2 Purpose To provide an advanced level of support to enhance the health and well being of Aboriginal and Torres Strait Islander people within the Toowoomba Hospital and Service with, or at risk of developing cardiac disease ing a Primary health multi-disciplinary, case management model to meet individual and community needs. Your key responsibilities Fulfil the responsibilities of this role in accordance with Queensland s core values, as outlined above. Provide assistance with the admission and discharge planning of Aboriginal and Torres Strait Islander clients with or spected of having cardiac disease. Provide effective support and advocacy for identified chronic cardiac or respiratory disease by conducting timely home visits, case management/ conferencing, support groups and follow-up and referrals to relevant health services/ programs. Develop partnerships with other government and non-government agencies to establish support programs that address the particular health needs of Aboriginal and Torres Strait Islander clients within the Darling Downs community. Coordinate and conduct an ongoing and regular screening program for chronic cardiac or respiratory disease risk factors with the Aboriginal & Torres Strait Islander Community. Coordinate and conduct ongoing chronic disease health promotion and education sessions with members/ families within the local Aboriginal and Torres Strait Islander community with or at risk of developing cardiac or respiratory disease. Participates in the review and development of cardiac educational resources for Aboriginal and Torres Strait Islander people in the Darling Downs community. Empowering consumers to make informed health care decisions through consultation and education. Deliver evidence-based care by working collaboratively and in partnership with consumers, carers, external agencies and other health professionals. Participates and contributes to reviews of policies and procedures to ensure cultural issues are considered. Maintain accurate and timely documentation through the provision of monthly activity reports, client file documentation and monitoring identification records. Qualifications/Professional registration/other requirements Appointment to this position requires proof of qualification and registration or membership with the appropriate registration authority or association. Certified copies of the required information mt be provided to the appropriate supervisor/ manager, prior to the commencement of clinical duties. Under s25 of the Anti-Discrimination Act 1991 (Qld), it is a genuine occupational requirement for the incumbent to be an Aboriginal and/or Torres Strait Islander person. Possession of a Diploma in Aboriginal and/or Torres Strait Islander Primary Care. Potential applicants are advised that the Commission for Children and Young People and Child Guardian Act 2000 requires Queensland to seek a working with children check from the Commission for Children and Young People and Child Guardian prior to appointment to this position. Hepatitis B immunisation is a condition of employment for Care s in Queensland who have direct patient contact (eg medical officers, nurses and allied health staff), as well as those staff who, in the course of their work, may be exposed to blood or body fluids, for example by exposure to contaminated sharps eg (but not confined to) plumbers. Proof of vaccination mt be provided upon application. Proof of vaccination can be provided via a letter from a general practitioner, infection control or occupational health department. The possession of a C class licence is required. Proof of this endorsement mt be provided before commencement of duty.

Planning, problem solving and coordination skills in the management of consumer care. High level interpersonal and communication skills. Ability to identify and manage risk. Ability to work without direct supervision. Demonstrated ability to show respect, honesty and integrity for others, remaining flexible, fair and willing to compromise when dealing with team members, patients, carers and staff. Demonstrated the ability to e information technology to support the provision of patient care. Demonstrated effective written communication skills. Ability to participate effectively in workplace meetings and other related forums. About the Service District/Division/Branch/Unit The Darling Downs Hospital and Service comprises of seven local government areas of Taroom Shire Council, Western Downs Regional Council, including Wandoan, Miles, Chinchilla, Jandowae, Glen Morgan, Meandarra, Tara and Moonie, South Burnett Regional Council, covering Murgon, Cherbourg, Wondai, Kingaroy and Nanango, Dalby Regional Council, Toowoomba Regional Council including Oakey, Southern Downs Regional Council comprising of Warwick and Stanthorpe and Goondiwindi Regional Council including Inglewood and Texas. The Hospital and Service, services a population of approximately 280,000 people. The Hospital and Service demographics are diverse and include metropolitan and small rural community settings. The Hospital and Service has a major teaching role, providing both undergraduate and postgraduate clinical experience for members of the multidisciplinary healthcare team.
