1 TM II HOME CARE SERVICES / CUSTOM COMPANION CARE ASSISTANCE
2 2 Your Home Care Service Custom Companion Assistance Solutions An Evolution In Senior Home Care According to Health and Human Services, 70% of Americans will need some type of home care in their lifetime. That is nearly three out of four of us. Many American seniors are under the impression that traditional health care programs will cover their care. Medicare, Medigap and most Medicare Advantage Plans are designed to cover hospital stays, doctor bills and some short term skilled nursing. Unfortunately, they may not cover the cost of everyday assistance in your own home. While your loved ones will have the best intentions to be there for you when a crisis arises, many are not prepared and quickly become overwhelmed by the amount of work involved in care giving. Purchasing traditional home health care insurance is limited to only the extremely healthy, creating challenges for seniors in need of another option. True Freedom Plans are a viable alternative to traditional home health care insurance. Our plans support seniors with their activities of daily living and companionship providing them independence in the comfort of their own home. All True Freedom Home Care Plans Feature: Guaranteed Acceptance No Health Questions No Claim Forms No Deductible No Co-pays No Age Limits Our plans can also provide urgent care benefits and overnight benefits in the comfort of your home. Services are provided by a network of home care providers while also giving you the option to choose a friend or neighbor to administer care. True Freedom II Plans are Service Contracts, NOT Insurance.

8 8 Letters Of Recommendation Dear Mr. DooIan, I am my elderly parents' power of attorney and have to deal with every aspect of their health among other things. The last few years have been particularly difficult. Not only because of their declining health, but because of insurance coverage and payment issues. We have had different companies insure home health assistance. We have paid a lot of money for this service for these companies declining or stopping payment for the aides who come and assist them for which we then have to pay out of pocket on top of the premiums. We have been with ASSI a short time and certainly this company is different from the others we have dealt with in the past. Personal service, timely payment to the health care workers and immediate response to questions has been my experience with ASSI. I am impressed and will recommend to others. With appreciation, Carmen Queral, PhD, MPH, PA-C Director of Clinical Education I want to thank ASSI for taking care of my wife without hesitation. You were there for us in our time of need. ASSI is without a doubt a first class organization that takes care of the seniors without a run around. Once again, a heart felt thank you! Regards, Donald Menk ASSI, Thank you so much for being there at my time of need. Your company is a pleasure to work with. Mr. Doolan made it so easy and thank him for his professionalism and being so kind. ASSI should be a model for all other companies. I will recommend you to everyone. God Bless! Sincerely, Cynthia May Dear ASSI : We highly recommend ASSI for any future health needs. They are efficient, caring, reliable and very compassionate. It has been a real pleasure to do business with this organization. Sincerely, Renne & Gerald Blumenthal, MD I just wanted to let you know how impressed and pleased I am with your service. I have had a lovely aid coming into my home for the past few weeks and this person has been a lifesaver for me. The service and support I have received from ASSI to get the care started has also been second to none! It really gives me a piece of mind knowing that they will take care of the aids and the Home Care agency for my services. I was worried I would have to find my own agency in the yellow-pages. Thankfully this was not the case. I highly recommend ASSI for their Home Care Service for anyone who might be living alone or even for married couples who just want to protect their assets. Thank you very much for your care and service. Sincerely, Rhea Brenner Original Letters On File At American Senior Services Inc. Home Office
