3 Who am I? Hello my name is I am a clinical nurse specialist and I work alongside your consultant and other health professionals in order to co-ordinate treatment and care for you and your family. There are two clinical nurse specialists at the Manchester Royal Infirmary involved in the care of patients with Head and Neck Cancer: Barbara Farrimond Philip Bryce Office: Office: Mobile: Mobile: What is a Head and Neck Oncology Clinical Nurse Specialist (CNS)? A head and neck oncology clinical nurse specialist is a nurse who specialises in cancers of the head and neck. Our role is to provide support to you, your family and/or carers and aim to see patients from diagnosis to completion of treatment. Contact times Monday to Friday 8.00 am to 4.00 pm It is the role of the clinical nurse specialist/key worker to support you and your family/carers throughout your cancer journey. This will be from diagnosis until you feel you no longer need us. Your Consultant/Lead Clinician is: 3

4 Diagnosis Head and neck cancers include cancer of the mouth, lip, tongue (oral cavity), throat (pharynx), voice box (larynx), salivary glands, nose, sinuses, middle ear and thyroid. Being told you have cancer can be a shocking and frightening time, and your clinical nurse specialist/key worker is there for your support. In some cases we are able to visit you at home during this time. Multi-disciplinary Team (MDT) When you have a diagnosis of cancer, your case will be discussed at the MDT meeting held at The Christie Hospital on Thursday mornings. There are a lot of people involved in your care, this group of people are known as the multi-disciplinary team and include: Consultants of surgery, oncology, pathology and radiology. There are also doctors, nurses, dietitians, speech and language therapists, physiotherapists and occupational therapists. Meeting so many new people involved in your care can be stressful and confusing but we act as a link between them and can also explain their roles. Your Clinical Nurse Specialist at the Manchester Royal Infirmary will be your key worker whilst you are having surgery at the MRI, or if you are a Central Manchester resident. Please note that you may have contact with other Clinical Nurse Specialists during your cancer journey, for example at your local hospital where you received your diagnosis. You can access ongoing support from your local CNS once discharged from the Manchester Royal Infirmary. You may also meet a CNS at the Christie Hospital if you are to receive any treatment there. 4

5 Your in-patient stay During your stay in hospital we will try to visit you on the ward regularly for ongoing reviews. We will also discuss your treatment and alleviate any worries and fears you may have. When it is time for you to go home we will work closely with the ward staff, occupational therapist, dietitian, physiotherapist, speech and language therapist, district nurses, GPs and social services in aid of planning a safe and seamless discharge home. Pre-assessment appointment An appointment will be made for you to attend The Christie Hospital on a Thursday morning. At this appointment your consultant surgeon, an oncologist and your specialist nurse will discuss with you the treatment options that are available to you. Depending on your type and stage of cancer, any of the following treatments may be required. It may be that you need a combination of these treatments. Surgery Radiotherapy Chemotherapy If you require surgery then this will be performed at Manchester Royal Infirmary. If you require radiotherapy and or chemotherapy, then this will be given at The Christie Hospital. 5

6 Pre-operative assessment clinic This usually takes place one to two weeks prior to your operation. The assessment can be done as an in-patient or an out-patient depending on your intended surgery. Your clinical nurse specialist will inform you which type of assessment and where this will take place. Your clinical nurse specialist and a doctor from your consultant s team will be present at this appointment. Should you be asked to attend this clinic then the aim is to: Give you a chance to ask any questions about your surgery. Give the doctors a chance to assess your general state of health and any problems you may be experiencing, such as weight loss and pain. Ensure you have the necessary tests required in preparation for your surgery such as: Routine blood tests ECG (electrocardiogram) heart trace X-ray of chest and/or jaw Lung function tests Dopplers scan of your veins/arteries Echocardiogram scan of your heart Depending on the nature of your surgery you may meet the dietitian and/or speech and language therapist at this clinic. They will assess you and help with any difficulties you may have with eating, drinking and speech. 6

7 Out-patient appointment You will be seen by your consultant/lead clinician s team on a regular basis for the next five years. This can be at either The Christie Hospital or here at the MRI, but can occasionally be arranged at your local hospital if your consultant has a clinic there. During these visits you will be able to discuss any problems or worries that may be concerning to you. You will also see your Clinical Nurse Specialist (CNS) at these appointments but you will always have direct contact with your CNS via the phone should you need further input regarding your care. How can you help? If you are a smoker and would like some help giving up or cutting down, then your clinical nurse specialist can help you access smoking cessation services. Eat a well balanced diet. If this is not possible, follow the advice given by the dietitian. If you have not yet seen your dietitian then discuss any problems with your clinical nurse specialist. Reduce your amount of alcohol intake to three units a day. If you need support and advice to reduce your alcohol intake you can discuss this with your clinical nurse specialist who will be able to offer help and advice. Feel free to bring a relative or friend with you to your clinic visits. Write down any questions that you would like to ask, so that we can answer them for you either over the phone or the next time you visit the clinic. There is space provided in this leaflet for you to write down your questions so it would be beneficial to bring this booklet with you at each appointment. Please remember that these clinics are busy and you may have to wait a while. 7

We can help you with, or refer you to people who can help you with: Pain Nausea Weight loss and appetite Swallowing and speech Complementary therapy Counselling Financial support Fear and anxiety Smoking Alcohol Travel insurance Difficulty with reading and writing Any questions, worries or concerns you may have 8

10 The Cancer Information Centre Within Manchester Royal Infirmary there is a Macmillan Cancer Information Support Centre. The centre provides a confidential service to anyone wanting information about any aspect of cancer. There is a cancer support nurse based at the centre accompanied by a specialist nurse and trained volunteers to help with any queries that you may have.
