" Interment charges include opening the grave, handling casket or other container, use of the lowering device (casket burials), greens, turf placement or replacement, reseeding or replacement of any groundcover, removal of grave flowers, and permanent recording of documents. All interments must be completed by 3:00 p.m. Monday thru Friday and by 11:00 a.m. on Saturday. An additional overtime fee of $ will be assessed to any service which extends past 4:00 p.m. Monday thru Friday or any service that extends past 12:00 p. m. on Saturdays. No cremation services past 11:30 p.m. on Saturdays.

2 # $ Effective immediately, Elm Grove Cemetery will need the following for ALL burials: Full Interment: Health Department Burial Permit. Payment for services at time of burial. Cremation: Cremation Affidavit Death Certificate (Photocopy). Payment for services at time of burial. All burials must meet these requirements. All burials must be paid no later than the day of the burial. Casket Containers: All burials in Elm Grove Cemetery (excluding cremation) must have a concrete casket container to preserve the integrity of the cemetery lots and to prevent grave collapse. Elm Grove can provide basic casket containers to Elm Grove Cemetery Association lot owners. All casket containers (i.e. vaults, crypts, grave liners) provided by a funeral home will require an additional $ fee. Elm Grove Cemetery reserves the exclusive right to place vaults and covers from any vault company at any time Elm Grove Cemetery management deems it necessary. IMPORTANT REGULATION All services are under direct control of the Superintendent or Superintendents assistant upon entering the main entrance. This includes funeral directors, clergy, members of the funeral party, vault companies, or any other persons or personnel. Saturday funeral services and cremation services MUST be pre-arranged with the cemetery office by 4:00p.m. on the previous Thursday. Monday funeral and cremation services must be scheduled by 12:00 p.m. on the previous Friday. Grave openings for funeral and cremation services scheduled after 12:00p.m. on the previous Friday will NOT be guaranteed. This also applies to holiday services.

3 THERE WILL BE NO INTERMENTS OR GRAVE OPENINGS ON SUNDAYS, THANKSGIVING, CHRISTMAS DAY, NEW YEARS DAY, MEMORIAL DAY, INDEPENDENCE DAY, OR LABOR DAY. HOLIDAY SURCHARGE APPLIES TO MARTIN LUTHER KING JR. DAY, PRESIDENTS DAY, VERTERANS DAY, and COLUMBUS DAY. % & Grave Location Research- No charge, however requires 48 hour appointment. Elm Grove Cemetery will not provide location or biographical information over the phone or internet. Genealogy : Requires appointment and $25.00 per half hour fee. Records: No charge for basic information but limited only to next of kin to the deceased. Photo copy of available records: $10.00 each Lot ownership determination: Title Search $25.00 Duplicate Deeds: $50.00 Inclusion of cremains in a casket at same time as burial: $ Removal of shrubs: $ Flush Marker resetting: $ Monument searches for inscriptions: $25.00 Additional Perpetual Care: $20.00 per square foot of lot space. 5 PREMIUM LOCATIONS Elm Grove Cemetery has a very limited selection of premium lots located near the scenic Mystic River. The pricing for select locations starts at $20, for a four grave lot which allows for a large monument memorial. Lots are situated on River Avenue and Rose of Sharon Avenue. Please inquire with the Superintendent for availability and details. CREMATED REMAINS SPACE COMING SOON Jasmine and Evergreen Path Located near Elm Groves historic center on Jasmine and Evergreen Path is our newest cremation area with wonderfully picturesque views overlooking the beautiful Mystic River. Graves are available for 1 and 2 cremated remains. Inscription rights on an elegant yet simple shared memorial obelisk. Space for 1 cremated remain $2, Space for 2 cremated remains $2,500.00
