Meeting the Contraceptive Needs of Teens and Young Adults: Youth- Friendly and Long-Acting Reversible Contraceptive Services in U.S. Family Planning Facilities. Journal of

Lessons Learned Before engaging in efforts to increase numbers of youth, health centers should: Assess current clinical practices is the health center implementing evidence-based clinical practices (e.g., QFP, MEC, SPR)? Are health center staff trained on continuous quality improvement and addressing the needs of adolescents? Are partnerships being made in the community for linkage and referral of youth to health center? Are staff in organizations that will provide the linkage/referral trained (e.g., schools, juvenile justice, WIC, foster care) Are there outreach efforts to reach at-risk

Evidence-based Clinical Recommendations and Guidelines Ensure reproductive health visits begin during early adolescence & include: Reproductive health counseling Screening for sexual activity Anticipatory guidance/delay counseling when appropriate Provision of contraception Screening for sexually transmitted infections (STIs)

Quality Family Planning (QFP) Recommendations Define what services should be offered in a family planning visit, & describe how to do so. Most relevant for teens: Provide quality contraceptive counseling Offer confidential services & observe all relevant state laws & any legal obligations Provide comprehensive information about how to prevent pregnancy Encourage communication between the adolescent & his or her parent(s) or guardian(s) about sexual & reproductive health Educate pregnant adolescents about contraceptive options for the post-pregnancy period, including benefits of immediate

U.S. MEC for Contraceptive Use: Recommendations for Evidence-based guidelines on which contraceptive methods can be used safely by women with certain characteristics and medical conditions 17 contraceptive methods >120 medical conditions can safely use all methods of contraception Including implants & IUDs Teens with medical conditions & characteristics Obesity Smoking Diabetes

U.S. Selected Practice Recommendations for Contraceptive Use (SPR) Exams & tests needed before starting contraception Guidance on the few exams or tests needed before starting contraception

Community-wide Initiatives: Efforts to Improve Implementation of Evidencebased Clinical Practices Synthesized recommendations, guidelines, & literature Developed evidence-informed checklist of best practices 31 Adolescent Reproductive Health Best Practices Provide technical assistance & training to ensure quality implementation of best practices Monitor & evaluate health center partners performance Identify opportunities for health center improvement Increase access and provision of youth-friendly reproductive health services Increase use of moderate and highly effective contraceptive methods

Conclusion Youth-friendly services that address the needs of adolescents have the potential to increase access to & utilization of reproductive health services. Improvement efforts should focus on strategies to build the capacity of health centers to ensure that evidence-based clinical practices are implemented and services are youth-friendly. Successful strategies to increase youth (referral and linkage) include partnerships, training, outreach, and integration with other youth serving organizations in the community.
