1 Using ChemTracker with Life Safety Boxes Overview ChemTracker produces reports used as part of a Life Safety Box (LSB) program intended to quickly provide detailed hazardous materials information to emergency responders in the event of fire, chemical spill, or other potentially catastrophic incidents. Location and Contents of Life Safety Boxes These Life Safety Boxes, located at the main entrances of labs, shops, and/or other rooms storing hazardous materials, contain the following types of information: 1. Generated in ChemTracker: Cover sheet using the DOT hazardous materials warning label system Guide to Hazard Quantities Hazardous Material Inventory 2. Completed by room personnel: Room Map indicating locations of safety-related items Emergency Actions for Hazardous Materials Information providing after hours instructions to emergency responders Life Safety Box Contents Sample Box Cover Sheet ChemTracker: Life Safety Boxes, 1/23/09 page 1

To be readily recognizable by emergency response personnel, the LSB box front covers use the DOT (Department of Transportation) hazardous materials warning label system. An LSB front cover has a sticker applied if the aggregated amount of any particular hazardous material exceeds the LSB sticker threshold listed below. These values can be customized for the organization s instance of ChemTracker. 4 Instructions for Users without Regulatory Reporting Authority Users without Regulatory Reporting Authority can submit a request in ChemTracker version 1 for LSB reports: Log in to version 1 Click Life Safefy Box/Request Report Enter the Building and Room Press the Send Request button. ChemTracker sends an to the local administrator with the name and address of the person making the request and the building and room the request is for Submitting an LSB Report Request Instructions for Users with Regulatory Reporting Authority Users given regulatory reporting authorization in their user profile can run the LSB reports after logging in to ChemTracker version 1. 5 Run the Reports in Version 1 Enter the Building and/or Room and Control Area search criteria Select the: Life Safety Box button and follow the instructions to print the LSB Cover and Guide to Hazard Quantities and Hazardous Material Inventory reports, or Title Pages Only button to print the Cover Sheet To be Completed by Room Personnel Room Map The map can contain the following: Building name, room number, and the date An outline of the room and fixtures including: o Doors o Benches and cabinets o Floor-standing equipment Symbols indicating locations of o Safety equipment o Refrigerators, floor-standing cabinets, fume hoods, gas cylinders and natural gas outlets o Chemical hazards exceeding a corresponding threshold o Other hazards such as Biosafety Level 2 or 3 materials storage, radiological materials storage, radiation-producing machines ChemTracker: Life Safety Boxes, 1/23/09 page 5
