Under the supervision of a registered health care professional, work independently with the health care team to provide clinical care for a defined caseload of patients. 2. Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals. 3. Ensure patient safety is maintained and patient experience is positive by treating all patients, family, relatives and colleagues with respect, dignity and courtesy in accordance with Trust values Key Dimension a. Dissemination of knowledge through engagement in data collection and other patient administrative documentation and reviewing, taking full responsibility for clinical accuracy and reliability and being sensitive to the wider implications of that dissemination.

3 Specific Core Functions Patient Safety, patient experience and use of resources. Work independently in specific areas as directed by the registered nurse/practitioner once competent. Organise own day to day tasks, exercising judgement and referring to senior staff issues outside own scope of practice. Ensure all possible steps are taken to safeguard the welfare, safety and security of patients, visitors and staff in accordance with Trust policies Promote individualised patient care managing the complex nature of specific needs. Maintain personal hygiene and physical comfort of a complex case mix ensuring that privacy and dignity is maintained at all times. Work in partnership with patients to respect their rights and choices. Assist in the emergency resuscitation of patients as per Portsmouth Hospital Trust Policy. Maintain a clean and safe environment for patients adhering to the Trust infection control policy Assist with patient admissions and discharges. Obtain, record and report information. Demonstrate competent IT skills. Daily access to PAS, APEX, and Outlook. Use of internet for research. Data input for audit. Promote and demonstrate effective communication at all times with patients, carers, colleagues and other wards/departments. Recognise the need for tact, consideration and confidentiality. Report, document and act on untoward incidents appropriately, escalating as indicated. Promote dignity, respect, and comfort whilst acknowledging the diverse needs of patients and their relatives during all aspects of care including end of life care. Obtain and record clinical observations in accordance with NICE guidance report deviations or concerns using the Trusts RSVP escalation model to an appropriate senior member of staff. Requirement to work in an environment with unpleasant working conditions e.g. bodily fluids and unpleasant odours. Assist in maintaining stock levels of consumable items e.g. restocking, ordering. Undertake specific specialist delegated tasks e.g. organise procedure lists, reporting any variance, extended skills as appropriate. Team Working As part of the team working alongside other professionals contribute to the delivery of individualised care, upholding the vision of the service. Undertake specific organisational and administrative duties as required. Assist other clinical areas within the Trust where appropriate as the clinical situation and staffing levels require. Participate in innovation and quality of healthcare by attending ward or departmental meetings, participate in projects, including audit and quality initiatives pertinent to the role within clinical area. Treat all patients, relatives and colleagues with respect in accordance with Trust values and Equality and Diversity Policy Actively engage in good team working to facilitate effective relationships with all health care professionals. 4 Support and supervise students and other support workers in delivery of care Develop skills and knowledge of nursing care and practice relevant to the service Take part in the orientation / induction of new staff. Provide support and development to enable them to develop skills and knowledge required for their role. Undertake mentorship for new HCSWs as appropriate, assisting with the completion of competencies. Act as a link or associate link for a specific area of practice e.g. :- infection control Participate in ongoing personal development by attending essential training. Participate fully in the appraisal and development review process Develop and maintain a professional portfolio for ongoing personal development. OTHER Job Holders are required to: 1.
