3 Student Biography Your student bio may not exceed 75 words. Please use the CAMBRIA REGULAR, 11 pt, and CAMBRIA BLACK for any bold text. For Italic fonts, choose CAMBRIA ITALIC. The important thing to remember is NOT to highlight and make something bold or italic when typing in the Microsoft Word document as this causes a printing problem. The font must be either a bold font (CAMBRIA BOLD) or an italic font (CAMBRIA ITALIC). Thank you! 4 Middle School Upper School Theatre Production Participation Form Note: This form MUST be completed and signed by you and a parent/guardian, and presented at the time of your audition. Thank you! Students must attend ALL rehearsals for which they are called. If the student should be absent for any reason, the director must be contacted. If rehearsals are missed without pre arrangement, an actor can be replaced at the director s discretion. Each actor is allowed ONE (1) EXCUSED absence. If an actor misses more than his/her one absence, the actor will be replaced. My son/daughter and I understand that the production of Dear Edwina must have first priority, after school work, September 4 December 7, Students must maintain a 2.5 GPA in order to participate. If this is not maintained throughout the rehearsal process, a student may be removed from the show. All students must arrange a way to get home and be picked up ON TIME! Rehearsals are CLOSED to ALL friends and family members. All students must come to rehearsal prepared with their script, a pencil, a recording device (for music) and a bottle of water. Gum is not allowed during the rehearsal period. Eating is not allowed during the rehearsal period. Water is permitted. Have your snack BEFORE rehearsal begins. Each person must give their full attention and cooperation to their fellow cast members and the production staff. You must be QUIET when the director is working with other people. Sit quietly and study lines while you are waiting to go on stage to work. Please accept constructive criticism from your Director, Musical Director, Choreographer and Technical Director. During Tech Week, when a member of the production staff interrupts rehearsals in the theatre, you are to stand quietly until rehearsal resumes.

5 A non refundable theatre production fee of $150 will be billed to the student s school account. This fee includes costumes, scripts, music CDs, props, make up, food and snacks during rehearsal period and tech week. This fee will be waived if a student sells at least $300 in ads for the show s program to be handed out at performance. All cast members are required to sell 10 (TEN) tickets to the show. Tickets are $5 a piece. By auditioning, you are agreeing to accept any role given to you. All actors MUST participate in the striking (the taking down of the set and cleaning everything up and storing away) of the set following the closing performance. RESPECT YOUR PEERS, TEACHERS, AND PRODUCTION STAFF. ABOVE ALL, REMEMBER, HAVING FUN IS THE KEY TO A SUCCESSFUL PRODUCTION. NOTE: By signing this agreement, you understand what is expected of you as a cast or tech member of this show. By signing this agreement, you are committing and agreeing to all rules and expectations and you are RESPONSIBLE if you break this commitment. Appropriate consequences apply if agreement is broken.
