1 Request for Applications Eastern Oregon Healthcare Transformation Project Grants Program Overview The Eastern Oregon Coordinated Care Organization (EOCCO) announces the availability of grants, through funding from the State of Oregon Transformation Center, for projects that support better health, better health care and lower costs for Eastern Oregon s Oregon Health Plan (OHP) members and their communities. Proposed projects should be innovative, scalable, transferable and related to overall transformation goals. Specific areas of emphasis include, but are not limited to, the following: Information technology systems and infrastructure, including additional investment in electronic medical records (EMR) and claims processing systems. Population health management, case management, disease management and achieving quality metrics. Provider panel and clinic enhancements to provide extended primary care services to high risk Oregon Health Plan members. Projects designed to improve patient engagement in and accountability for a patient s own health, disease prevention and wellness activities. Pilot projects testing the use of flexible or otherwise innovative services. EOCCO encourages collaborative applications between caregivers and community partners and anticipates awarding funds for at least one project in each county. Example projects include, but are not limited to, the following: Projects that encourage members to use health services appropriately and in appropriate settings. Integrating mental health providers into the primary care setting. Supporting efforts to obtain Patient Centered Primary Care Homes (PCPCH) designation or progress toward becoming a Tier 3 PCPCH medical home. Alignment between multiple care providers (e.g. physical, behavioral, public health, school, etc.) for care coordination and disease management activities. Investments in telemedicine. 2 Eligible Organizations Community organizations and providers throughout the healthcare community including, but not limited to: hospitals, primary care clinics, rural health clinics, federally qualified health centers, mental health and public health, are eligible for these grants. Applicant organizations must provide services to Oregon Health Plan members within the twelve-county EOCCO service area: Baker, Gilliam, Grant, Harney, Lake, Malheur, Morrow, Sherman, Umatilla, Union, Wallowa and Wheeler. Allowable and Unallowable Costs Funded activities may include, but are not limited to: personnel, travel expenses, meetings and supplies, consultants, and indirect expenses associated with the project such as administrative support, telephone, and computers. Non-project related indirect expenses, funds for capital expenditures (e.g. major non-technology equipment, building renovations) and costs related to enhancing reimbursements or supporting state-covered services cannot be funded through these grants. Anticipated Available Funding & Project Period Funding for this program is provided by the State of Oregon Transformation Center. EOCCO anticipates awarding $1.6 million in individual grant amounts ranging from $30,000 to $100,000. Grantee Requirements Grantees will be required to share intermediate and final process and outcomes data on their projects, participate in periodic telephone meetings with EOCCO representatives and, at times, other grantees. Grantees will produce periodic reports for the EOCCO to share with the state and a final report that will be publically available. Review Criteria All applications will undergo a comprehensive evaluation by an impartial review panel. Applications will be assessed with regard to the following review criteria: Significance. Does the project address an important problem or barrier to better health, better health care and lower costs for Eastern Oregon s OHP members and their communities? If the goals of the project are achieved, how will the outcomes affect these problems or barriers and support overall transformation needs and goals? How will successful completion change the technologies, treatments, services, or preventive interventions for OHP members? Innovation. Does the project propose novel concepts or approaches to address the needs of OHP members and their communities or address intractable problems in promising new ways? Approach. Is the project design well-reasoned and likely to achieve the stated goals? Are the methods and timeline thoroughly described and reasonable given limitations of the environment, time available and funding? Is the budget clear, reasonable and appropriate to the work proposed? Does the project include the stakeholders (e.g. organizations and community partners) likely to contribute to its success? Potential Impact. Does the project hold promise for making a significant improvement in the health, health care or costs of care for Eastern Oregon s OHP members and their communities? Is it scalable and transferable to other similar environments? Is the project sustainable and likely to continue beyond the grant period? 2

3 How to Apply Notice of Intent to Apply To assist us with planning for proposal review, please submit a non-binding notice of intent to apply for a grant by March 7, 2014 by 5 p.m. PST. The should be sent to lyzells@ohsu.edu and should contain the following information: Name of organization applying for funding Up to a paragraph describing the project the organization plans to propose Name of project director/principal investigator for the project Mailing address, address, phone Full Application Submit the following materials by 5 p.m. PST on April 4, 2014 in a single PDF document via to lyzells@ohsu.edu. You will receive a confirmation that your grant has been received. I. Cover Sheet Complete all sections of the Application Cover Sheet (Attachment 1) including signature from the authorized representative. This page should be included as the top page of the Application. II. Narrative Application The narrative application should address the following items in the order listed below. Use the titles provided for each section. Application narratives comprising sections A through D, described below, must be no more than 6 pages in length. Use an 11 pt. or greater font size for narrative text and no smaller than half-inch margins. Diagrams, tables and budgets may be in smaller font sizes. a. Executive Summary (1/2 page) Provide a brief summary of the project(s) including project goals. b. Project Description (4 pages) Give a detailed description of the proposed project including: overall goals, target populations, the intervention, detailed activities and any community partners that will be working on the project and the tasks they will undertake. Describe the individuals tasked with portions of the project and their roles and experience. Explain the expected outcomes and how they help meet the project goals. Describe potential risks and how you plan to address them. c. Measurable Project Objectives and Metrics (1 page) Describe the goals you expect to have reached by the end of the project. Present the key evaluation metrics you will use to measure progress toward reaching these goals and whether the goals have been achieved. Include a timeline with key interim steps, responsible individuals and deliverables for the project. d. Spread of Innovation (1/2 page) Explain how the project is innovative, scalable and transferable. Describe how the project, if successful, will be sustained within your organization and how it could be spread to other organizations. III. IV. Budget and Budget Narrative Provide a budget in the format described in Attachment 2. Also include a brief budget narrative. Other Attachments You may include Attachment 3 to designate confidential materials. Attachments other than those included in this notice and letters of support are discouraged. 3

7 ATTACHMENT 3 - Applicant s Designation of Confidential Materials Applicant Name: Instructions for completing this form: As a public entity, the Oregon Office of Contracts & Procurement (OC&P) is subject to the Oregon Public Records Law which confers a right for any person to inspect any public records of a public body in Oregon, subject to certain exemptions and limitations. See ORS through Exemptions are generally narrowly construed in favor of disclosure in furtherance of a policy of open government. Your Application will be a public record that is subject to disclosure except for material that qualifies as a public records exemption. It is OC&P s responsibility to redact from disclosure only material exempt from the Oregon Public Records Law. It is the Applicant s responsibility to only mark material that legitimately qualifies under an exemption from disclosure. To designate a portion of an Application as exempt from disclosure under the Oregon Public Records Law, the Applicant should take the following steps: I. Clearly identify in the body of the Application only the limited material that is a trade secret or would otherwise be exempt under public records law. In order for records to be exempt from disclosure as a trade secret, the records must meet all four of the following requirements: The information must not be patented; It must be known only to certain individuals within an organization and used in a business the organization conducts; It must be information that has actual or potential commercial value; and, It must give its users an opportunity to obtain a business advantage over competitors who do not know or use it. Keep in mind that the trade secret exemption is very limited. Not all material that you might prefer be kept from review by a competitor qualifies as your trade secret material. OC&P is required to release information in the Application unless it meets the requirements of a trade secret or other exemption from disclosure and it is the Applicant s responsibility to provide the basis for which exemption should apply. In support of the principle of an open competitive process, bottom-line pricing that is, pricing used for objective cost evaluation for award of the RFGA or the total cost of the Grant or deliverables under the Grant will not be considered as exempt material under a public records request. Examples of material that would also not likely be considered a trade secret 7

8 would include résumés, audited financial statements of publicly traded companies, material that is publicly knowable such as a screen shot of a software interface or a software report format. To designate material as confidential and qualified under an exemption from disclosure under Oregon Public Records Law, an Applicant must complete this Attachment form as follows: Part I: List all portions of your Application, if any, that Applicant is designating as exempt from disclosure under Oregon Public Records Law. For each item in the list, state the exemption in Oregon Public Records Law that you are asserting (e.g., trade secret).
