Adair County NEW LIFE CHURCH OF STILWELL Circle Inn Motel, Stilwell Serves: Stillwell F 6 PM - 8 PM (8/3/12) Provides free backpacks (no school supplies) to needy families. Walk in during one day event on Friday, 8/3/12. Must be a family in need. Bryan County VICTORY LIFE FOOD BANK OF DURANT 724 W Main, Durant Serves: Atoka County, Bryan County, Choctaw County, Johnston County, Marshall County First M of the month Provides free haircuts from professional hair stylists to anyone in need. Appointment required. Hair must be washed and clean prior to the appointment time. FAMILIES FEEDING FAMILIES - BACK 2 SCHOOL PACKS 114 S 7th, Durant Serves: Bryan County M 9 AM - 12 noon (8/6/12) Provides a backpack filled with school supplies through the "Back 2 School Packs" program. Distribution will occur Monday, 8/6/12 from 9 AM - 12 noon. Walk in; no appointment required. SALVATION ARMY - DURANT 1303 W Waco, Durant Serves: Bryan County Offers school supplies. Those wishing to get school supplies through the program, must contact the school their children attend and tell the secretary or the school social worker that they want to get school supplies through the Salvation Army program. Need to mention "Salvation Army program for school supplies". Children in need must be present when picking up supplies. Cherokee County CHEROKEE NATION EDUCATION SERVICES S Muskogee Avenue, Tahlequah Serves: Adair County, Cherokee County, Craig County, Delaware County, Mayes County, McIntosh County, Muskogee County, Nowata County, Ottawa County, Rogers County, Sequoyah County, Tulsa County, Wagoner County, Washington County Provides Native American students with school supplies. Contact school district to be referred to the JOM Representative. The JOM Representative will refer eligible students to the JOM school supply distribution program. Not all schools within the Cherokee Nation jurisdictional boundaries are a part of this program. Must reside in service area, attend a school that is affiliated with the JOM Program and be a member of a federally recognized tribe. ZOE INSTITUTE - BACK TO SCHOOL BASH 1009 S Muskogee Ave, Tahlequah Serves: Cherokee County M, T, W 9 AM - 12 noon, 1 PM - 3 PM; Th 9 AM - 12 noon, 2:30 PM - 3 PM Provides a one-day Back to School Bash in conjunction with the Cherokee County Christian Ministerial Alliance on Saturday 8/4/12 at Tahlequah Community Building, 908 S College Ave. Free backpacks, school supplies and haircuts will be available. Pre-registration required at Zoe Institute offices. Ticket will be given for entrance to Back to School Bash either
2 for a 9 a.m. time slot or for a 10:30 a.m. time slot. No walk-ins allowed on day of event. Must attend school in Cherokee County. Student must be in K-8 grade. Craig County FIRST NATIONAL BANK VINITA FIRST CARES 102 W Illinois, Vinita Serves: Craig County M, T, W, Th, F 9 AM - 4 PM Provides assistance with school supplies. Contact school counselor for referral. Must bring proof of residency and documentation of school attending. SALVATION ARMY - VINITA 224 W Sequoyah, Vinita Serves: Vinita Provides school supplies once a year. Professional referral required, contact school counselor. Must be enrolled in Vinita schools. Creek County CARING COMMUNITY FRIENDS OF SAPULPA 12 W Burnham, Sapulpa Serves: Sapulpa M, W, Th 10 AM - 2 PM; F 10 AM - 12:30 PM Offers clothing for needy children ages 4-15 years enrolled in Sapulpa Schools. Application form required. This service begins 7/16/2012 and runs throughout the school year. Must provide photo ID with current address, Social Security cards for household, proof of income or no income. Must be enrolled in Sapulpa Schools and qualified for free or reduced lunch. Must be under 16 years old. BRISTOW SOCIAL SERVICES 206 S Main, Bristow Serves: Bristow, Depew, Gypsy, Milfay M, T, W, Th, F 9 AM - 3:30 PM (8/2/12-8/17/12) Provides school supplies for those in need, grades K-10. By appointment only. Call agency to set up appointment which are being scheduled between 8/2/12-8/17/12. Must provide Social Security card(s) proof of income, proof of residence and utility bill. Must meet income guidelines. CARING COMMUNITY FRIENDS OF SAPULPA 12 W Burnham, Sapulpa Serves: Sapulpa M, W, Th 10 AM - 2 PM; F 10 AM - 12:20 PM Offers school supplies. Registration begins 7/16/2012. Application form required, appointment required. Must provide photo ID and proof of income/no income for parent; Social Security cards for parents and children. Must be an enrolled student in the Sapulpa schools. Delaware County DELAWARE COUNTY COMMUNITY PARTNERSHIP 1310 S Main Street, Grove Serves: Delaware County W 10 AM - 2 PM (8/8/12) Offers a one day Back to School Extravaganza event on 8/8/12 to be held at the Grove Civic Center, 1720 S Main, Grove. Provides free school supplies, haircuts, and dental exams along with community informational booths helpful to parents and students. Walk in; no appointment required. Children from Pre-K through 5th grade. Garvin County FIRST BAPTIST CHURCH OF ELMORE CITY 107 S Texas, Elmore City Serves: Garvin County Sa 8 AM - 8 PM (8/4/12) Provides school supplies, clothes and household items for school children going back to school. Walk in; no appointment required. HIGHER GROUND CHURCH - BACKPACK & BURRITO DAY 5353 Old Antioch Road & Hwy 19 W, Pauls Valley Serves: Garvin County 9 AM - 12 noon, 8/4/12 Provides backpacks filled with all the basic supplies needed for students in grades K-12. Along with the school supplies, sack lunches with burritos, chips, and a drink are served to all children and adults. There will be a presentation of the Gospel for all participants. Walk in; no appointment required. Muskogee County ARK OF FAITH 401 E Broadway, Muskogee Serves: Muskogee County Sa 8/18/12 9 AM - 2:30 PM Provides school clothing to children K-12 grade. Walk in; no appointment required. Children must have parent or legal guardian present. Legal guardians must provide proof of guardianship. Okmulgee County MUSCOGEE (CREEK) NATION OF OKLAHOMA 2971 N Wood Drive, Okmulgee
3 Serves: Oklahoma M, T, W, Th, F 8 AM - 5 PM (5/2/12-10/26/12) Program provides students a grant of $200 to purchase school clothing items and/or foot apparel. Grants will be provided in the form of a check, payable to the head of household listed on the application and will be awarded once per school year. Checks will be delivered by mail through the U.S. Postal Service. All applicants should allow 1-6 weeks for processing. Documentation of clothing purchases is required. All original receipts must be returned to the School Clothing Program by mail or in person within 90 calendar days of the check issuance date. Application form required. Available on-line or contact agency. Applications will also be available at any MCN Indian Community Centers and MCN Child Development Centers. All completed applications will be accepted on a first come, first serve basis until funds are exhausted or October 26, 2012, whichever comes first. The custodial parent must complete the application. If the applicant is not the custodial parent, documentation in the form of a Legal Custody Order or Guardianship Order through a court must accompany the application. Notarized statements are NOT accepted. A copy of the student's Muscogee (Creek) Nation citizenship card must accompany the application. CDIB cards will not be accepted. If you have three or more eligible children, you are required to fill out the attached W-9. Must provide verification of school enrollment. The school enrollment form must be completed by a school official. Must provide verification of home school (if applicable). Must be a Creek Nation student. Must be 3 years old and enrolled in a certified Head-Start; at least 4 years old and enrolled in a public/private school pre-k program, no higher than the 12th grade. Home Schooled children ages 5 to 18 years old an participating in an accredited curriculum are also eligible. Ottawa County OKLAHOMA DHS - OTTAWA COUNTY 2114 Denver Harnar Dr, Miami Serves: Ottawa County T 9 Am - 12 noon (8/8/12) Offers free school supplies, immunizations, eye exams at the Miami Civic Center on 8/8/12 for students in grades K-12. Walk in; no appointment required. Must provide shot records to receive immunizations. Must have a child in grades K-12 and qualify for free or reduced lunch. MIAMI TRIBE - NATIONAL HEADQUARTERS 202 S Eight Tribes Trail, Miami Serves: Oklahoma M, T, W, Th 8 AM - 4:30 PM; F 8 AM - 12 noon Provides $35 for PreK students and $50 for K-6th, $60 7th -8th, and $80 9th-12th grade. Checks mailed within 3 weeks after deadline. Application available online. Application form required every semester. Deadline for first semester of school year is 7/27/12. Must provide proof of Miami Tribe membership. Student must be a tribal member enrolled in school grades PreK-12. Pontotoc County CHICKASAW NATION EDUCATION - CLOTHING, LAP- TOP, TEXTBOOK GRANTS 124 E 14th, Ada Serves: Bryan County, Carter County, Coal County, Garvin County, Johnston County, Love County, Marshall County, Murray County, Pontotoc County M, T, W, Th, F 8 AM - 5 PM Clothing - Provides Chickasaw students attending college full time $ clothing grant for each classification as they progress through college. Part time undergraduate students enrolled in 6-11 hours will be eligible for a $ scholarship for every 14 hours completed. Graduate/doctorate students must be enrolled in 6-8 hours to be eligible for $ scholarship for every 15 hours completed. The scholarship will be distributed to the student in the form of a pre-paid Visa card. Required forms are available at area office. Must have CDIB card. Must be Chickasaw Nation member. Must be attending college. Laptops - Offers full time students in college or professional programs with a GPA of 3.0 and at least at sophomore level with laptops. Must provide official college transcript and full time class schedule. Must be Chickasaw and a full time student, must be college sophomore or better and must have GPA of at least 3.0. Textbooks - Provides grants that may be awarded to full time or part time undergraduate, graduate or doctoral students enrolled in at least three college credit hours who maintain a 2.0 GPA or higher each semester. Part time students are awarded $ per semester and full time students may be awarded $ per semester. Must have CDIB card. Applications may be obtained at any area office. Must be a Chickasaw student with a minimum enrollment of three credit hours and maintain a GPA of 2.0 or higher. Seminole County SEMINOLE NATION SOCIAL SERVICES OF WEWOKA 2007 W Wrangler Boulevard, Seminole Serves: Seminole Nation M, T, W, Th, F 8 AM - 12 noon; 1 PM - 5 PM Provides a grant through the Judgement Fund Program to the parents/legal guardian of a student pre-enrolled or enrolled in an attending school. Pre-K through 5th grade will receive $ and 6th grade through 12th grade will receive $ Applications available for download from the website 8/1/12-10/31/12. Only original or downloaded applications will be processed. No ed, faxed, or copied 3

Sequoyah County CATHOLIC CHARITIES HELPING CENTER - PASTOR S FELLOWSHIP BACK TO SCHOOL GIVEAWAY 409 North Adams St, Sallisaw Serves: Sequoyah County Th 9 AM - 1 PM (8/2/12) Provides a one day community-wide event on Thursday, 8/2/12, called the Pastor's Fellowship Back to School Giveaway where needy families can receive free school supplies. Walk in; no appointment required. One day event on 8/2/12 located at Sallisaw Civic Center, 115 E Choctaw Ave, Sallisaw. Must bring SS card for each child. If custodial parent or legal guardian cannot attend event, they may send a representative in their place along with a signed permission letter from them indicating the representative by name who can apply for assistance. CDIB card holders are not eligible in order to avoid duplication of services. Tulsa County NEW HEIGHTS CHURCH - BACK2SCHOOL MISSION 106 N Main Street, Owasso Serves: Broken Arrow, Catoosa, Claremore, Collinsville, Glenpool, Oologah, Owasso, Sand Springs, Sapulpa, Skiatook, Sperry, Tulsa W 8 AM - 8 PM (8/1/12) Office: M, T, W, Th, F 8 AM - 4 PM Provides school supplies in backpacks appropriate for elementary level students on Wednesday, 8/1/12. Also provides haircuts, clothing closet and eyeglass screening during event. Walk in; no appointment required. ASSISTANCE LEAGUE OF TULSA - OPERATION SCHOOL BELL 3408 E 11th Street, Tulsa Serves: Tulsa Public Schools, Owasso, Sperry,Skiatook, Berryhill, Glenpool, Jenks, Kellyville, Keystone, Sand Springs, Union, Catoosa school districts. Contact school counselor. Provides school clothing. School counselors identify and verify need. They also supply the proper forms to be completed by the parent. Family/parent interview with the student's school counselor required. Must be a student in one of the defined school districts. Eligible children are those who receive free lunch. JOHN 3:16 MISSION - KLOTHES FOR KIDS BACK TO SCHOOL FESTIVAL 2027 North Cincinnati, Tulsa Serves: Tulsa County Sa 10 AM - 3 PM (8/11/12) Offers free backpacks, jeans, and uniform pants to youth in Tulsa or surrounding area on Saturday, 8/11/12, during the Klothes 4 Kids Back to School Festival. Will also have an expanded clothing shopping area with new and gently used clothing. Activities for the kids (inflatables, temp. tattoos & crafts) and food for the whole family (hot dogs, chips, snow cones, and drinks). Walk in; no appointment required. Registration paperwork to be filled out at door. Need to know the school the child is attending, grade and clothing sizes. Must bring photo ID; SS cards/birth certificates for each child. Must bring shots records to receive immunizations. Child must be school age (Pre K-12th grade). SAND SPRINGS COMMUNITY SERVICES 4th & Garfield, Sand Springs Serves: Sand Springs M, T, W 10 AM - 2 PM (8/13/12-8/15/12) Provides Wal-Mart clothing voucher and school supplies for Sand Springs students. Application required Must provide current proof of enrollment, proof of address, Social Security card for each member of household, and proof of free lunch assistance from Sand Springs schools. Must be enrolled in Sand Springs school system and receive free lunch assistance. BIXBY OUTREACH CENTER - BACK TO SCHOOL BASH 4 E Dawes Avenue, Bixby Serves: Bixby, Haskell, Jenks, Leonard, Mounds Registration: T, W, Th 9 AM -12 Noon; 1 PM - 4 PM Provides school supplies for needy families. After signing up, applicants will be asked to come back to pickup supplies 8/9/12 at the "Back to School Bash." Application required. Must bring Social Security card(s) card for all household members, proof of income and address. Must be in service area grades K -12. BROKEN ARROW NEIGHBORS - BACK TO SCHOOL BASH 322 W Broadway Avenue, Broken Arrow Serves: Broken Arrow Sa 9 AM - 12 noon (8/4/12); Office: M, T, W, Th 9 AM - 4 PM Provides a free health and safety Back to School Bash 4
5 in conjunction with the Broken Arrow Police Department on Saturday, 8/4/12. The event will include free school supplies; haircuts; vision, screening, blood sugar tests; Child find ID program. Remaining school supplies left over from the Bash event will be distributed to needy students in the Broken Arrow school districts through a walk-in visit to the Broken Arrow Neighbors offices. Walk-in. Back to School Bash event will take place at Tulsa Technology Center, 4600 S Olive Ave, Broken Arrow. Must provide proof of address (BA residents only), proof of income, valid ID and/or birth certificate. Must be a Broken Arrow student to receive school supplies directly from Broken Arrow Neighbors after the Bash event.
