2 Prom 2016 Roll Out the Red Carpet for a Night in the Spotlight Date: Saturday, May 14 th, 2016 Place: The Westin Chicago North Shore 601 N Milwaukee Ave Wheeling, IL Time: 6:00 p.m. to 11:00 p.m. (bus departure from hotel) Menu: Crudités of Fresh Vegetables with Ranch Dip and Hummus Caesar Salad with Parmesan Crisp & Caesar or Balsamic Dressing Warm Rolls with Butter Pretzel Crusted Chicken Breast with Dijon Mustard Sauce Mashed Potatoes Snipped Green Beans Chocolate Mousse with a Chocolate Covered Strawberry After dinner snack: Chips, Salsa, and Guacamole Soda bar provided throughout the night Special Note: Please tell the ticket sellers at the time of purchase if you want a vegetarian meal or if you have any food allergies. Music: Dance Party DJ s Photographer: Icon Video & Photo Booth Company will be in the lobby of the Westin providing complimentary photo booth pictures from about 8:00-11:00 p.m. Photographers for the yearbook will take pictures throughout the evening. 2

The buses will promptly leave at 6:00 p.m. If students don t make the bus, they will not be able to attend Prom. There will be NO refunds. Drinks & Appetizers 6:30-7:15 p.m. Dinner :15-8:30 p.m. Coronation to Follow Dessert about 8:15 p.m. Dancing :30-11:00 p.m. Board Bus for Departure back to LZHS :00 p.m. 3

4 Ticket Sale Information Prom Ticket Cost: $75.00 per person Ticket Sales are in the school cafeteria during the lunch hours from Friday, April 22 nd through Tuesday, April 26 th. ~~ EXCLUSIVE OFFER: Tickets will be on sale on Thursday night April 21 st from 3:30-6:00pm in the cafeteria. This is a great opportunity for your entire group to purchase together and guarantee that you are on the same bus. Please review the following guidelines before purchasing your 2016 Prom tickets: 1. The purchaser must be a current Lake Zurich Junior or Senior. 2. Check or cash is accepted. Please make checks payable to LZHS, and print the student s name on the memo line. We can no longer accept a check written on a business account. Only personal checks, money orders, cashier s checks, or cash will be accepted. Students will be turned away if they come with a business check. 3. You must come prepared with a school ID. If you are bringing a guest who does not attend LZHS, you must borrow or get a copy of the guest s license or state ID and have a completed Visitor Form on file with the Dean s Office. Once the Dean s Office approves your guest, they will give you a Guest Card that you need to purchase the tickets. A copy of the Visitor Form may be found on page Bus reservations must be made upon purchase. Once you have purchased your ticket, you must reserve a seat for your bus. Reservations cannot be switched after they are placed. Busses will seat up to 48 students. 4

5 5. The dining room will have open seating. Once students arrive at the hotel, they will choose their seats. A table layout is included on the next page. Each table seats 12 people. 5

7 Guidelines for the Entire Evening *You must have your LZHS school ID (photo ID for guests) with you to enter Prom!* 1. Check in for Prom begins at 5:00 p.m. The buses leave promptly at 6:00 p.m. You may not attend Prom unless you ride the bus. Prom concludes at 11:00 p.m. 2. Once you enter the check-in, you will not be allowed to leave and reenter. Do not forget your camera or purse in your car! 3. You may not leave Prom at any time. You must return on the same bus you rode on to Prom. 4. NO limousines are allowed to drop-off or pick-up Prom-goers. You will not be allowed to attend Prom if you arrive in a limo. 5. All District 95 rules apply to Prom. 6. Guests who do not attend LZHS are to follow all District 95 Rules and Policies. This includes dates over the age of 18 understanding the rules apply to them while in attendance at Prom. You are responsible for the conduct of your guest. For example, Prom is smoke-free regardless of age. 7. Guests 21 years of age or older are not allowed t0 attend the LZHS Prom. 8. Prom Court family members are welcome to attend the coronation ceremony between 8:00-8:30 p.m. 7

8 9. If you are suspected of being under the influence of or in possession of alcohol and/or other drugs, parents and local authorities will be contacted and you will be asked to leave the dance. A breathalyzer test may be used. If the breathalyzer is refused, it is considered an admission of guilt as stated in the student handbook. You will also be prohibited from participating in any activity related to graduation, including, but not limited to, signing the class wall, attending the senior breakfast, and participating in the graduation ceremony. 10. If you are in possession of or seen using tobacco, you may be asked to leave the dance. You may also lose all graduation privileges as well (see guideline #9). 11. Students should remain in Prom attire for the duration of the evening. No bags or change of clothes are allowed at Prom. 12. All ticket sales are final; there are no refunds or exchanges. 8

9 LAKE ZURICH HIGH SCHOOL DANCE GUEST APPROVAL FORM A student requesting to bring a guest who is not a Lake Zurich High School Student must have this form completed and approved by the Dean s Office. Guests must be at least a freshman in high school and not over the age of 20. A photocopy of the guest s high school ID or state driver license/state ID card MUST be attached to this form. Guests must present a current photo I.D. at the door the night of the dance and have it available at all times, upon request. I further understand that I cannot be in possession of tobacco, alcohol, or drug products while at the dance. Otherwise, I will be dismissed from the dance and may be subject to legal action. My permission is granted to the supervising adult to allow him/her to take all necessary actions should an emergency arise. In case of accident or incident requiring medical attention, the supervisor permission to arranged for medical attention and I accept financial responsibility for the medical attention.
