7 (OPTIONAL FORM) Telephone and Voic Request Form Boarding students have access to local phone service in their rooms if they chose to use it. Although many students prefer to use their own cell phones, the service on campus is not always reliable. Therefore, local calling (including on campus calls) is provided to all boarding students in their dorm rooms as part of the communication fee. If you are interested in activating a dorm telephone line please complete the form below and return to the Director of Technology via at jalford@mxschool.edu. The Technology Office will ensure that your phone line is active upon your arrival. Day and Boarding students also have access to voic service. Messages can be accessed from any phone on campus, including phones in public spaces, and from home. If you are interested in having the Technology Office create a voic account for you, please check the appropriate box below. Long Distance service is provided by Campus Communications, a private billing service that works with many schools. If you choose to sign up for long distance calling services, you will be contracting directly with Campus Communications for connection services and you will receive a monthly billing statement from them. For questions regarding signing-up for long distance calling services, please Martha Wright at mwright@mxschool.edu. Day students may make local calls from any school phone and from wall phones in the Student Center, the dorm lobbies, the gym, Eliot Hall, the Library and the TAC. Student phones will be turned off during evening study hours. Please refer to the Student Handbook for specific times. 8 Guide to Establishing an Account on My SchoolBucks 1. Go to 2. Select Register for a Free Account and complete all required information a. For the State/Province option, please select Massachusetts b. For the School District option, please select Middlesex School 3. Once the account is established, you will be brought to the Getting Started page a. To add students to your account: i. Select My Household in the upper left ii. Select Add a student 1. When adding a student, please use their formal first and last name (as compared to their preferred nickname) 2. The student ID is the same alpha ID that is noted on the monthly student bill. iii. When a match is found, select Add Student 4. Making Payments a. Log -in and select Make Payment b. Select the amount you want to deposit c. Checkout and please make sure to hit the final place order on the confirmation screen d. Note: there will be a $2.50 convenience fee for processing the payment 5. Current Balance Review a. When you log into the account, the home page (My Household) will indicate the current account balance 6. Activity Report a. Log-in and select Cafeteria Purchases on the left hand column i. Please note: this account is used for both grill purchases and cash withdrawals from the student bank. The Cafeteria Purchases is a bit misleading, but it will show both student bank and grill activity. 8

11 MYSCHOOLBUCKS LOGIN SCREEN Congratulations, your school district now offers myschoolbucks, a convenient and secure online payment and parent information portal! With myschoolbucks you can deposit money into one or more student accounts, track purchase history, create low-balance reminders and even set-up an automatically recurring payment. As you will soon discover, myschoolbucks is designed to be intuitive and easy to use, but to help you get started, this document will guide you through the process of: creating a new parent account adding one or more students to your account making a payment. Step 1 - Creating a New Parent Account Access myschoolbucks at This screen will be used by both new and returning users. A returning user will complete the ACCESS YOUR ACCOUNT and click the LOG IN button. All new parent users will click the REGISTER FOR A FREE ACCOUNT button. P a g e 3 - C O N F I D E N T I A L
12 Step 2 Select State Select the state where the school district is located and click the CONTINUE button. Step 3 Select School District Select the School District from the drop down box, and then click the CONTINUE button. P a g e 4 - C O N F I D E N T I A L

14 Step 5 Create Parent User Credentials Complete the Login ID and Password fields then click the Security Question drop down. Select a question and enter the answer in the Security Answer field. Before moving forward, a NOTICE disclaimer alerts new users of the convenience fee that may be imposed for payments/deposits to student accounts. The user will then click the box and REGISTER. Click the FINISH button to complete the myschoolbucks user account setup. P a g e 6 - C O N F I D E N T I A L
15 ADDING STUDENTS TO YOUR ACCOUNT ( MY HOUSEHOLD ) Once you have created a parent account, click My Household on the left-side navigation bar to add students to your account. Step 1 Student Lookup To add one or more students to your Household, click look up your students. Step 2 Enter Student Information Select your school district from the drop-down menu. Enter the student s first name, last name and Student ID Number. Then click Find Student. P a g e 7 - C O N F I D E N T I A L
16 Step 3 Add Student Student Name K A student s name and grade will be displayed when the search is complete. If this is the correct student, click Add Student. If the information is not correct, click Cancel and try your search again. Step 4 Finish or Add Another Student You have now successfully added a student to your Household. To associate more students with your account, click Add Another Student and complete Steps 1-3 until all students have been added. If there are no other students to add, click Finish. 17 DEPOSITING FUNDS INTO A STUDENT ACCOUNT Once you have added students to your Household, their names and schools will appear by clicking the My Household link. This page will also display the current balance available for each student. From this page you can deposit money into a Student Account, view purchase history and add or remove students. Step 1 Making a Deposit Student Name School To deposit money into one or more student accounts, click Make a Payment. Step 2 Enter Deposit Amount Student Name Select the amount you want to deposit into each student account, and then click Add to Basket. P a g e 9 - C O N F I D E N T I A L
18 Step 3 Review Deposit Amount(s) Review the amount(s) you have entered and verify the information is correct. If you need to adjust any amount, click Continue Shopping. If the information is correct and you are finished, click Check Out Now. Additionally, this is where you would also establish a recurring payment schedule by clicking Setup a payment schedule. Step 4 Payment Information Enter your payment information, making sure to complete all required fields, then click Continue. P a g e 10 - C O N F I D E N T I A L
19 Step 5 Card Verification Code If paying with a credit or debit card, enter the three- or four-digit Verification Code that appears on the card, and then click Continue. Step 6 Review Order This screen will show the amount of the purchase, a subtotal of the payment and the total payment amount including any convenience fee (amount varies). 20 Step 7 Payment Confirmation & Receipt When your order is complete, you will receive a confirmation number that can be used to locate this transaction at a later date. We recommend you print this page and keep a copy for your records. After clicking Print Oder, a printable receipt (sample below) will open in a new window.
