1 GESA CREDIT UNION GESA VISA RELOADABLE PREPAID DEBIT CARD AGREEMENT Card Center: This Prepaid Card Agreement contains the terms and conditions of the purchase and use of the Prepaid Card (Card) with Gesa Credit Union (Credit Union)."We" "us" and "our" refer to Gesa Credit Union. "You" and "your" refer to the person who has obtained a Card from us. "Card" means any VISA reloadable Prepaid Card obtained from us as provided in this Agreement and based on our policies and procedures. By obtaining a Card from us, activating the card, and by using the Card or by permitting anyone else to use the Card, you agree to the terms of this Agreement on your behalf and your authorized users. 1. Nature of Card. The Card is a prepaid payment device which must have funds loaded to it prior to use. The Card is not a gift card, credit card or charge card and does not constitute a checking, savings or other demand deposit or consumer asset account. The Card is not a payroll card and cannot be used to make payroll to anyone. The Card may only be used for personal, family, or household purposes. You agree to use the Card only for lawful purposes. You cannot use your Card for online gambling or any illegal transaction. The Card will remain our property and must be surrendered upon demand. The Card is nontransferable, and it may be canceled, repossessed, or revoked at any time without prior notice subject to applicable law. IMPORTANT INFORMATION ABOUT PROCEDURES FOR A PERSON WANTING A CARD - To help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. What this means for you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver s license or other identifying documents. 2. Card Purchase and Authorized Users. The Card can be purchased by anyone who has a valid Credit Union account in his or her name. A minor who is an account owner and at least 14 years of age may purchase a card with the written permission of the minor s parent or guardian. The Card is and will remain our property. However, you will be solely responsible for the possession, use and control of the Card. You must surrender the Card to us immediately upon request. The Card is non-transferable. The person who purchases the Card is the only person authorized to use the Card. Please do not give your Card to anyone else to use. You may request one (1) additional Card for any additional user that you would like to authorize, which must be separately activated and which can be used in the same way as the Card at ATMs and merchants that accept VISA debit cards. The additional user must be an account signer who must be present when purchasing the card and registered as the user. Cards may not be purchased for individuals under 14 years of age. You are responsible for all authorized transactions made with this additional Card. Cards may not be used for Internet lottery, betting, or gambling transactions or for any illegal transactions.

2 3. Card Activation a. Card Activation Steps. You will be required, for validation purposes, to provide personal information. At the time of activation, for your protection and for regulatory purposes, we may request, and you agree to provide, some additional identification verification information such as your home phone number, date of birth, social security number and zip code. We may use this data for a range of purposes, including but not limited to facilitating refunds if your Card is lost or stolen, enhancing usage at merchants that may require zip code authorization, and aiding in collection efforts in the event of a shortage, as defined herein. We will hold your information in confidence in accordance with the section below entitled "Privacy." If you do not wish to provide the data, or if the data you provide us cannot be appropriately verified, we reserve the right to cancel your Card and reimburse you for the Initial Load Amount. b. PIN Issuance. You will be issued a PIN when you purchase your card at one of our branches. You will need to activate your card by following the instructions that come with the card. You will be mailed a personalized card shortly thereafter that must also be activated by following the instructions that come with the card. At the time of activation, your card balance will be transferred to the new card, and the old card will be deactivated. 4. Card Use You will need your PIN to access cash from ATMs and to use at all merchants abroad. This card may not be used at merchants requiring a chip card. If you enter your PIN incorrectly three or more times, your Card will be locked. You must contact the Card Center on the first page of this agreement to reset your PIN if your card is locked or you have forgotten your PIN. They are available at any time 24/7/365. a. Card Transactions. You may use the Card to make the following transactions: To make purchases and to obtain cash back when you make a purchase at any merchant that accepts VISA or any of the networks listed on the back of your Card (though some merchants may not provide cash back). To obtain cash from participating ATMs that accept VISA or any of the networks listed on the back of your Card or from participating financial institutions. There are daily, weekly, and monthly limits on the frequency of transactions, amount of cash you obtain from ATMs, the amount that can be loaded on the card, the amount that can be withdrawn in cash, and the amount that can be used for purchases. See the Fees and Limits for Gesa Credit Union Visa Reloadable Prepaid Debit Card sheet for a list of fees and restrictions. The frequency, amount, and timing of cash withdrawals made from ATMs may be limited by the ATM operator. There may be other limitations on the types, frequency, and timing of cash withdrawals established by us from time to time, without notice for security purposes. b. Card Loading and Reloading. You may reload the card to increase the maximum value stored on the card at any time after issuance. You may load value to your card at a Credit Union branch or by calling us at By transferring available funds to your card, you will be able to use funds sooner. Even so, funds may not be available for

3 immediate use after you have initiated the transfer. If you use checks to fund the card, a 24 hour hold for government checks or a 48 hour hold for other checks will be placed on the funds. You may also load funds by direct deposit of non-government payments to your card. In the future Green Dot service may be available for use in the loading of cards. Green Dot service would allow cardholders to add funds to their cards at select merchants. c. Direct Deposit Account. The Credit Union s routing number and direct deposit account number are for the purpose of initiating direct deposits to your Card only. You are not authorized to provide the Credit Union s routing number and direct deposit account number to anyone other than your employer or payer. You agree not to direct deposit federal payments to your reloadable card. If presented for payment, preauthorized direct debits will be declined and your payment to the merchant or provider will not be processed. d. No Share Insurance. If you move funds from your Credit Union checking or savings account to the Card, those funds will no longer be subject to NCUA share insurance. e. Card Balances. You may only use the Card when there is a balance available on the Card and only up to the amount left on the Card and any transaction fee due. All use at an ATM or for purchases will be deducted from your Card, including any transaction fees. Any transaction made that exceeds the available balance will be declined. The Card does not permit you to overdraw any account that you may own, and any activity on the Card that exceeds the value stored on the Card will normally be rejected. In addition, if the Card does not have sufficient value stored for a transaction, the entire transaction will be rejected. However, there can be times when a VISA merchant processes a transaction against the Card without prior authorization. If an overdraft occurs, you will be required to either reload funds onto the Card or make a payment to us to cover the negative amount. In all cases, you authorize us to withdraw sufficient funds from any of your deposit accounts to cover any negative amount that may arise. Some merchants may, subject to limits, allow you to use your Card for cash back on purchases, and cash may be advanced at financial institutions. Cash back is only available at US merchants with your Card; it is not available at international merchants. f. Split Tender Transactions. To make a purchase for more than the Available Balance on the Card, you may be able to use your Card with another form of payment to make a purchase. This is known as a Split Transaction. To make a Split Tender Transaction : Present the Card for payment and tell the cashier the Available Balance and ask whether another form of payment will be accepted for the balance of the purchase. The cashier will process the transaction and the Available Balance will be authorized for payment The cashier will then ask for a second form of payment to cover the balance of what you owe for your purchase. g. Authorization Holds. When you use your Card to pay for goods or services, certain merchants may ask us to authorize the transaction in advance and may estimate its final value. When we authorize the transaction, we commit to make the requested funds available when the transaction finally settles and may place a temporary hold on your Card s funds for the amount indicated by the merchant. These holds count toward the daily spending limits for the card as listed on the Fees and Limits for Gesa Credit Union Visa Reloadable Prepaid Debit Card sheet.

4 If you use your Card to purchase meals or drinks many restaurants and bars factor a tip into the price of up to 20% when preauthorizing Card purchases, but you will be debited for only the actual amount you authorize. Hotels and cruise liners operate under a similar policy, usually preauthorizing a 20% tip. Should you use your Card to preauthorize a reservation (such as with a hotel, airline, travel agency, cruise company, rental car company, tour operator, or amusement park); any amount requested in a preauthorization will not be available to you to spend using the Card, although you will only be finally debited with the actual amount you spend based on the provider's policies. If seven days elapse without you authorizing an amount, any preauthorization will typically be cancelled and that amount will again be available to you. Some merchants preauthorization holds may last as long as 30 days. Hotels are one such example. Automobile rental businesses may add 20% to the rental to allow for any additional expenses although the amount finally debited should match the final bill. In addition, the rental contract may authorize the rental company to debit the Card for damage caused to a rental car. You should read the rental contract carefully to understand the policy of the particular rental company for additional debits to the Card. Gas stations will routinely obtain an authorization on the Card for an estimated purchase amount to ensure that the Available Balance will cover the final purchase. To avoid having a gas station obtain authorization for an amount that is higher than the Available Balance, we suggest that you prepay inside at the cashier for an amount equal to, or less than, the Available Balance on the Card. When making a purchase by phone, mail, or online, the access information you provide must be the same information as registered for the Card. A difference may lead to a declined authorization. You can also use your Card to make a purchase over the internet or by mail-order. However, some internet and mail-order merchants do not permit Split Transactions. If you experience any difficulty making a purchase online or by mail-order with the Card, please call the Customer Service Number for assistance. h. Card Activity Information. You can review your Card balance and recent transaction activity records by using the Card Center contact information on the first page of this agreement. This information is available to you 24 hours a day, 7 days a week. Your Card number and some of the information requested when you ordered the Card are required for authentication. i. Merchant Purchase and Returns. Any merchant honoring the Card is required to obtain approval or authorization for any transaction. The balance available for authorizing transactions with the Card is the lesser of your available balance or any dollar limitation disclosed for the transaction being performed. If you use the Card to buy goods and you want to use some other method of payment for part of the cost, please advise the store in advance and use the Card first before paying any amount by other means. Each merchant may approach the ordering of payments differently, and you cannot be assured that the Card will be used first for payment. We are not responsible for any injury to you or to anyone else caused by any goods or services purchased or leased with your Card. If you are entitled to a refund for any reason for goods or services obtained with a Card, you agree to accept credits to your Card in place of cash. If you wish to return any merchandise purchased with the Card, you will be subject to the merchant s return policies. If you are entitled to a refund for any reason for goods or services obtained with your Card, you agree to accept

5 credits to your Card for such refunds. The amounts credited to your Card for refunds may not available for up to five (5) business days from the date the refund transaction occurs. j. Foreign Transactions. For security reasons, some countries may be blocked from access. If you are traveling out of the country, contact us before leaving to inquire about which countries are blocked and how to enable usage. k. Card Expiration. Your Card will expire on the date shown on the front of the Card. The Card is our property and we may revoke the Card at any time without cause or notice. You must surrender a revoked Card and any additional Card, and you may not use an expired or revoked Card. Upon revocation any amount remaining on your Card, after deducting all applicable fees, will be returned to you. You will notify us promptly within a reasonable time if the Card is lost or stolen. A replacement Card will be issued for the balance remaining on the Card at the time it is reported lost or stolen (subject to unauthorized use of the Card as set out below). Any remaining balances on a Card after expiration will be deposited into your Credit Union account or if no account is found the funds will be mailed via cashier s check to the last known address on file. If the mailed funds are returned to us, we will follow our standard procedures regarding unclaimed property. 5. Security of Card & PIN You agree to take all necessary steps to protect your PIN and never disclose your PIN to anyone. For security purposes, never write your PIN on the Card and never carry a record of your PIN in your purse or wallet. If you permit someone else to use your Card and you give that person your PIN, we will treat this as if you have authorized this person to use your Card and you will be responsible for any transactions initiated by such person with your Card. Any person to whom you may give an additional Card should be advised to treat the PIN for that Card in the same manner. If you have forgotten your PIN, call the Card Center contact number on the first page of this agreement, especially before attempting to use it at a merchant or an ATM. Please refer to the section above entitled Card Activation for more information about use of an incorrect PIN, a locked Card, and replacement of the Card. If your card or PIN is lost, stolen or otherwise compromised, immediately call the Card Center contact number on the first page of this agreement. 6. Card Fees and Charges See the Fees and Limits for Gesa Credit Union Visa Reloadable Prepaid Debit Card sheet for a listing of fees, charges and usage limitations for this card. A fee may be imposed by an ATM operator when you initiate a withdrawal or balance inquiry and by any ATM network used to complete the transaction. 7. Our Liability Unless otherwise provided by law, we assume no liability for completion or failure to complete any transactions with the Card. We are not responsible or liable to you (i) for any interruption in your use of the Card, (ii) for the quality, safety, legality, or other aspect of any goods or services

6 purchased from any merchant with your Card, (iii) if any merchant refuses to honor your Card or special offers, and/or (iv) for any other problems you agree to settle the dispute directly with the merchant. If a merchant fails to honor the Card, please call us to report the incident. 8. Errors or Questions If you think there is an error involving a transaction made with your Card such as a purchase you didn't make or an incorrect amount charged to your Card, please call the Card Center contact number on the first page of this agreement. You may also write us at Gesa Credit Union, P.O. Box 500, Richland, Wa If you tell us orally, we may require that you send your claim or question in writing within ten business days. We must hear from you no later than sixty days after the transaction is deducted from your Card. Tell us your name and Card number. Describe the error or the transaction in question, and clearly explain why you believe it is an error or why you need more information. Tell us the dollar amount of the suspected error. We will send you a written explanation after we finish our investigation. 9. Liability for Unauthorized Use of Your Card a. Your Liability. You are fully responsible for the transactions and use of the Card and Card number by you or any person you authorize or allow to use the Card. It is important to continually monitor your monthly statement to identify any unauthorized transactions. For all Card transactions not covered by VISA Zero Liability protection, you may lose the entire value stored on the Card. If you notice fraudulent activity on your card, promptly contact the Card Center contact number on the first page of this agreement. b. Zero Liability. VISA Zero Liability protection may protect you from liability for certain unauthorized transactions. However, this protection does not apply to ATM transactions. Should someone steal your card number VISA Zero Liability will apply provided you were not grossly negligent or fraudulent in handling your Card. VISA Zero Liability protection applies only to transactions processed through VISA. Provisions in this Agreement applicable only to VISA transactions (such as VISA s Zero Liability protections) will not apply to non-visa debit transactions. Please refer to for more information. The VISA Zero Liability protection will not apply if you have been grossly negligent or have been engaged in fraud. If you are grossly negligent or have engaged in fraudulent conduct, you could lose all the money stored on the Card. 10. Card Suspension & Cancellation You may cancel your Card at any time by calling at the Card Center contact number on the first page of this agreement. Any remaining balances on a Card will be deposited into your Credit Union account or if no account is found the funds will be mailed via cashier s check to the last known address on file. We reserve the right to investigate all requests for cancellation and refund and to enable all Card transactions made prior to Card cancellation to post before processing a refund. We reserve the right to suspend or cancel your Card if (i) you neither load funds to nor use your Card for a period of one year or more, (ii) if you violate this Agreement, (iii) if you fail to pay any Fee within forty-five (45) days after the date it is due, (iv) if there are potential fraud, legal or security risks associated with your Card, as determined by us, (v) for any reason allowed by law. We may, in our sole discretion, decide not to exercise our right to suspend or cancel your Card upon the occurrence or an event described in the preceding sentence; but forbearing from exercising our right to suspend or cancel your Card (either once

7 or repeatedly) will not constitute a waiver of or preclude the exercise of our right to do so in the future upon the occurrence of an event described in the preceding sentence. If we cancel your Card after the Valid Thru date has passed, any Available Balance remaining on the Card upon such cancellation, after payment for any applicable Fees and/or other amounts payable to us, will be deposited into your Credit Union account or if no account is found the funds will be mailed via cashier s check to the last known address on file. If we cancel your Card prior to the passing of the Valid Thru date on the Card, we may condition reimbursement upon return of the Card to us. A cancellation of your Card will not affect any of our rights or your obligations arising under this Agreement prior to termination. 11. Amendment We may from time to time adopt new or amended provisions, which will be effective after notice has been mailed to you at the last address or address shown for the Card on our records or has been posted on the Card Center Web site shown on the front of this Agreement or as otherwise permitted by law. Provisions changed pursuant to regulatory authorities shall be effective per such regulation without further notice. You agree to keep your personal information including address updated through the Card Center that is shown on the first page of this Agreement. 12. Privacy Please refer to the following link for our policies on information collection and sharing: ATM Safety If the ATM you are using is inside of a building, close the entry door completely upon entering. Individuals who have a legitimate need to gain access to an ATM should have an access device. Put withdrawn cash in a secure place before stepping away from the ATM. Be aware of your surroundings. If you believe you are being watched or followed, cancel your transaction, then go to a well-populated area. If necessary, call the local police. Do not write your PIN on or near the Card. Do not give your PIN to anyone else. In case of an emergency, call 911.
