related) infrastructure development: Rehabilitation and modernization of existing storage and processing facilities and equipment Nutrition sensitivity: Effective food processing and storage enables nutrients to stay for a long time and cover the lean periods when food availability is a challenge. Proposed way of improving nutrition sensitivity: Food processing and storage is a challenge in Sierra Leone as indicated by the abundance wastage after harvesting. More initiatives and efforts needed in promoting appropriate food processing and storage methods.

10 Key Findings: The National Sustainable Agriculture Development Plan (NSADP, ) Policy statement: Sector coordination and management including with the ministry of health Nutrition sensitivity: Call for multi-sectoral responses provide an opportunity for agriculture to closely work with nutrition sector for improved nutrition outcomes. Farmers need to be in good health and thus provision of health services is needed. Malaria and schistosomias which lead to anaemia are common in rice growing areas due to the flooded rice paddies. Proposed way of improving nutrition sensitivity: Although there is evidence of linkage between agriculture and nutrition sector, there is no intentional effort that is geared towards improvements in nutrition outcomes. Although there is women in agriculture and nutrition unit in the ministry of agriculture, there is no nutritionist. However recruitment of nutritionist for theministrybyfaoisinprogress. Need for health education among farming folk especially in prevention of Malaria and schistosomiasis (which can lead to anaemia) and other vector borne illnesses.

11 RECOMMENDATIONS There are nutrition sensitive agriculture strategies and/or approaches/activities that are being implemented by government partners which need to be articulated by the policy documents. A policy/strategy for all relevant sectors to include nutrition objectives in programming should be instituted. there is need to add indicators to adequately monitor and evaluate nutrition sensitive agriculture. Develop and promote dietary guidelines to guide various groups on what available foods to consume. Increase nutrient density of staple foods through community-based fortification of rice and cassava as part of value addition. Review agricultural policies to include nutrition sensitive strategies. Institute agricultural and food policies that have the propensities to prevent overweight, obesity and other diet related noncommunicable illnesses.

12 KEY MESSAGES Large scale agriculture projects should focus on nutrition at country level. During program implementation agriculture and nutrition should not be considered as separate entities. For nutrition programs to have sustainable results, the need for agriculture based activities involving identification and production of crop types that will help close the energyprotein gaps (and provide micronutrients) Making agriculture activities focus on nutrition sensitive interventions across the value chain targeting crops with high economic and nutritional value (fruits, vegetables, and legumes) are essential.
